桡骨远端骨折不同治疗方法临床疗效比较   总被引:8,自引:3,他引:5  
[目的]探讨不同分型桡骨远端骨折的有效治疗方法.[方法]分析236例桡骨远端骨折的治疗,根据AO分型为A、B、C 3型,分别采用闭合手法复位中医小夹板外固定及石膏外固定、切开复位"T"形钢板内固定3种固定方法,分别进行骨折的复位评分、功能评分,并比较不同固定方法之间复位评分及功能评分优良率.[结果]236例病人术后均获随访,随访时间6~20个月,平均14.6个月.3组当中A、B型骨折复位评分优良率无显著性差别,功能评分小夹板外固定组及"T"形钢板内固定组优良率明显优于石膏外固定组,功能评分小夹板外固定组和"T"形钢板内固定组比较优良率无显著性差别;C组当中无论是复位评分还是功能评分,"T"形钢板内固定组优良率明显优于其它2组.[结论]对于C型骨折,建议行骨折切开复位内固定;对于A、B型骨折,闭合手法复位小夹板外固定能取得较好的临床疗效.  相似文献

BACKGROUND: Wood dust is known to cause allergic occupational asthma and obeche (Triplochiton scleroxylon) is a prominent exponent in this field. However, the knowledge about wood allergens is still limited. The aim of this study was to identify and characterize obeche wood allergens. METHODS: Obeche extracts were prepared from freshly ground in comparison to 7 years stored wood dust and investigated by Sodium dodecyl sulfate-polyacrylamid gel electrophoresis, enzyme-linked allergosorbent test and immunoglobulin (Ig)E-immunoblot. Allergens were detected by specific IgE of seven obeche allergic patients' sera and protein analysis was performed by mass spectrometry. Cross-reactivity was demonstrated by ImmunoCAP-inhibition with sera of seven obeche and four latex-allergic patients. RESULTS: Obeche extracts showed different protein pattern and IgE-binding capacities depend on the age of the wood dust. A 38 kDa protein was identified as major obeche wood allergen, detected by six of seven (85%) obeche allergic patients' sera and was entitled as Trip s 1. Trip s 1 is homologous to plant class I chitinases and exhibited enzyme activity demonstrated by chitinolysis. Co-recognition or cross-reactivity of Trip s 1 according to structural similarity was seen in sera of latex allergic patients. IgE inhibition studies with obeche as solid phase and Trip s 1 and latex hevein as inhibitor demonstrated that Trip s 1 was a more effective inhibitor in obeche as well as in latex allergic patients' sera. CONCLUSIONS: Trip s 1 is a new obeche wood allergen of the plant class I chitinase family. This finding may explain the dominant role of obeche in sensitization against wood dust.  相似文献

A study of wood ducks (Aix sponsa) was conducted along the Coeur d'Alene River system in northern Idaho in 1986 and 1987. Most of this area has been subjected to severe contamination from lead and other metals from mining and smelting since the 1880s. In 1986, a preliminary study of wood duck nesting was conducted in the contaminated area; incubating hens captured in nest boxes were bled and weighed. Blood samples were used to determine lead and cadmium concentrations and physiological characteristics. In 1987, an intensive study of wood ducks involved trapping and monitoring nest boxes in the contaminated area. Blood and tissue samples were also taken from wood ducks from a reference area without known contamination from metals. Lead levels in blood and tissues of most wood ducks from the contaminated area frequently exceeded those considered hazardous to birds; maximum levels (wet weight) of lead were 8 g g–1 in blood and 14 g g–1 in liver. Changes in physiological characteristics constituted the only evidence of potentially adverse effects from lead. In the contaminated area, nesting success (55% unadjusted, 35% Mayfield estimate) was less than in other areas where predation was low and nest boxes were used; but lead concentrations and physiological characteristics of blood were similar in successful and unsuccessful hens.Values of ALAD, hemoglobin, and body mass were negatively correlated with blood concentrations of lead, whereas protoporphyrin was positively correlated with lead levels in the blood. Some of the protoporphyrin values (1,091 g dl–1 in a male and 756 g dl–1 in a female) equalled those associated with lead toxicosis in experimental birds. ALAD activity was low in most birds from the contaminated area; values of 0 were obtained from 11 birds. Lead levels in blood, ALAD, protoporphyrin, and hemoglobin were significantly different between birds from the contaminated and reference areas. Concentrations of lead in ingesta of wood ducks ranged from 0.9 to 610 g g–1 in the contaminated area and 0.2 to 0.6 g g–1 in the reference area. Levels of cadmium in kidneys of wood ducks ranged from 1g g–1 to 20 g g–1 in the contaminated area and from only to 0.1 g g–1 to 1 g g–1 in the reference area. Cadmium concentrations were less than known effect levels.  相似文献

