早期无创正压通气治疗急性左心衰竭的临床研究

目的：观察早期无创正压通气治疗急性左心衰竭的疗效。方法：选择16例经常规治疗方法无效伴有低氧血症的急性左心衰竭患早期进行无创正压通气，并监测治疗前后临床症状、平均动脉压、心率、呼吸和血气分析(PH，PaO2，PaCO2,SaO2)。结果:13例患治疗有效，3例患治疗失败，需要有创通气，有效率81．2％。治疗后，患临床症状改善，生命体征稳定，氧分压和氧饱和度明显提高，结论:早期无创正压通气是治疗急性左心衰竭合并低氧血症的有效方法之一，可显降低对气管插管的需求。

Clinical study on noninvasive positive pressure ventilation for treatment of acute left heart failure

Objective To observe early noninvasive positive pressure ventilation for the treatment of acute left heart failure. Methods 16 patients with hypoxemia due to acute left heart failure with non-reaction to the normal treatment received noninvasive posive pressure ventilation and the clinical symptoms, mean artery pressure(MAP), heart rate(HR), respiratory rate(RR) and blood gas analysis (PH, PaO 2, PaCO 2, SaO 2) were observed before and after treatment. Results 13 patients were treated effectively and 3 patients needed invasive ventilation, the effective rate is 81.2%. After the treatment, clinical symptoms of the patients were improved, the vital signs were stable and PaO 2, SaO 2 went up. Conclusion Early noninvasive potisive pressure ventilation is one of the effective methods for the treatment of acute left heart failure with hypoxemia,it will reduce the need of endotracheal intubation.
