60例传染性单核细胞增多症临床分析

目的 分析传染性单核细胞增多症(传单)的临床特点、实验室检查与并发症,为进一步明确该病的诊断与治疗.方法 回顾性分析我院儿科收治60例确诊为传单病例资料.结果 76.7%病例发生于7岁以下儿童,以秋季发病居多,100%有淋巴结肿大,90%有发热,93.3%有咽峡炎,76.7%有眼睑浮肿及鼻塞.并发症发生率88.3%,尤以肝脏损坏最常见.93.4%的病例变异淋巴细胞＞10%, EB-IgM阳性91.7%,EB-IgG阳性40%.结论 大多数传单具有较典型的临床表现,并发症可累及多种器官、系统,尤以肝脏损坏最常见;本病有免疫功能异常,绝大多数预后良好,但重症可致死.提高对本病的认识有助于减少临床误诊和漏诊.

Clinical analysis of 60 cases of infectious mononucleosis

Objective To analyze the clinic features,the laboratory examinations and the complications of infectious mononucleosis(IM) in children.Methods The data of 60 cases of children with IM were analyzed retrospectively.Results 76.7% cases occured among children under 7 years old.Although IM occurs over the year,there was a high peak in autumn.100% cases of IM had lymph node swell,90% cases had fever,93.3% cases had angina and 76.7% cases eyelid edema and snuffle.The occurring rate of the complications was 88.3%,especially deteriorated liver function was the most familiar.Above 10% of the atypical lymphocytes of 93.4% cases was raised.The positive of EB-IgM was 91.7% and EB-IgG was 40%.Conclusion The majority of IM of the patients had typical clinical features of infectious mononucleosis syndrome,the complications of IM were common and diversified which affected multiorgan and multi-system,especially deteriorated liver function.Abnormity of immune function of IM was common.The majority of cases of IM were benign and self-limiting.Understanding of the clinic features,the laboratory examinations and complications of IM will be helpful to reduce misdiagnosis.