Four thousand nine hundred and eight male workers of the Hamburg gas plant, employed for 10 or more years during the period from January 1, 1900 to December 31, 1989, have been traced for cancer mortality. Based on their jobs, they were grouped into three subcohorts: gas furnace workers, workers in other parts of the plant, and white-collar workers. Despite the general "healthy worker effect," gas furnace workers showed a significant incidence of cancer of all sites (standardized mortality ratio [SMR] = 186), particularly cancers of the lung (SMR = 288), stomach (SMR = 177), and the colon-rectum (SMR = 184). In comparison to the death rates of white-collar workers, the manual workers in other parts of the plant also showed an excess of cancer mortality for cancer of these sites. The reason for this excess in cancer mortality seems to be due to the working conditions. The results contribute to the current evidence that exposure to coal carbonization fumes causes not only lung cancer, but also cancers of the stomach and colon-rectum.  相似文献

Inflammation is regarded as an important mechanism behind mortality and morbidity experienced by cardiorespiratory patients exposed to urban air particulate matter (PM). Small-scale biomass combustion is an important source of particulate air pollution. In this study, we investigated association between inflammatory responses and chemical composition of PM1 emissions from seven different small-scale wood combustion appliances representing old and modern technologies. Healthy C57Bl/6J mice were exposed by intratracheal aspiration to single dose (10?mg/kg) of particulate samples. At 4 and 18?h after the exposure, bronchoalveolar lavage fluid (BALF) as well as serum was collected for subsequent analyses of inflammatory indicators (interleukin (IL)-6, IL-1β, IL-12, and IL-10; tumor necrosis factor-α (TNF-α); keratinocyte-derived chemoattractant (KC), and interferon-γ (IFN-γ)) in multiplexing assay. When the responses to the PM1 samples were compared on an equal mass basis, the PM from modern technology appliances increased IL-6, KC, and IL-1β levels significantly in BALF at 4 and 18?h after the exposure. In contrast, these responses were seen only at 4?h time point in serum. Increased cytokine concentrations correlated with metal-rich ash related compounds which were more predominant in the modern technology furnaces emissions. These particles induced both local and systemic inflammation. Instead, polycyclic hydrocarbon (PAH) rich PM1 samples from old technology (OT) evoked only minor inflammatory responses. In conclusion, the combustion technology largely affects the toxicological and chemical characteristics of the emissions. The large mass emissions of old combustion technology should be considered, when evaluating the overall harmfulness between the appliances. However, even the small emissions from modern technologies may pose significant toxic risks.  相似文献

In Sumatra and Kalimantan in Indonesia and Sabah in Malaysia, the spread of two diseases, aggravated by damage by fauna, and by the humid tropical environment, has forced a change of planted species from Acacia mangium to Eucalyptus pellita and related interspecific hybrids, at a scale unprecedented in the history of plantation forestry. This experience highlights the risks of relying on any single species for large contiguous plantation estates in environments with endemic biotic and abiotic stresses. There is a need to transition to multiple-species plantation forestry, adopting the opportunities for changeover of species and varieties in short-rotation systems. Industry’s responses for coping with this rapid change have been helped by earlier collaborative research on acacia which promoted management practices that conserved site resources and avoided site degradation during the critical harvesting and inter-rotation phase. The current growth rates of E. pellita and its hybrids are lower than that of A. mangium. Estimates of production from plantations and expanding capacity of the pulp and paper mills highlight a risk of significant shortfalls in wood production. Improving the quality and consistency of forest operations and revision and refocussing of research and development are critical to set and achieve realistic targets for stable, incremental improvements in productivity. Each company should consider investing systematically in contemporary adaptive research for developing and applying integrated management practices that are efficient and feasible on large scales and suitable for their circumstances. There is also a critical need for research to understand the dynamics of local human capital and its relationships with the forestry sector, with focus on improving access to a stable labour force.  相似文献
