白蛋白输注对重症胰腺炎伴ARDS患者术中影响的研究

目的 观察、比较不同成份液体输注在重症胰腺炎伴ARDS患者术中的应用效果.方法 选取该院行胰腺坏死组织清除术的重症胰腺炎伴ARDS患者60例,随机分为对照组与观察组各30例,对照组术中予林格液输注,观察组予白蛋白输注,比较2组患者输注前后气道峰压、中心静脉压、呼吸参数、胸肺顺应性及多巴胺用量.结果 观察组输注后的气道峰压、中心静脉压、呼吸参数、胸肺顺应性均优于对照组,多巴胺用量低于对照组,2组比较差异有统计学意义（P＜0.05）.结论 白蛋白输注对重症胰腺炎伴ARDS患者术中的效果较佳,对心肺功能的作用更为积极,故价值更高.

Application effect study of albumin infusion in patients with severe pancreatitis complicated with ARDS during the operation

Objective To observe and compare the application effect of different infusion liquid components in pa- tients with severe pancreatitis complicated with ARDS during the operation. Methods 60 cases of patients with severe pancre- atitis and ARDS and underwent debridement of necrotic pancreatic tissue in our hospital were selected as research object, and they were randomly divided into control group（ Ringer＇s solution group）and observation group（ albumin group）, each of 30 ca- ses. Then the peak airway pressure,central venous pressure,respiratory parameters,chest-lung compliance before and after the infusion and dopamine amount of 2 groups were compared. Results The peak airway pressure, central venous pressure, respir- atory parameters, chest-lung compliance of the observation group after the infusion were all better than those of the control group, and dopamine amount was less than that of control group, the differences were statistically significant （ P 〈 0.05 ）. Con- cluslon The application effect of albumin in patients with severe pancreatitis complicated with ARDS during the operation is better and plays a more active role for the cardio-pulmonary function, so its value is relatively higher.
