翁舌汤灌肠治疗大肠湿热型溃疡性结肠炎临床研究

目的:观察翁舌汤灌肠治疗大肠湿热型溃疡性结肠炎的疗效及其对患者血清炎症因子水平的影响。方法:将80例大肠湿热型溃疡性结肠炎患者按随机数字表法分为观察组和对照组各40例。观察组给予翁舌汤灌肠治疗,对照组给予美沙拉嗪栓肛门塞入治疗,两组均连续治疗8周。比较两组患者的临床疗效、肠黏膜征象积分和血清炎症因子水平。结果:治疗后,观察组和对照组总有效率分别为95.00%、70.00%,两组疗效比较,差异有统计学意义(P<0.05)。治疗后,观察组患者炎症增生、黏膜溃疡、黏膜糜烂、黏膜水肿、黏膜充血积分均下降,与治疗前比较,差异有统计学意义(P<0.05);且观察组各项积分均低于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清白细胞介素-8(IL-8)、肿瘤坏死因子-β(TNF-β)水平均降低,血清白细胞介素-10(IL-10)水平均升高,与治疗前比较,差异均有统计学意义(P<0.05);且观察组血清IL-8、TNF-β水平明显低于对照组,血清IL-10水平明显高于对照组,差异有统计学意义(P<0.05)。结论:翁舌汤灌肠治疗大肠湿热型溃疡性结肠炎疗效较好,可改善患者肠黏膜征象积分,调节血清炎症因子水平。

Clinical Study on Enema with Wengshe Decoction(翁蛇汤)in Treatment of Ulcerative Colitis of Large Intestine Dampness-Heat Syndrome

Objective:To observe the curative efficacy of enema with Wengshe Decoction(翁蛇汤)in the treatment of ulcerative colitis of large intestine dampness-heat syndrome and its influence on inflammatory factors in serum of the patients.Methods:A total of 80 patients with ulcerative colitis of large intestine dampness-heat syndrome were divided into observation group and control group according to the random number table method,with 40 cases in each group.The observation group was given enema with Wengshe Decoction,and the control group was given anal insertion of mesalazine suppository.Both groups were treated continuously for 8 weeks.The clinical curative efficacy,intestinal mucosal symptom scores and levels of serum inflammatory factors were compared between the two groups.Results:After treatment,the total effective rate of the observation group was 95.00% and that of the control group was 70.00%.The difference of curative efficacy between the two groups was statistically significant(P<0.05).After treatment,the scores of inflammatory hyperplasia,mucosal ulcer,mucosal erosion,mucosal edema,and mucosal hyperemia were decreased in the observation group as compared with those before treatment(P<0.05),and the above scores in the observation group were lower than those in the control group(P<0.05).After treatment,both the two groups showed lower levels of IL-8 and TNF-β but higher level of IL-10 than before treatment(P<0.05).The levels of IL-8 and TNF-β in the observation group were lower than those in the control group,and the level of IL-10 in the observation group was higher than that in the control group.The differences between the two groups were statistically significant(P<0.05).Conclusions:Enema with Wengshe Decoction has good curative efficacy in the treatment of ulcerative colitis of large intestine dampness-heat syndrome.It can improve the patients’intestinal mucosal symptom scores and regulate levels of serum inflammatory factors.
