三级筛选法治疗儿童肱骨髁上骨折及肘内翻预防研究

目的制定儿童移位肱骨髁上骨折的规范化治疗方案,降低肘内翻等并发症的发生。方法 2008年1月-2010年4月,89例患者根据骨折类型及局部软组织等特点用三级筛选法进行治疗,最后对其疗效进行分析评价。结果手法复位成功43例,牵引复位成功22例,手术切开复位24例。随访89例,时间5-28个月,骨折均在3个月内愈合,疗效评价优58例,良23例,可7例,差1例,优良率达91.0%。结论三级筛选法是儿童移位肱骨髁上骨折的一种理想治疗手段;避免和减少了肘内翻并发症的发生。

Screening treatment of supracondylar facture of humerus in children and prevention of cubitus varus

Objective To enact normative scheme for the treatment of displaced supracondylar facture of humerus in children so as to lessen the complication such as cubitus varus.Methods From Feb.2008 to Apr.2010,89 patients were treated by screening scheme according to standard such as fracture classification and soft tissue swelling.Clinical effects of the screening scheme were analyzed and evaluated.Results Forty-three cases got success with manipulative reduction and 22 cases got success with traction reduction.Twenty-four cases got success with operative reduction.All cases were followed up from 5 to 28 months.All the fractures got healing in 3 months.According to effect evaluation,58 cases were excellent,23 cases good,7 cases normal,and 1 case poor.Conclusion The screening scheme is an ideal therapeutic tool to displaced supracondylar facture of humerus in children.It can avoid and decrease incidence of cubitus varus.
