中医学古籍中罕有关于“胰腺”的明确记载,缺乏直观、系统的理论论述。本文试图通过对各个时期具有代表性的医学典籍所记载内容的分析,结合现代解剖学相关理论,梳理中医学对胰腺实体解剖的认识过程。通过分析,笔者认为中医学对胰腺实体解剖的认识具有阶段性:①先秦两汉时期,存在胰腺实体解剖,但并非认为胰腺是脏器;②唐宋时期,胰腺实体解剖更加清晰,在医学上胰腺附属于脾,并非为独立的脏器;③明清时期,胰腺实体解剖明确,部分医家以独立脏器论之,出现“脾”、“胰”之争。中医学理论缺少对胰腺的单独论述,目前学界的主流观点多为“胰属脾”,线性归属以脾笼统代之略显单薄,不利于理论的丰富与发展。笔者认为胰腺藏象应独立于脾单独讨论,现代解剖学对胰腺命名同一,形态结构清楚,位置描述明确,可直接补充进中医学胰腺藏象(藏)理论中,为完善胰腺藏象理论搭建解剖学基础。  相似文献

目的探讨1 470 nm激光剜除治疗高危前列腺增生的手术技巧及临床效果。 方法回顾分析2018年6月至2018年9月中山大学附属第三医院泌尿外科采用1 470 nm激光治疗共89例高危前列腺增生患者的临床资料,年龄平均(68±3)岁,前列腺体积(57.4±2.6)ml。所有患者均采用"寻找层面,先易后难,剜切结合"的层面递进法思路行激光腔内前列腺剜除术,比较患者术中及术后情况。 结果89例均顺利完成手术,与术前相比,术后3个月患者最大尿流率明显增加,[(6.9±2.1) ml/s vs(19.8±3.6) ml/s]。国际前列腺症状评分显著好转,[(24.6±1.7) vs(8.0±1.2)]。术中无输血、无电切综合征、无直肠和膀胱穿孔病例,无输尿管损伤、大出血、心脑血管意外等严重并发症发生。 结论层面递进法激光剜除技术构想对于高危前列腺增生外科包膜层面的寻找、减少术后并发症有独到优势,且易于掌握,或可为业界同行提供一个新的思路。  相似文献

IntroductionCentral venous catheter applications and complications are closely related to the tip position. Previous studies have reported some rare cases of catheter misplacement. Here, we report a case of misplacement of a peripherally inserted central catheter into the lateral thoracic vein.Case reportA 56-year-old cancer patient underwent placement of a peripherally inserted central catheter through the left basilic vein under ultrasound-guided puncture. The catheterisation procedure was uneventful, so the catheter was believed to be in the superior vena cava. However, the post-anterior chest X-ray image revealed that after the catheter advanced towards the axilla, it turned downwards and outwards in the direction of the left lateral thoracic region, with the projection of the catheter tip giving the appearance of termination in the subcutaneous tissue of the lateral thoracic wall on the two-dimensional image. The catheter was then repositioned in the distal superior vena cava.DiscussionPeripherally inserted central catheters can be potentially misplaced into the lateral thoracic vein because these catheters can pass through the orifice of the lateral thoracic vein which flows into the axillary vein. Some pathological cases and clinical conditions can cause dilatation of the lateral thoracic vein, which increases the probability of catheter misplacement. Three principles were proposed to avoid this rare complication: a comprehensive review of the patients’ medical history, real-time image-guided catheterisation and routine radiographic identification of the tip position.  相似文献

Objective Based on the Helical Ventricular Myocardial Band (HVMB) theory proposed by Torrent-Guasp,the ventricular myocardial hand extends from the root of the pulmonary artery to the root of the aorta with two helical coils.This new theory is considered as a revolutionary concept for further understanding the global, three-dimensional and functional architecture of the ven- tricular myocardium. No repot had described techniques for disecting HVMB while keepin~ the integrity of the coronmy artery sys- tern. We explored techniques for dissecting HVMB in swine.Methads 33 fresh swine hearts were randomly divided intoll groups, 3 bearts in each. 160% barium sulfate (type I)suspmmion was injected into the coronary artery system. The coronary arteries were li- gated. The strial tissue was removed following puuing the hearts in boiling water then cooling for several hours. The superficial coro- nary vessels and fat tissue around the atrio-ventricular taxi inter-ventricular sulcus we~'e preserved. Some branches of the left anterior descending artery, distal segment, of posterior descending branch, and middle and distal segment of obtuse marginal branches were mu- tilated appropriately. HVMB dissection was completed with fingers in accordnce with Torrent Guasp' s technique. Results A contin- ued bundle of muscle, originated at the root of pulmonary artery and ended at the root of aorta, was unwrapped along the major dire- tion of the cardiac muscle fiber in all of the 33 hearts with spating of the coronary artery. The swine hearts' ventricular myocandium was cumosed of two loops, with basal loop firm the root of the pulmonart artery to the anterior papillary muscle and apical from the beginning of the anterior papillary muscle to the root tithe aorta. Each loop consisted of two segments: the right segment-coincid- ing with the right ventricular free wall and the left segment-coinciding with the basal d the left ventricular free wall. Posterior papillary muscle, which belongs to the descendant segment, denmrcated the border between the descendent and the ascendant of the HVMB's apical loop. Conclusion Although controversies about the theory of the HVMB remain, we have dissected the HVMB in the swine hearts' ventricular myocardium successfully with sparing of the coronary artery systems. This dissection procedure provides technical information for the studies of associated diseases based on the theory of HVMB.  相似文献

