Objective To compare the prognosis of small hepatocellular carcinoma patients with hepatitis B virus infection versus hepatitis C virus infection. Methods 413 patients receiving curative resections at Tianjin Cancer Hospital for small HCC (≤ 3 cm) from January 1997 to December 2003 were divided into four groups: HCV only (n = 75), HBV only (n = 251), HBV and HCV (n = 33), and neither HBV nor HCV (NBNC, n = 54). The preoperative status and postoperative recurrence were recorded. Survival analysis were used to assess the impact of HBV/HCV status on HCC recurrence. Results Patients with HCV were associated with older age, lower mean preoperative platelet counts and albumin levels, higher mean prothrombin time, alanine aminotransferase and total bilirubin levels. Tumors in patients with HCV are mul-tinodular and less differentiated, and were associated with a higher incidence of vascular invasion and cirrhosis. During the follow-up, the HCV group showed a higher incidence of intrahepatic recurrence and multiple recurrent lesions than the other patients. Conclusions HCC patients with HCV infection tended to be older, and were characterized by more severe cirrhosis and higher incidence of tumor multinodular. The statistically significant determinants of reoccurrence in patients with small HCC were HCV infection, presence of vascu- lar invasion and multiple tumors.  相似文献

目的:探讨免疫及靶向药物联合肝动脉灌注化疗术(hepatic arterial infusion chemotherapy,HAIC)治疗晚期肝细胞癌(hepatocellular carcinoma,HCC)的疗效与安全性。方法:回顾性分析2021年4月至2022年4月天津医科大学肿瘤医院收治的34例行HAIC联合信迪利单抗及贝伐珠单抗生物类似物治疗的晚期HCC患者,以首次治疗为起点,以患者死亡、疾病进展及不可耐受的不良反应为终点,按照实体肿瘤的疗效评价(mRECIST)1.1标准进行疗效评估,随访截至2023年4月。主要研究终点为客观缓解率(objective response rate,ORR),次要研究终点为疾病控制率(disease control rate,DCR)、总生存期(overall survival, OS)、无复发生存期(disease-free survival,DFS)、手术转化率及安全性。结果:ORR为52.9%,DCR可达到85.3%,手术转化利率为41.1%。部分缓解(partial response,PR)组1年OS及DFS分别为94.4%、50.0...  相似文献

目的探讨在胰十二指肠切除术中,为达到根治切除目的,联合肠系膜上静肪/门静脉(SMV/PV)切除的临床价值。方法回顾性分析1999年1月-2005年12月间天津医科大学附属肿瘤医院118例胰腺癌行胰十二指肠切除术患者,其中21例联合了不同程度的SMV/PV的切除,对两组患者的临床病理特征、并发症发生率、围手术期死亡率及预后进行分析。结果两组间在手术并发症、围手术期死亡率及术后生存率上比较无统计学差异。血管切除组中,共有8例患者术后病理证实SMV/PV受累,预后较差。但与同期未切除受累血管、断端阳性患者相比,血管切除组预后好于癌残留组(P〈0.01)。结论对于胰头癌局部浸润SMV/PV,但无远处转移的患者,应积极行手术切除。胰腺癌联合SMV/PV切除可以有效提高手术切除率,但并未增加术后并发症及围手术期死亡率。联合SMV/PV切除对提高胰腺癌的根治率和生存率有重要的意义。  相似文献

腹部恶性纤维组织细胞瘤47例临床和预后分析   总被引:3,自引:0,他引:3  
目的探讨腹部恶性纤维组织细胞瘤(MHF)的临床病理特点、治疗策略及预后相关因素。方法回顾性分析1970年1月至2000年1月间天津医科大学附属肿瘤医院47例腹部恶性纤维组织细胞瘤患者手术切除治疗情况。所有患者均经手术病理证实,病变单一。结果术后随访中共有32例患者出现复发、转移,术后单纯局部复发13例(29.5%),出现肺及胸膜转移7例(15.9%),肝转移4例(9.0%),骨转移6例(13.6%),肾脏转移2例(4.5%),复发 转移5例(11.4%)。术后1,3,5年无瘤生存率分别为65.9%、40.9%和29.5%,累计生存率分别为70.5%、52.3%和31.8%,中位生存期为18个月。影响预后的主要因素为肿瘤发生的部位、治疗方式、病理类型、肿瘤的局部复发等。以手术为主的综合治疗较单纯手术及放化疗预后好,术后辅以放疗可降低局部复发率,尤其对那些切端阳性患者。结论腹部MHF侵袭性强,复发率高,病理分级差,以手术切除为主辅以放疗有助于提高患者的远期生存率。  相似文献
