ABSTRACTObjectives: Our was objectives were to (1) assess the validity of a smartphone-based application to obtain spatiotemporal gait variables relative to an established movement monitoring system used previously to evaluate post-concussion gait, and (2) determine the test-retest reliability of gait variables obtained with a smartphone.Methods: Twenty healthy participants (n = 14 females, mean age = 22.2, SD = 2.1 years) were assessed at two time points, approximately two weeks apart. Two measurement systems (inertial sensor system, smartphone application) acquired and analyzed single-task and dual-task spatio-temporal gait variables simultaneously. Our primary outcome measures were average walking speed (m/s), cadence (steps/min), and stride length (m) measured by the inertial sensor system and smartphone application.Results: Correlations between the systems were high to very high (Pearson r = 0.77–0.98) at both time points, with the exception of dual-task stride length at time 2 (Pearson r = 0.55). Bland-Altman analysis for average gait speed and cadence indicated the average disagreement between systems was close to zero, suggesting little evidence for systematic bias between acquisition systems. Test-retest consistency measures using the smartphone revealed high to very high reliability for all measurements (ICC = 0.81–0.95).Conclusions: Our results indicate that sensors within a smartphone are capable of measuring spatio-temporal gait variables similar to a validated three-sensor inertial sensor system in single-task and dual-task conditions, and that data are reliable across a two-week time interval. A smartphone-based application might allow clinicians to objectively evaluate gait in the management of concussion with high ease-of-use and a relatively low financial burden. 相似文献

Thiol‐water proton exchange of glutathione,cysteine, and N‐acetylcysteine: Implications for CEST MRI

Amide‐, amine‐, and hydroxyl‐water proton exchange can generate MRI contrast through chemical exchange saturation transfer (CEST). In this study, we show that thiol‐water proton exchange can also generate quantifiable CEST effects under near‐physiological conditions (pH = 7.2 and 37°C) through the characterization of the pH dependence of thiol proton exchange in phosphate‐buffered solutions of glutathione, cysteine, and N‐acetylcysteine. The spontaneous, base‐catalyzed, and buffer‐catalyzed exchange contributions to the thiol exchange were analyzed. The thiol‐water proton exchange of glutathione and cysteine was found to be too fast to generate a CEST effect around neutral pH due to significant base catalysis. The thiol‐water proton exchange of N‐acetylcysteine was found to be much slower, yet still in the fast‐exchange regime with significant base and buffer catalysis, resulting in a 9.5% attenuation of the water signal at pH 7.2 in a slice‐selective CEST NMR experiment. Mumps, a vaccine-preventable disease, cause inflammation of salivary glands and may cause severe complications, such as encephalitis, meningitis, deafness, and orchitis/oophoritis. In India, mumps vaccine is not included in the universal immunization program and during 2009 to 2014, 72 outbreaks with greater than 1500 cases were reported. In August 2016, a suspected mumps outbreak was reported in Jaisalmer block, Rajasthan. We investigated to confirm the etiology, describe the epidemiology, and recommend prevention and control measures. We defined a case as swelling in the parotid region in a Jaisalmer block resident between 23 June 2016 and 10 September 2016. We searched for cases in health facilities and house-to-house in affected villages and hamlets. We tested blood samples of cases for mumps immunoglobulin M (IgM) enzyme-linked immunosorbent assay (ELISA). We found 162 cases (60% males) with a median age of 9.4 years (range: 7 month-38 years) and 65 (40%) were females. Symptoms included fever (70%) and bilateral swelling in neck (65%). None of them were vaccinated against mumps. Most (84%) cases were school-going children (3-16 years old). The overall attack rate was 2%. Village A, with two hamlets, had the highest attack rate (hamlet 1 = 13% and hamlet 2 = 12%). School A of village A, hamlet 1, which accommodated 200 children in two classrooms, had an attack rate of 55%. Of 18 blood samples from cases, 11 tested positive for mumps IgM ELISA. This was a confirmed mumps outbreak in Jaisalmer block that disproportionately affected school-going children. We recommended continued surveillance, 5-day absence from school, and vaccination. 相似文献
