摘    要:目的探讨^99mTc-MIBI静息心肌断导工室腔与心肌计数(C/M)比值和肺摄取(LHR)判断心肌梗塞(MI)患者左心功能状况的临床价值。方法;71例MI患者1周内完成静息心肌显像和平衡法核素心室造影(ERNA),自心肌采集图像的第9顿(前位)原始图像计算出LH挫心肌断层的短轴图像计算出C/M值,并与ERNA的左室射血分数(LVEF)行相关分析。结果:C/M比值与LVEF显著正相关,r值为0.07

Abstract:Objective: To assess the clinical value of cavity-to-myocardial count ratio (C/M ratio) and lung uptake (LHR) duing rest 99mTc-MIBI myocardial SPECT imaging in the evaluation of left ventricular function with myocardial infarction (MI). Methods: 71 patients with MI were investigated with rest 99mTc-MIBI myocardial SPECT imaging and radionuclide ventriculography (RNV) within 1 week. LHR were calculated from the anterior projection of the SPECT sets. C/M ratio was determined on short-axis tomograms and LVEF were calculated from RNV. The correlatin among LVEF and C/M ratio, LHR were analyzed. Results: A positive correlation was observed between C/M ratio and LVEF (r=0.701, p<0.001). There was a negative correlation between LVEF and LHR (r=-0.576, p<0.001). The sensitivity of C/M ratio and LHR for detecting left ventricular dysfunction with MI was 81.1%, 51.4%; the specificity was 79.4%, 91.2%; the accuracy was 80.3%, 70.4% respectively. The sensitivity of C/M ratio was superier to LHR, there was significant difference (p<0.01). The specificity of LHR was superier to C/M ratio, but there was no significant differene (p>0.05). Conclusions: Both C/M ratio and LHR duing rest 99mTc-MIBI myocardial SPECT imaging is useful in the evaluation of left ventricular function with MI. They have their own advantages and limitations and they are complementary.
