锡类散联合美沙拉秦肠溶片治疗大肠湿热型溃疡性结肠炎临床观察

目的研究锡类散灌肠结合美沙拉秦肠溶片对大肠湿热型溃疡性结肠炎(UC)患者的影响。方法按照随机数字表法将葫芦岛市中心医院2019年11月—2020年2月收治的100例大肠湿热型UC患者分为对照组和治疗组,各50例。分别接受美沙拉秦肠溶片口服单独治疗、美沙拉秦肠溶片口服联合锡类散灌肠治疗。观察2组的疗效以及不良反应,患者治疗前后的临床症状积分变化、患者结肠组织中Th1/Th2细胞的细胞因子γ干扰素(IFN-γ)、B细胞生长因子-1(IL-4)的表达水平。结果治疗组的总有效率94.00%(47/50),显著高于对照组82.00%(41/50)(P<0.05)。2组患者的临床症状积分、纤维结肠检积分都有降低,且治疗组治疗后的上述积分明显低于对照组(P<0.05);2组患者的肠黏膜IFN-γ降低,IL-4表达水平升高,且治疗组治疗后的肠黏膜IFN-γ明显低于对照组,IL-4水平明显高于对照组(P<0.05);本研究中并未发现不良反应。结论用锡类散灌肠治疗结合口服美沙拉秦肠溶片在治疗大肠湿热型UC中更加安全有效,能更好调节Th1/Th2细胞平衡,临床可进一步应用。

Effect of Xilei Powder combined with mesalazine enteric-coated tablets in treatment of ulcerative colitis

Objective To study the effects of Xilei powder enema combined with mesalazine enteric-coated tablets on Th1/Th2 cells drift in patients with ulcerative colitis(UC)of large intestine damp-heat type.Methods A total of 100 UC patients of large intestine damp-heat type who were admitted to the hospital from November 2019 to February 2020 were divided into control group and treatment group by random number table method,50 cases in each group.The two groups were treated with oral mesalazine enteric-coated tablets or mesalazine enteric-coated tablets combined with intestinal canal Xilei powder,respectively.The curative effect and adverse reactions,changes in scores of clinical manifestations before and after treatment,expression levels of Th1/Th2 cytokines interferonγ(IFN-γ)/B cell growth factor-1(IL-4)in colon tissues of both groups were observed.Results The total response rate of treatment group was significantly higher than that of control group(94.00%vs 82.00%)(P<0.05).After treatment,scores of clinical symptom and expression level of IFN-γin both groups were decreased,and the above-mentioned scores in treatment group were significantly lower than those in control group(P<0.05).The expression level of IL-4 in both groups was increased,which was significantly higher in treatment group than control group(P<0.05).There were no adverse reactions in this study.Conclusion It is safer and more effective to treat UC of large intestine damp-heat type with Xilei powder enema combined with oral mesalazine enteric-coated tablets,which can better adjust the Th1/Th2 cell balance.
