新鲜羊膜移植治疗早期重症眼部烧伤

目的 探讨新鲜羊膜移植联合结膜、角膜移植对早期重症眼部烧伤的治疗效果。方法 急性重症眼部烧伤患者21例22眼，分别行羊膜移植联合结膜移植12例12眼和羊膜、结膜移植联合板层或穿透性角膜移植9例10眼。术后观察羊膜、结膜及角膜植片情况，长后随访2～16个月，平均7个月。结果 19眼羊膜植片愈合良好；20眼穹隆部结膜植片全部存活；羊膜移植联合结膜移植中10眼术后未见角膜融解，角膜缘均未见新生血管长入。羊膜、结膜移植联合板层或穿透性角膜移植，1例2眼出现轻度排斥反应，另8眼角膜植片均恢复透明。结论 新鲜羊膜移植可有效地促进上皮细胞分化移行、抑制局部的炎症反应、阻止新生血管和瘢痕形成、阻止烧伤的病理发展减轻和推迟角膜植片的排斥反应。

Fresh amniotic membrane transplantation for early severe ocular burns

Objective To investigate the therapeutic effect of fresh amniotic membrane on conjunctival and corneal transplantation in early severe ocular burn . Methods Twenty-one cases (22 eyes) with severe ocular burns have been studied . Amniotic membrane combined with conjunctival transplantation 12 cases (12 eyes ) , or amniotic membrane combined with conjunctival and corneal transplantation 9 cases (10 eyes ) was performed. They were followed up for 2 ~ 16 months after operation(mean 7 months ) . Results It had been found that the grafts of amniotic membrane ( 19 eyes / 22 eyes ) and conjunctiva (20 eyes ) were healed up . No progressive melt and perforation of cornea was found in 10 eyes after amniotic membrane and conjunctiva transplantation and no neovascularization also. After amniotic membrane combined with conjunctiva and corneal transplantation (10 eyes ) , light degree graft rejection occurred in 1 case ( 2 eyes ) , 8 eyes corneal graft transparency resumed , edge of corneal graft had light neovascularization. Conclusion Fresh amniotic membrane can effectively advance the polarization and migration of epithelium cell , reduce local inflammation reaction, neovascularization and fibrosis , stop the burned pathologic process , relief the graft rejection .