目的 :为前交通动脉复合体部位显微手术避免损伤Heubner回返动脉提供相关的应用解剖学资料。方法 :对经双侧椎动脉 (VA)、颈内动脉 (ICA)乳胶灌注的尸头标本 2 2例 ,按翼点入路方向 ,在手术显微镜下解剖观测 2 0例Heubner回返动脉的起点、管径、行径及其毗邻关系。 2例模拟手术操作。结果 :(1)本组观测到 4 3支回返动脉 ,其中 39支Heubher回返动脉 ;4支副Heubner回返动脉。 (2 )Heubner回返动脉 4 6 .5 % (2 0 / 4 3)起于A2 段 ,2 5 .6 % (11/ 4 3)起于A1段 ,2 7.9% (12 /4 3)起于前交通动脉 (ACOA)水平的大脑前动脉 (ACA)。 (3)Heubner回返动脉起点管径 :左侧 0 .81± 0 .2 1mm(0 .35~ 1.17mm) ;右侧 0 .84± 0 .2 6mm(0 .37~ 1.2 1mm)。 (4 )回返动脉发出后 ,与ACA反方向成锐角 ,沿A1上壁、外侧及后内侧 ,越过ICA两分叉 ,经大脑中动脉 (MCA)始段前面穿通入脑 ,少数回返动脉行走于前穿质 (APS)的后部。A1近端 3~ 5mm或中 1/ 3段穿通支少。结论 :在前交通动脉复合体部位手术时 ,A1近端 3~ 5mm或中 1/ 3段夹闭 ,并解剖寻找、剥离Heubner回返动脉 ,可避免损伤Heubner回返动脉及其它穿通支 ,最大限度减少术后并发症  相似文献

目的探讨第2足趾腓侧皮瓣移植的应用解剖及临床应用的适应证、手术方法和优缺点等。方法新鲜成人尸体足左、右各10只,经灌注、解剖、测量,了解皮瓣的血供范围和相关血管、神经的外径。临床应用17例修复手部皮肤缺损,了解皮瓣的成活及功能恢复情况。结果灌注剂可达整个第2足趾及相邻趾蹼,测得第2足趾跖趾关节处腓侧固有动脉、趾背静脉分别为(1.0±0.2)mm、(0.9±0.2)mm;临床应用17例移植皮瓣全部成活,手、足部均无感染。经半年以上随访,被修复手指外观、颜色接近正常,供足无疼痛和植皮破溃现象发生。结论第2足趾腓侧皮瓣血供可靠,口径适于吻合,质地好,适于手指皮肤确实的修复。  相似文献

Abstract 10 embalmed cadaver forearms and wrists were dissected to determine the anatomical course of the superficial branch of the radial nerve in the distal forearm. The superficial radial nerve bifurcated in two branches at a mean of 54,7 mm proximal to the radial styloid. From the styloid process of the radius, the mean distance to the closest dorsal branch of the superficial radial nerve was 3,5 mm and the mean distance to the closest volar branch was 9,8 mm. The mean distance between the closest branch of the superficial radial nerve and Lister?s tubercle was 16,4 mm. The crossing point between the nerve and the cephalic vein was located at a mean of 54,3 mm proximal to the styloid process. At the level of styloid process the mean distance between the closest dorsal branch of the superficial radial nerve and the first dorsal compartment was 15,2 mm and between the closest volar branch and the first dorsal compartment 4,4 mm. Detailed knowledge of anatomic characteristics of the superficial branch of the radial nerve may help prevent injury during operations and treat traumatic lesions of the nerve. Because of great variations in the course of the superficial radial nerve we could not define an absolute safe zone for surgical procedures on the distal forearm. Iatrogenic lesions of the superficial radial nerve are described complications of percutaneous procedures. Therefore open surgical approaches are recommended. The role of proteolytic enzymes in the hCG-induced increase in testicular vasopermeability and neutrophil extravasation was studied using protease inhibitors. An intra-testicular injection of hCG together with incubation medium conditioned by polymorphonuclear leucocytes (PMNs) caused a significant increase in vasopermeability and a coincident extravasation of PMN's from the postcapillary venules in the rat testis. When p-aminobenzamidine, a serine protease inhibitor which inhibits urokinase-type plasminogen activator, was administered together with hCG in the incubation medium, both the permeability increase and PMN extravasation were prevented. Aprotinin, another serine protease inhibitor, and Eglin C, a specific neutrophil elastase and cathepsin G inhibitor were, however, without effect. None of these inhibitors caused any non-specific vascular effects in the testis at the concentrations used. These results support the concept that the hCG-induced increase in vasopermeability in the rat testis is related to extravasation of PMNs and suggest that urokinase-type plasminogen activator is involved in migration of these cells through the postcapillary venular walls.  相似文献

目的寻求健侧C7神经根移位治疗全臂丛神经根性撕脱伤手术中尺神经与桡神经的最佳吻合部位。方法在10具20侧福尔马林固定的成人尸体上肢标本上观察桡神经及其肱三头肌肌支的解剖学特征;尺神经的解剖学特征;尺神经不同水平与对侧颈根部的距离。结果桡神经从后侧束发出部位到发出肱三头肌长头的第一支肌支之间的距离为(8.2±1.4)cm,从发出长头的第一个肌支部位到外侧头最后一个肌支发出部位之间的距离为(4.8±0.7)cm。尺神经肘部以上几乎无分支,尺神经在发出部位的直径为(6.7±0.6)mm;在肘部的直径为(6.3±0.5)mm;在腕部的直径为(4.0±0.4)mm;从锁骨下尺神经发出部位到肘部的长度为(29.0±2.6)cm;从锁骨下尺神经发出点到对侧颈根部的距离为(18.0±1.8)cm。结论健侧C7神经根移位修复桡神经,尺神经与桡神经的最佳吻合部位是锁骨下区,在此部位吻合不但能保证肱三头肌功能恢复,而且大大缩短桥接神经的长度。  相似文献
