中药分阶段结合化疗治疗晚期肺腺癌临床研究

目的观察中药分阶段结合化疗治疗晚期肺腺癌的临床疗效。方法采用前瞻、随机、对照的临床研究方法,将103例患者分为治疗组52例和对照组51例。对照组仅给予单纯化疗;治疗组在化疗期间服用抗瘤增效方,化疗结束后长期服用肺岩宁方。观察两组近期疗效、生存期、不良反应及治疗前后生活质量、主要临床症状、免疫功能、肿瘤标志物变化情况。结果治疗组有效率（CR＋PR）为28.85%,稳定率（CR＋PR＋NC）为82.69%;中位生存期为14.47个月,1、2、3、5年生存率分别为55.77%、25.00%、13.46%、5.77%。对照组有效率为21.57%,稳定率为76.47%;中位生存期为10.97个月,1年、2年生存率分别为43.14%、7.84%,无3年、5年生存病例。治疗组在改善患者生活质量、神疲乏力、气短、腰膝酸软、头晕及免疫功能、肿瘤标志物等方面优于对照组（P〈0.01）;不良反应观察,治疗组出现白细胞减少症、恶心呕吐等消化道反应病例少于对照组（P〈0.05）。结论中药分阶段结合化疗能够延长晚期肺腺癌的生存期,明显改善患者生存质量。

Clinical study on treatment of advanced stages of lung adenocarcinoma by staged herbal administration plus chemotherapy

Objective To observe the clinical effects of combined Chinese medicine and chemotherapy for advanced lung adenocareinoma. Methods 103 subjects were divided into two groups by prospective randomized, controlled clinical research methods： control group in which 51 cases were were treated with chemotherapy a- lone, and treatment group in which 52 cases were treated with ＂ Kangliu Zengxiao Decoction＂ during chemotherapy and ＂ Feiyanning Decoction＂ after chemothera- py. The near-term effect, survival period, adverse reactions, quality of life, main clinical symptoms, immune function and changes in tumor markers before and af- ter treatment were observed in the two groups. Results In the treatment group, the effective rate was 28.85%, stable rate 82.69% ; the median survival time was 14.47 months, 1-, 2-, 3-, 5-year survival rates were 55.77%, 25.00%, 13.46% and 5.77% respectively. In the control group, the effective rate was 21.57%, stable rate 76.47% ; the median survival time was 10.97 months, i- and 2-year survival rates were 43.14% and 7.84% respectively, no cases survived after 3 years. The treatment group had better effects than the control group in quality of life, fatigue, shortness of breath, lumbar and knee limp, dizziness and immune function and tumor markers（ P 〈0.01 ） ; adverse reactions such as leukopenia, nausea, vomiting and other gastrointestinal reaction were fewer in the treatment group than in control group （ P 〈0.05 ）. Conclusion Combined Chinese medicine and chemotherapy is effective to extend survival time and improve quality of life in patients with advanced lung adenocareinoma.
