临床路径在腹腔镜手术治疗卵巢良性肿瘤中的实施效果评价

目的评价临床路径在腹腔镜手术治疗卵巢良性肿瘤中的实施效果，并分析其应用价值。方法根据纳排标准，将实施临床路径和一般诊疗流程的患者分别纳入临床路径组和非临床路径组，收集分析两组患者基本情况、住院医疗费用、平均住院天数和日均住院医疗费等数据，并分析临床路径组的变异情况。结果临床路径组的住院费用低于非临床路径组，其中药费、化验费、检查费和床位费明显降低，手术费、治疗费和护理费无明显差异；临床路径组的平均住院天数小于非，临床路径组；两组的日均住院医疗费无明显差异；临床路径组负性变异发生率为19．60％。结论腹腔镜手术治疗卵巢良性肿瘤的临床路径对于降低住院费用、缩短住院时间具有显著作用，同时并不影响医院单个床位的日均收入。

Evaluation of the Clinical Pathway on Laparoscopic Surgery for Benign Ovarian Tumor

Objective To evaluate the effect of the clinical pathway for laparoscopic surgery in treatment of benign ovarian tumor. Method According to the inclusion and exclusion criteria, patients were selected into clinical pathway group or control group based on their treatment procedure. The data of hospitalization expenses, average hospitalization days and daily hospitalization expenses were analyzed. Result The hospitalization expenses of clinical pathway group were significantly less than that of the control group, especially in the expenses for drugs, tests, inspections and sickbeds. However, no difference was found on operation cost, therapy and nursing expense. The average hospitalization days of clinical pathway group were also less than that of the control group. There was no statistical difference on the daily hospitalization expenses. The incidence of negative variance was 19.60% in the clinical pathway group. Conclusion The clinical pathway on laparoscopic surgery for benign ovarian tumor could obviously re- duce hospitalization expenses, shorten average hospitalization days. However, it didnt ＇affect daily mean income of single bed.
