血清铁蛋白与小儿脓毒症预后相关性的临床研究

目的探讨血清铁蛋白水平与小儿脓毒症预后的相关性。方法将30例脓毒症患儿按治疗转归分为存活组及死亡组,记录并分析治疗前后血清铁蛋白水平的变化。结果治疗前,存活组患者的血清铁蛋白的含量为（3572.7±3353.8）ng/ml,治疗后为（879.8±827.9）ng/ml,组内治疗前后比较,差异有统计学意义（P=0.0013）,治疗后血清铁蛋白明显降低;治疗前,死亡组患者的血清铁蛋白的含量为（2248.9±1556.1）ng/ml,治疗后为（8148.9±1893.4）ng/ml,组内治疗前后比较,差异有统计学意义（P=0.0001）,治疗后血清铁蛋白明显升高。两组治疗后的血清铁蛋白含量比较,差异有统计学意义（P=0.0000）。结论血清铁蛋白监测水平高值提示脓毒症病情恶化及预后不良,为小儿脓毒症的预防与治疗提供有效的参考,值得临床推广。

Clinical study of relevance of serum ferritin and prognosisin of sepsis in children

Objective To investigate the relevance of serum ferritin and prognosis in children with sepsis. Methods 30 patients with sepsis were divided into the survival group and the death group according to treatment outcome,and change of serum ferritin were recorded and analyzed before and after treatment. Results Before treatment,the content of serum ferritin of patients in the survival group was （3572.7±3353.8） ng/ml,after treatment,the content of serum ferritin was （879.8±827.9）ng/ml, and there was a statistical difference of serum ferritin before and after treatment in the sur- vival group （P=0.0013） and after treatment,serum ferritin decreased significantly.Before treatment,the content of serum ferritin of patients in the death group was （2248.9±1556.1） ng/ml,after treatment,the content of serum ferritin was （8148.9±1893.4） ng/ml,and there was a statistical difference of serum ferritin before and after treatment in the death group （P=O.0001） and after treatment,serum ferritin increased significantly.There was a statistical difference of serum ferritin in the two groups （P=0.0000）. ConcLusion The high value of serum ferritin suggests patients＇ condition deterio- ration and poor prognosis of sepsis and it can provide effective reference for the prevention and treatment of sepsis and is worthy of clinical promotion.
