Objectives

Dentin biomodification using collagen cross-linkers has been proposed as one of the strategies to improve bond durability of adhesives to dentin. However, literature is not very consistent regarding their benefit, in particular when cross-linkers are applied in clinically realistic application times. This study investigated the effect of three cross-linkers on the mini-interfacial fracture toughness (mini-iFT) of four adhesives bonded to dentin following either etch&rinse (E&R) or self-etch (SE) modes.

Methods

60 molars were randomly divided in accordance with the three variables: cross-linker, adhesive and bonding mode (n = 5). The cross-linkers glutaraldehyde (5 wt%; GA), proanthocyanidin (6.5 wt%; PA), or UVA-activated riboflavin (0.5 wt%; RB), and distilled water (control) were applied on dentin for 60 s after acid-etching (E&R) or before self-etching (SE). The 3-step E&R adhesive (3E&Ra) OptiBond FL (Kerr), the 2-step SE adhesive (2SEa) Clearfil SE Bond 2 (Kuraray Noritake) and the universal adhesives G-Premio Bond (GC) and Prime&Bond Active (Dentsply), the latter two employed in both E&R and SE modes, were applied following the respective manufacturer’s instructions. Composite buildups (8 × 8 × 8 mm) were made using Filtek Supreme XTE (3M) prior to 1-week storage in artificial saliva. After the teeth were sectioned into mini-specimens (1.5 × 2.0 × 18 mm), a single notch was prepared at the adhesive–dentin interface. Half of the specimens were immediately loaded until failure by 4-point bending to determine the mini-iFT, while the remaining specimen set was tested upon 6-month aging. Data were statistically analyzed with a linear model (p < 0.05).

Results

No significant decrease in mini-iFT was noted only for PA (p < 0.05), while the mini-iFT decreased for both other cross-linkers and in quite a similar way as when solely water (Wa) was applied.

Significance

The cross-linker proanthocyanidin (PA) applied in clinically relevant conditions was able to maintain a stable mini-iFT after 6-month aging. The incorporation of UVA-activated riboflavin (RB) and glutaraldehyde (GA) in the dentin-bonding protocol appeared not effective to improve the stability of adhesive–dentin interfaces.  相似文献

Objective

To assess light irradiance (LI) delivered by two light-curing units (LCU’s) and to measure the degree of conversion (DC) of three composite cements, when cured through different thicknesses of two novel CAD–CAM block materials.

Methods

100-μm-thick films of a dual-curable composite cement (G-CEM LinkAce, GC), a light-curable flowable resin-based composite (RBC) (G-ænial Universal Flo, GC) and a micro-hybrid RBC (G-ænial Posterior, GC) were investigated as luting agents. Two ‘polymer–ceramic’ CAD–CAM blocks (Cerasmart, GC; Enamic, Vita Zahnfabrik) were sectioned in slabs with different thicknesses (1, 3 and 5 mm). LI at the bottom of the specimens was measured using a calibrated spectrometer, while being light-cured through the CAD–CAM block slabs for 40 s with a low- (±500 mW/cm2) or high- (±1,600 mW/cm2) irradiance LCU (n = 5). After light-curing, micro-Raman spectra of the composite films were acquired to determine DC at 5 min, 10 min, 1 h and 24 h. LI data were statistically analyzed by Kruskal–Wallis followed by post-hoc comparisons, while a linear mixed-effect model was applied for the DC analysis. In addition, the CAD–CAM blocks ultrastructure was characterized upon argon-ion slicing using scanning transmission electron microscopy (STEM). Finally, light transmission (LT) through each CAD–CAM block material was assessed using a spectrophotometer.

Results

Curing-light attenuation and DC were significantly influenced by thickness and type of the overlying material. LCU only had a significant effect on DC of the micro-hybrid RBC. DC significantly increased over time for all composite cements. CAD–CAM block structural analysis revealed a relatively small and homogenous filler configuration (mean filler size of 0.2–0.5 μm) for Cerasmart, while Enamic contained ceramic grains varying in shape and size (1–10 μm), which were interconnected by the polymer-based network. LT was much higher at a wavelength range of 300–800 nm for Cerasmart than for Enamic.

Significance

Light-curable composite cements can be cured through a restoration up to 2.7-mm thickness, depending on the kind of CAD–CAM material. A high-irradiance LCU only has a limited effect on the maximum thickness of the polymer–ceramic CAD–CAM material that can be cured through.  相似文献

Objectives

The objective of the study was to evaluate the effectiveness of a short fibre-reinforced composite (FRC) applied in combination with a conventional filler composite (CFC) on the fatigue resistance, fracture strength, failure mode and stress distribution, for restorations of premolars under two loading angles.

Material and methods

Thirty-two inferior premolars received extensive cavities with removal of the lingual cusp. Teeth were restored directly using ‘FRC (EverX Posterior, GC) + CFC (G-aenial, GC)’ or ‘CFC only’ and received two fatigue/fracture loadings at two different angles (0°/45°) (n = 8). Data were submitted to two-way ANOVA (α = 5 %) and Tukey test. Failure mode was analysed using SEM. Four 3D finite element (FE) models were constructed and static, linear and elastic analyses were performed. Maximum principal and von Mises stresses were evaluated.

