降酐合剂保留灌肠治疗毒邪蕴结型肾功能不全

目的:观察降酐合剂保留灌肠治疗毒邪蕴结型肾功能不全患者的临床疗效。方法:138例慢性肾脏病患者(肾功能不全3-5期)随机平均分为对照组(基础治疗)、活性炭组(基础治疗+药用炭片爱西特保留灌肠)、降酐合剂组(基础治疗+降酐合剂保留灌肠),每组均治疗1个月。比较各组患者治疗前后肾功能肌酐(creatinine,CRE)、血清尿素氮(blood urea nitrogen,BUN)、尿酸(uric acid,UA)、胱抑素C(cystatin C,Cys C)]、炎症因子单核细胞趋化蛋白-1(monocyte chemoattractant protein-1,MCP-1)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)]的水平。结果:治疗后,活性炭组、降酐合剂组的CRE、BUN、UA、Cys C水平均低于对照组(P<0.01),降酐合剂组以上指标水平均低于活性炭组(P<0.01)。治疗后,活性炭组、降酐合剂组的MCP-1、TNF-α、IL-6水平均低于对照组(P<0.05),降酐合剂组以上指标均低于活性炭组(P<0.05)。结论:降酐合剂灌肠治疗毒邪蕴结型肾功能不全患者临床疗效显著,能有效改善患者肾功能、减轻患者炎症反应。

Retention Enema with Jiangan Mixture in the Treatment of Renal Insufficiency with Toxin Accumulation

Objective:To observe the clinical effect of Jiangan Mixture retention enema in the treatment of renal insufficiency of turbid-toxin accumulation syndrome.Methods:138 patients with chronic kidney disease(renal insufficiency stage 3-5)were randomly divided into control group(basic treatment),activated carbon group(basic treatment+retention enema of medicinal charcoal tablet),and Jiangan Mixture group(basic treatment+retention enema of Jiangan Mixture).Each group was treated for 1 month.The changes of renal functioncreatinine(CRE),blood urea nitrogen(BUN),uric acid(UA),cystatin C(Cys C)],inflammatory factorsmonocyte chemoattractant protein-1(MCP-1),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were compared before and after treatment.Results:After treatment,the levels of CRE,BUN,UA and Cys C in the two groups were lower than those in the blank control group(P<0.01),and the above indexes in the Jiangan Mixture group were lower than those in the medicinal charcoal tablet control group(P<0.01).After treatment,the levels of MCP-1,TNF-αand IL-6 in the two groups were lower than those in the blank control group(P<0.05),and the above indexes in the Jiangan Mixture group were lower than those in the medicinal charcoal tablet control group(P<0.05).Conclusion:The clinical effect of Jiangan Mixture enema in the treatment of renal insufficiency of turbid-toxin accumulation syndrome is significant,which can effectively improve the renal function of patients and reduce the inflammatory reaction of patients with renal failure.
