| 摘 要： | 目的:探讨治疗高龄股骨颈骨折如何选择全髋关节置换(THA)或双动股骨头置换(BFHR),并分析使用小切口技术的优势和操作技巧。方法:高龄股骨颈骨折行髋关节置换43例,对伤前能快步或大步行走的25例行THA,对伤前持杖或小碎步行走的18例行BFHR。使用髋后外侧小切口入路,THA切口7～9cm,BFHR切口5～6cm。强调术后早期活动。结果:全部病人于术后3周持拐下地。随访时间13～26个月(平均18个月),患者关节功能良好,无疼痛,X线片假体位置正常,无松动及下沉,THA的Harris评分为91.6,BFHR的Harris评分为90.4。结论:对股骨颈骨折的高龄患者,如伤前能快步或大步行走者选择THA,反之则选择BFHR。推荐使用小切口技术进行手术。
