成熟避孕方法综合推广对川渝农村育龄妇女计划生育满意度的影响

目的:了解成熟避孕方法综合推广研究项目对农村育龄妇女计划生育服务过程和现用避孕方法满意度的影响,评价干预效果。方法:2002年12月在重庆市和四川省分别选择一个经济和文化处于当地中等水平的县,分别针对服务对象、服务提供者制定综合干预方案,并逐步落实,分析比较基线、中期、终末评估时服务对象对服务过程和避孕节育方法使用的满意情况。结果:基线、中期、终末三期育龄妇女对计划生育服务过程评价为"较好"的比例(4.4%、88.6%、95.8%)和对现用避孕节育方法的满意度(52.5%、67.4%、95.4%)均呈递增趋势。经多元Logistic回归调控地区、年龄、职业、孕产史和避孕方法等潜在混杂因素后,中期和终末评估时服务对象对服务过程评价为"较好"的比例(OR中期=2.67,95%CI=1.39-5.14;OR中期=7.77,95%CI=5.34-11.31)以及对现用避孕节育方法满意度干预后均显著增高(OR中期=1.87,95%CI=1.18-2.96;OR中期=18.81,95%CI=10.91-32.43)。结论:综合干预措施能有效提高农村育龄妇女对计划生育服务过程和现用避孕方法的满意程度,推动计划生育服务的改善。

Effects of Integrative Extension of Contraceptive Methods on the Satisfaction of Family Planning among Rural Reproductive Aged Women

Objective:To evaluate the effects of intervention measures on the satisfaction of current contraceptive methods and family planning service process among rural reproductive aged women.Methods:Two counties at the middle-level of economical and social development in Chongqing Municipality and Sichuan Province were selected for this study.Integrative intervention strategies were established and implemented,aiming at clients and service providers.Baseline,mid-term and final evaluations were conducted to comprehend the satisfaction of service process and the usage of contraceptive methods.And the results of the baseline,mid-term and final evaluations were analyzed and compared.Results:The proportions of reproductive aged women satisfied with family planning services were 74.4%,88.6% and 95.8% in baseline,mid-term and final investigations,respectively.And the proportions of women satisfied with current contraceptive methods were 52.5%,67.4%,and 95.4%,respectively.After adjustment for potential confounders including region,age,occupation and pregnancy history by using multivariate logistic regression analysis,the results showed that,compared with the baseline evaluation results,the proportions of women satisfied with family planning services(mid-term evaluation:OR=2.67,95%CI:1.39-5.14;final evaluation:OR=7.77,95%CI:5.34-11.31)and current contraceptive methods(mid-term evaluation:OR=1.87,95%CI:1.18-2.96;final evaluation:OR=18.81,95%CI:10.91-32.43)increased significantly after intervention.Conclusion:The integrative intervention measures can effectively promote the satisfaction of family planning service process and current contraceptive methods among rural reproductive aged women and improve the quality of family planning services.