Results

All specimens survived the mechanical fatigue simulation. No statistical difference in fracture resistance was recorded between FRC + CFC and CFC only, considering both loading angles (p = 0.115). However, the 0° loading showed a statistical significant higher strength than the 45° loading (p = 0.000). Failure mode analysis revealed more repairable fractures upon 0° loading, versus more root fractures (unrepairable) upon 45° loading. FE revealed a higher amount of stress upon 45° loading, with tensile stress being imposed to the lingual cervical area.

Conclusion

The fracture strength was not increased using the FRC. Loading at a 45° decreased significantly the fracture resistance.

Clinical relevance

The restoration of extensive cavities in posterior tooth is a challenge for the clinicians and the choice of the material that increases the fracture strength of tooth-restoration complex is required.

Metabolic syndrome (MS) has been associated with proteinuria and reduced glomerular filtration rate. Immunosuppressive agents increase the incidence of traditional risk factors for cardiovascular disease (CVD) and have known effects on MS components after kidney transplantation. The purpose of this meta‐analysis was to evaluate the impact of MS on relevant outcomes after kidney transplantation. MEDLINE, EMBASE, and Cochrane Library were searched up to November 7, 2015. Papers that compared patients with and without MS and assessed one of the following outcomes, graft loss, death by cardiovascular disease, and all‐cause mortality, were included. Of 585 studies identified, five studies including 1269 patients were evaluated. MS was identified as a risk factor for graft loss [relative risk, 3.06; 95% confidence interval (CI), 2.17, 4.32; I² = 0%; P heterogeneity = 0.72] and death by CVD (relative risk, 3.53; 95% CI, 1.27, 9.85; I² = 0%; P heterogeneity = 0.40). Results on the association between MS and all‐cause mortality were inconclusive (relative risk, 2.61; 95% CI, 0.70, 9.81; I² = 58%; P heterogeneity = 0.09). Graft loss and death by CVD were associated with the presence of MS after transplantation. Randomized clinical trials should be conducted to define whether interventions on each MS component would result in better outcomes after transplantation.  相似文献

With the advancement in ceramic restorations bonded to tooth structure, the treatment has become a feasible and conservative option to restore teeth with alteration in shape when owing to high demand in esthetics, with or without minimum tooth preparation. This article describes a report of a 32‐year‐old woman who was dissatisfied with her smile. The patient reported that she still had deciduous teeth, and that the maxillary lateral incisors had been restored with direct resin composite to correct the teeth shape. After discussing the restorative possibilities, a decision was made to place all‐ceramic crowns (lithium disilicate glass‐ceramic) with minimal tooth preparation, which figures as a conservative full‐coverage approach. An esthetic outcome resembling a natural‐looking smile resulted in the patient's satisfaction. Bonding of all‐ceramic crowns onto minimally prepared deciduous teeth figures as a conservative treatment in case of partial anodontia, which decreases the chance of root resorption induced by trauma and benefits from good adhesion to enamel.  相似文献

Statement of problem

Controlled clinical trials comparing the effectiveness of the walking bleaching (WB) technique and the inside-outside (I-O) technique used in a short daily regimen are lacking.

Purpose

The purpose of this randomized clinical trial was to investigate the effectiveness of WB with that of the I-O technique conducted over 4 weeks and to compare color changes after 1 year.

Material and methods

Discolored and endodontically treated anterior teeth received a cervical seal and were randomly divided into groups according to the technique. In the WB group (n=9), a mixture of sodium perborate and 20% hydrogen peroxide was applied in the pulp chambers and replaced weekly up to 4 weeks. For the I-O group (n=8), participants applied 10% carbamide peroxide in the pulp chambers and wore custom-fitted trays for 1 hour per day over 4 weeks. CIELab parameters were obtained using a spectrophotometer at baseline, during bleaching (1, 2, 3, and 4 weeks) and after 1 year. Changes in color (ΔE), lightness (ΔL*), green-red axis (Δa*), blue-yellow axis (Δb*), and absolute color parameters (L*, b*, and a*) for each evaluation time were calculated and analyzed by repeated measures analysis of variance (ANOVA) and post hoc Bonferroni test (α=.05).

Results

No significant differences between WB and I-O techniques were observed for ΔE, ΔL*, Δa*, Δb*, L*, a*, or b* values (P>.05); however, significant differences were observed among the evaluation times (P<.05). Color changes observed after 2 weeks were stable after 1 year; ΔL* and Δa* values after 1 year were not significantly different from the 1-week evaluation, and significant changes in Δb* after 3 weeks were maintained at the 1-year follow-up. The same trend was observed for the absolute CIELab color parameters.

Conclusions

Both WB and I-O regimens were similarly effective as shown by significant ΔE after 2 weeks and no color regression after 1 year.  相似文献
