乌司他丁对兔心肌缺血再灌注损伤pHi的影响

目的:观察兔心肌缺血再灌注时胃粘膜pH值(pHi)的变化,探讨乌司他丁对心肌缺血再灌注兔pHi的影响。方法:20只成年雄性新西兰大白兔随机分成乌司他丁组(n=10)及对照组(n=10)。乌司他丁组在制模前30min静脉给予乌司他丁50000U/kg,对照组给予等量生理盐水。开胸结扎兔冠状动脉左室支30min,再灌注后1h、2h分别记录心电图,左室内收缩压(LVSP)、左室收缩压上升最大速率(+dp/dtmax)、心率和pHi值。结果:乌司他丁明显降低心电图S-T段的升高程度并改善血流动力学变化。pHi在缺血再灌注后显著降低,但乌司他丁组pHi降低幅度在再灌注后1h、2h明显小于对照组(P<0.05)。结论:在兔缺血再灌注前,静脉输入乌司他丁可以明显改善胃肠道微循环灌注和组织氧合。

Effect of ulinastin on the gastric intramucosal pH after myocardial ischemic reperfusion injury in rabbits

Objective:To investigate the changes in gastric intramucosal pH (pHi) and the effect of ulinastin on pHi after myocardial ischemia-reperfusion injury in rabbits. Methods:Twenty adult Newzeland white rabbits were randomly divided into two groups: ulinastin group (n=10) and control group (n=10). ulinastin group received the infusion of ulinastin 50 000 U/kg at 30min before modeling,control group received the routine treatment infusion of saline instead of ulinastin. The myocardial ischemic reperfusion model (I/R) was established ,ECG and homodynamic index were monitored simultaneously. Hemodynamics index and pHi were measured before myocardial ischemia and 30 min after myocardial ischemia and 1h、2h after reperfusion. Results:Ulinastin can lower ST segment and improve index of hemodynamics. pHi decreased significantly after myocardial ischemia-reperfusion injury in both groups. Compared with control group , pHi was significantly higher at 1h、2h after reperfusion in ulinastin group (P<0.05). Conclusion:pHi decreases significantly after myocardial ischemia-reperfusion injury in rabbits. Ulinastin infusion can improve gastro-intestinal micro-circulatory perfusion.
