BiPAP呼吸机治疗慢性阻塞性肺疾病合并呼吸衰竭32例疗效观察

目的探讨BiPAP无创正压通气治疗慢性阻塞性肺疾病合并呼吸衰竭的疗效。方法64例慢性阻塞性肺疾病并呼吸衰竭患者随机分为治疗组和对照组各32例，治疗组除常规治疗外加用BiPAP呼吸机辅助通气治疗，对照组给予抗感染，通畅气道，持续低流量吸氧，改善通气等常规治疗。结果治疗组总有效率为93．8％，对照组总有效率为68．8％，两组疗效比较有显著性差异（P〈0．01）；治疗前后血气分析表明，治疗组的PaO2明显增加，PaCO2,心率和呼吸频率均显著降低（P〈0．01），而对照组相关指标改善不明显。结论BiPAP呼吸机辅助通气治疗慢性阻塞性肺病合并呼吸衰竭疗效好，值得临床推广。

Observation of the Effects of BiPAP Ventilation in Treating 32 patients of COPD with Respiratory Failure

Objective To analyze the effects of BiPAP ventilation in treating COPD with respiratory failure. Methods 64 patients of COPD with respiratory failure were randomly equally divided into two groups, BiPAP ventilation together with regular therapy group（treatment group ） and regular therapy group （ control group ）, the patients＇ condition, blood and vital energy parameter were recorded. Results The treatment group＇s total effectiveness is 93.8% ,and the control group＇s total effectiveness is 68.8% ,the effectiveness between the two group has the significant difference（P〈 0.01 ）; In the treatment group when ventilated for 1 cycle, PaCO2 increased and heart rates, respiratory rates and PaO2 decreased（P〈 0.01 ）. Conclusion BiPAP ventilation can be used as the better choice for the treatment of COPD with respiratory failure.
