2型糖尿病合并冠心病患者血清脂联素水平测定及临床意义

摘    要:目的探讨2型糖尿病(DM)合并冠心病(CHD)患者血清脂联素水平与体重指数(BMI)、腰臀比(WHR)、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、胰岛素抵抗指数(Homa-IR)等相关因素的关系。方法用放射免疫法检测单纯2型DM组、2型DM合并CHD组及正常对照组的脂联素和胰岛素,用日立全自动生化分析仪测定FBG、血脂,测量所有观察对象的身高、体重、腰围和臀围并测定其尿白蛋白排泄率。结果单纯2型DM组、2型DM合并CHD组血清脂联素水平均低于正常对照组(P〈0.01);2型DM合并CHD组显著低于单纯2型DM组(P〈0.01)。2型DM2组Homa-IR均高于正常对照组。脂联素与WHR、Homa—IR呈独立负相关(P〈0.01)。结论2型DM合并CHD患者血清脂联素水平与胰岛素抵抗之间存在密切的相关性,血清脂联素在2型DM及合并CHD的发生发展中起着重要作用。

Abstract:Objective To investgate the relationship between serum adiponectin concentration and their relevant indexes such as body mass index(BMI) ,waist hip ratio(WHR) ,fasting blood glucose( FBG), glycosylated hemoglobin (HbA1c), for clarifying the effect and mechanism of adiponectin in the ocourrence and development of type 2 diabetes mellitus(DM) with coronary heart disease (CHD). Methods The levels of adiponectin and fasting insulin(FINS) in patient with type 2 DM, type 2 DM with CHD and normal control were measured by radioimmunoassay ( RIA), FBG were measured by automatic biochemistry analyzer, and their body height, weight and waite and hip were measured for BMI and WHR. Results The levels of serum adiponectin in the two type 2 DM groups were lower than that in the normal control group (P 〈0.01 ). The level of adiponectin in the group of type 2 DM with CHD was lower than that in the group of type 2 DM ( P 〈 0.05 ). The Insulin resistance (Homa-IR) in the two type 2 DM groups was higher than in normal control group ( P 〈 0.01 ), the adiponectin were negatively correlated with BMI and WHR ( P 〈 0.01 ). Conclusions Adiponectin plays an important part in the occurrence and developent of type 2 and type 2 DM with CHD.
