腹主动脉灌注丙泊酚减轻脊髓缺血-再灌注损伤与抗氧化和减轻钙超载的关系

摘    要:目的 本研究拟探讨腹主动脉灌注丙泊酚减轻脊髓缺血-再灌注损伤是否与其抗氧化和减轻钙超载相关.方法 新西兰大耳白兔60只,随机均分为生理盐水(NS)组、10%脂肪乳(P0)组、丙泊酚30mg/kg(P30)组、40mg/kg(P40)组、50mg/kg(P50)组和60mg/kg(P60)组.全麻后开腹阻断肾动脉远端腹主动脉及双侧髂总动脉30min,并从阻断即刻开始,经股动脉预置导管向腹主动脉分别灌注生理盐水、10%脂肪乳及不同剂量的丙泊酚溶液至开放停止.再灌注48h,取L5脊髓节段,检测脊髓组织的超氧化物歧化酶(SOD)活性、丙二醛(MDA)含量和脊髓前角神经细胞内Ca2 荧光强度.结果 与NS和P0组比较,P30、P40、P50和P60组的SOD活性显著升高(P<0.05),MDA含量和Ca2 荧光强度显著降低(P<0.05).SOD活性:P50组最高(P<0.05),P40、P60组高于P30组(P<0.05).MDA含量:P50组最低(P<0.05),P60组低于P30、P40组(P<0.05).Ca2 荧光强度:P60组低于P50组,P50组低于P30、P40组(P<0.05).结论 丙泊酚可剂量依赖性地保护脊髓组织SOD活性、降低MDA含量和脊髓前角神经细胞内Ca2 荧光强度,提示该保护作用与其抗氧化和减轻神经细胞钙超载有关.

Abstract:Objective To investigate whether propofol infusion through aorta relieving spinal cord ischemia-reperfusion injury(IRI) is owing to its properties of antioxidantion and attenuating Ca2 overload.Methods Sixty New Zealand white rabbits were assigned randomly to 6 groups of normal saline(group NS),10% intralipid(group P0),propofol 30 mg/kg(group P30),propofol 40 mg/kg(group P40),propofol 50 mg/kg(group P50) and propofol 60 mg/kg(group P60) with 10 rabbits each.Spinal cord ischemia was induced by clamping the infrarenal aorta and common iliac arteries for 30 min under general anesthesia.During occlusion,normal saline,10% intralipid or different doses of propofol were infused via a catheter in the clampted aorta segment.At 48 h after reperfusion,the L5 spinal cord were harvested for the measurement of SOD activity,MDA production and Ca2 fluorescence density of the neurons in the anterior horn.Results Compared with groups NS and P0,SOD activities were significantly higher in groups P30,P40,P50 and P60(P<0.05) with the highest in P50 group(P<0.05).MDA productions were significantly less in the 4 propofol groups(P<0.05)(the least in group P50) and the Ca2 fluorescence density was also less in the 4 propofol groups.There was no significant difference between groups NS and P0.Conclusion Propofol can relieve IRI of the spinal cord dose-dependently,suggesting that the protection is related to the properties of antioxidantion and attenuating the endocellular Ca2 overload of propofol.
