左向右分流伴肺动脉高压先天性心脏病患儿内皮素-1和一氧化氮检测临床意义

摘    要:目的探讨左向右分流伴肺动脉高压(PH)先天性心脏病(CHD)患儿内皮素-1(ET-1)和一氧化氮检测的临床意义。方法选择有左向右分流伴肺动脉高压的患儿42例(患者组);左向右分流无肺动脉高压的患儿26例(对照组);健康体检儿童20例(健康组),应用放射免疫法和硝酸还原法检测样品中ET-1和NO2 /NO3 含量。结果患者组ET-1含量明显高于对照组(P<0.001)和健康组(P<0.001),对照组和健康组无明显差异(P>0.05);NO2 /NO3 含量明显低于健康组(P<0.001)和对照组(P<0.001),对照组明显高于健康组(P<0.001)。结论具有左向右分流伴肺动脉高压的CHD患儿血中ET-1含量明显增高和NO2 /NO3 含量明显减低,两者的测定对治疗和疗效观察具有良好的指导意义。

Abstract:Objective To explore the cinical significance of Plasma Endothelin-1(ET-1) and Nitrous Oxide(NO) in the children associated with left to right shunt congenital heart defect(CHD) with Pulmonary Hypertension(PH).Methods 42 children associated with left to right shunt congenital heart defect with pulmonary hypertension(as patient's group),26 children associated with left to right shunt congenital heart defect without pulmonary hypertension(as heathly group),20 children had the checkup(as control group).Plasma ET-1 and NO2 /NO3 was cletected in all groups by radioimmunoassay and nitric acid-restored method.Results ET-1 of patient's group was significantly higher than that of control group(P<0.001) and heathly group(P<0.001).Contral and heathly group was no significantly diversity(P>0.05);NO2 /NO3 of that was significantly lower control group(P<0.01) and heathly group(P<0.001);but control group was significantly higher than heathly group(P<0.01).Conclusions ET-1 was noticeable higher and NO2 /NO3 was lower in blood that children of CHD which had left to right shunt with PH.TO detemine ET-1 and NO2 /NO3 was a good direction which CHD was cured and the curative effect was observed.
