基因芯片检测乙肝病毒YMDD变异及其临床意义

目的 检测拉米呋啶治疗慢性乙肝病人YMDD变异情况与血清HBV DNA、ALT水平等指标的关系。方法 利用基因芯片、ELISA及生化检测技术检测180例慢性乙肝患者服用拉米呋啶6～24个月后YMDD变异、HBV DNA及ALT水平变化。结果 180例患者中，168例HBV DNA、ALT不同程度下降，随用药时间的延长，部分患者（2g例）的YMDD出现变异，同时HBV DNA及ALT水平出现反跳。结论服用拉米呋啶的慢性乙肝患者的YMDD发生率与时间长短有关，血清HBV DNA及ALT与YMDD相关，由此可以预测拉米呋啶治疗的乙肝病人的YMDD发生率。

Study on YMDD Mutation and Clinical Significance of Chronic Hepatitis B Using Gene Chip

Objective To investigate YMDD mutation and its relationship to the level of serum HBV DNA and ALT in chronic hepatitis B patients treated with lamivudine.Methods YMDD mutation, HBV DNA and ALT of 180 chronic hepatitis B patients were determined with gene chip, ELISA and biochemistrical methods alter they were treated lamivudine, Results HBV DNA and ALT level of 168 cases were decreased in 180 cases, but with the time going on, HBV DNA and ALT of 28 cases were increased, the YMDD mutation were occurred in 28 cases.Conclusion Serum ALT and HBV DNA level may increase with YMDD mutation .It may be helpful predict the emergence of YMDD mutation to monitor closely the patients on lamivudine therapy.
