鼻咽癌组织中XIAP的表达与放疗预后的相关性

摘    要:目的:观察放疗后鼻咽癌(NPC)组织中X染色体连锁的凋亡抑制蛋白(XIAP)的表达,评估XIAP在NPC患者放疗预后的诊断价值。方法:回顾性分析2010年1月至2012年12月收治的94例NPC患者的临床及病理资料,所有患者均接受放疗。采用免疫组织化学法检测患者鼻咽部病理标本中XIAP蛋白表达,临床随访并分析其与放疗预后关系。结果:XIAP在NPC组织中总的阳性表达率为69.15%(65/94)。XIAP阳性表达率与p-TNM分期和肿瘤分化呈负相关(均P<0.05)。单因素生存分析表明,XIAP高表达、高p-TNM分期以及肿瘤低分化程度均与NPC患者存活率下降明显相关(均P<0.05)。多因素Cox回归分析显示XIAP表达、p-TNM分期以及肿瘤分化程度是影响NPC患者放疗生存的独立危险因素。结论:XIAP在放疗的NPC组织中高表达,XIAP表达、p-TNM分期以及肿瘤分化程度是影响NPC患者放疗生存的独立危险因素。

Abstract:Objective: To investigate the relationship between X-linked inhibitor of apoptosis protein (XIAP) expression and prognosis of patients with nasopharyngeal cancer (NPC) that received radiotherapy. Methods: A retrospective analysis was performed with the data of 94 NPC patients from January 2010 to December 2012. The expression of XIAP in NPC tissues was detected with immunohistochemistry (IHC), and analyzed the prognosis relevance after radiotherapy. Results: The overall XIAP positive expression rate in NPC tissues was 69.15% (65/94) and the XIAP positive expression rate was negatively correlated with the p-TNM staging and the tumor differentiation (all P<0.05). Kaplan-Meier analysis showed that the high XIAP expression, high p-TNM staging and poorly differentiated were significantly correlated with decreased survival rate of NPC patients (all P<0.05). Multivariate Cox regression analysis showed that XIAP expression, p-TNM staging and degrees of differentiation were independent risk factors for the survival of NPC patients after radiotherapy. Conclusion: The high XIAP expression, p-TNM staging and degree of differentiation are independent risk factors for the survival of NPC patients after radiotherapy.
