Simulating the pharmacokinetics of tabletized ambroxol using the dynamics of drug distribution in saliva

The pharmacokinetics of ambrogexal (generic) and ambroxol (INN) tablets were investigated in 18 healthy volunteers after peroral administration in a single dose of 60 mg. The levels of ambroxol hydrochloride in saliva were significantly higher (statistically reliable) than those in the blood plasma for the first 1.5 hours after administration due to features of the drug absorption in the gastrointestinal tract. There are statistically reliable differences between the pharmacokinetic parameters (C _max, T _max, T _1/2, V _z) calculated from the dynamics of ambroxol hydrochloride concentration in the saliva and plasma. Using the proposed method of extrapolation of the drug concentration, it is possible to use saliva for studying the pharmacokinetics of ambroxol administered in tabletized drug forms. Use of the coagglutination reaction for typing Leptospira cultures

The coagglutination test has been employed for the serologic typing of Leptospira cultures. A set of diagnostic agents with a maximum kit of anti-Leptospira sera of various serogroups has been prepared on the basis of a 10% suspension of formalin-treated Cowan 1 staphylococcus strain. To prepare the diagnostic agent, 0.1 portion of the serum and 9 portions of phosphate buffer solution, pH 7.2 should be added to 1 portion of a 10% staphylococcal suspension. 1% staphylococcal suspension has been prepared for control. The diagnosticum and the culture to type (0.01 ml each) have been mixed on the slides; the control test has been carried out with 1% non-sensitized staphylococcal suspension. The test has been read by microscopy in the dark visual field after the slides' 15 min exposure in a humid chamber. Typing of 17 Leptospira cultures of known serologic groups has yielded positive responses with the homologous diagnosticums only; no cross reactions have been recorded. The coagglutination test may be used for the rapid differentiation of mixed Leptospira cultures belonging to different serologic groups.

Surgical treatment of patients with a bleeding giant ulcer of the stomach and duodenum]

The results of surgical treatment of 112 patients with a bleeding giant gastric, or duodenal ulcer, comprising 18.6% of the number of patients with acute ulcerous gastrointestinal bleeding operated on in the emergency order are presented. The use of active surgical tactics permitted to increase the specific weight of radical operations to 62.8%, reduce the incidence of early postoperative complications 1.8-fold, lethality--3.4-fold when compared with the results of the use of active-expectant tactics. An operation of choice in bleeding giant gastric ulcer is Billroth-I resection of the organ, or its suprapyloric resection; in ulcer location at the pyloroduodenal region--selective proximal vagotomy with ulcer excision, or selective vagotomy with antrectomy. To arrest bleeding in performance of minimal, or organ-preserving operation with vagotomy, it is necessary to perform ulcer excision, because after closing the bleeding ulcer, or suturing the bleeding vessel in an ulcer, 25.8% of the patients developed recurrency of the bleeding at the nearest postoperative period.     

The acid-base status and oxygen-transport function of the blood in patients in the early period after surgical interventions for ulcerative gastroduodenal hemorrhage]

Changes in the acid-base state and acid-transport function of blood were studied in 59 patients with gastroduodenal ulcer bleedings. More than 50% of the changes were more pronounced in patients with a deficient globular volume and its recovery was somewhat slower. In spite of reduced values of the concentration of hemoglobin in blood, 2,3-DPH and ATP of erythrocytes, most patients were found to have satisfactory compensation of the acid-transport function and balanced oxygen affinity for hemoglobin during the whole postoperative period.
