早期切痂植皮治疗深度手烧伤的临床研究

目的为解决手部深度烧伤的难题,探讨早期切痂植皮治疗的临床效果。方法总结性回顾2010年5月—2012年5月在该院施行切痂植皮手术手部深度烧伤患者100例,分析临床资料,将其随机分为早期切痂植皮组和延迟切痂植皮组,早期切痂植皮组50例患者,延迟切痂植皮组50例患者,观察两组恢复的效果,针对患者术后出现功能障碍感染、创面愈合时间、植皮成活率不同进行比较,得出结论。结果手术后,早期切痂植皮组患者功能障碍感染出现例数6例,感染率12%,创面愈合时间为(12±5.5)d,植皮成活例数为49例,成活率为98.%,这三项数据均优于延迟组,差异有统计学意义(P<0.05)。结论对手部深度烧伤患者实施早期切痂植皮手术治疗,可减少瘢痕增生,可有效促进手部功能的恢复,保护手功能,重视按摩、理疗等综合性康复治疗,减少并发症,缩短创面愈合时间和住院时间,对患者早期康复有一定的影响。

Early escharectomy and skin grafting in treatment of deep hand burn in clinical research

Objective In order to solve the problem of hand deep burn wounds of early escharectomy and skin grafting, clinical curative effect. Methods The Summary Review 2010 --2012 May in our hospital implementation of eseharectomy and skin graft- ing operation of hand deep burn wounds in 100 cases, analysis of clinical data, were randomly divided into early eseharectomy group and delayed escharectomy group, early escharectomy group 50 patients, delayed escharectomy and skin grafting in 50 cases, two groups were observed in patients with postoperative recovery effect dysfunction infection, wound healing time, the survival rate of skin graft with different comparison, conclusion. Results After the operation, the early escharectomy group of patients with dys- function infection cases a number of 6 cases, the infection rate was 12%, the wound healing time was （12±5.5）d, skin graft sur- vival cases for 49 patients, the survival rate was 98%, the three data are better than the delay group, P〈0.05, the difference was statistically significant. Conclusion The opponent of deep burn patients with early escharectomy and skin grafting operation treat- ment, can reduce the scar hyperplasia, which can effectively promote the functional recovery of hand, preservation of hand func- tion, pay attention to massage, physiotherapy and other comprehensive rehabilitation therapy, reduce complications, shorten wound healing time and length of hospital stay, on the early rehabilitation of patients have a certain effect.
