268例妇女带器前宫腔测量结果分析

摘    要:对选用Multiload Cu 250宫内节育器避孕的268例妇女于置器前作宫腔测量。置器后两年随访中发现脱落移位者27例,与268例总平均值相比,其有效宫腔长度(Ca.L)较长(P<0.01),宫底横径(T_1)及相继下降1cm处的子宫第2、3、4横径(T_2,T_3、T_4)较宽(P<0.05),而有效宫颈长度(Ce.L)较短(P<0.05)。其中9例曾有脱环史,而这次再次发生脱落或下移者,其以上特点更明显。有脱环史的49例与续用组的224例宫测值比较,差异有显著性意义,均说明宫腔长而宽、宫颈相对短的大子宫易造成IUD下移甚至脱落。

Abstract:Uterine cavity measurements were performed prior to Multiload Cu250 IUDs insertion in 268 volunteers. Expulsion and downward displacement took place in 27 cases during 2 years of clinical follow-up after insertion. In these cases the mean effective uterine cavity length (Ca.L) is longer (P<0.01), the mean foundus transversal diameter (T1) and other transversals of the uterine cavity (T2, T3, T4) are wider (P<0.01-0.05) and the effective cervical length (Ce.L) is shorter(P<0.05) than the total averages for 268 women. The above mentioned anatomic characteristics of the uterine cavities are more pronounced and remarkable in 9 cases with a history of expulsion. There is also a statistically significant difference between the 49 cases with an expulsion history and the 224 cases with IUDs in situ our results demonstrate that in cases of expulsion or downward displacement the uterine cavities are generally longer and wider, while the cervical lengths are shorter. Wider uterine transversal diameters in conjunction with relatively narrow IUDs might be one of the reasons for expulsion or displacement.
