目的探讨大剂量静脉滴注免疫球蛋白治疗系统性红斑狼疮的疗效。方法选取2015年12月~2018年12月在我院收治的系统性红斑狼疮患者100例,将其随机分为对照组与观察组,每组50例,对照组采用环磷酰胺静脉滴注、强的松口服、甲基强的松龙冲击治疗。观察组在对照组治疗的基础上,使用大剂量静脉滴注免疫球蛋白治疗。比较两组患者的治疗效果。结果观察组患者血小板达高峰平均时间,明显短于对照组,差异有统计学意义(P0.05)。治疗前,两组患者系统性红斑狼疮疾病活动度评分(SLEDAI)、系统性红斑狼疮生活质量量表(SLEQOL)评分比较,差异无统计学意义(P0.05)。治疗后,观察组患者的SLE-QOL评分、SLEDAI评分明显低于对照组,差异有统计学意义(P0.05)。观察组患者4周内血小板上50%以上的为100%,明显高于对照组的76%,差异有统计学意义(P0.05)。两组不良反应发生情况比较无明显差异(P0.05)。结论大剂量静脉滴注免疫球蛋白对系统性红斑狼疮的治疗有积极的作用,能够控制疾病活动度,提高患者的生活质量,不会增加不良反应。 相似文献

Background
There is a lack of studies addressing the occurrence of negative intraoperative findings (that is the absence of intussusception) after an unsuccessful hydrostatic reduction of an ileocolic intussusception. The aim of this study is to determine the incidence of negative intraoperative findings after unsuccessful hydrostatic reduction of ileocolic intussusception.Methods
We conducted a multicentre retrospective study of all children aged 0–18?years treated for ileocolic intussusception from January 1, 2010 to December 31, 2015 in 9 Dutch hospitals. Primary outcome measure was the percentage of children without an intussusception during surgical exploration after unsuccessful hydrostatic reduction.Results
In the study period 436 patients were diagnosed with an ileocolic intussusception. Of these, 408 patients underwent hydrostatic reduction of an ileocolic intussusception. 112 patients (27.5%) underwent surgery after an unsuccessful hydrostatic reduction. In 13 (11.6%) patients no intraoperative evidence of intussusception was found. Patients who underwent surgical intervention after unsuccessful hydrostatic reduction were significantly younger than patients who had a successful hydrostatic reduction; there was no gender difference.Conclusion
A substantial number of children (11.6%) underwent a laparotomy after unsuccessful hydrostatic reduction in whom no intussusception was found intraoperatively. We suggest initiating laparoscopy instead of laparotomy when surgery is necessary.Level of evidence

Aggressive immunosuppressive therapies have been proposed to treat primary angiitis of the central nervous system (PACNS). Here, we report the first successfully stabilized case of childhood, small-vessel PACNS with intravenous immunoglobulin (IVIG) therapy.A 12-year-old boy was admitted to our hospital complaining of recurrent headaches and upper-left homonymous quadrantanopia, since the age of 11 years. Brain computed tomography scans revealed fine calcification in the right temporal and occipital lobes. Brain magnetic resonance imaging scans revealed white matter lesions, with gadolinium enhancement, which waxed, waned, and migrated for 1 year, without immunomodulatory therapies. A cerebrospinal fluid study showed pleocytosis (12 cells per µl). No clinical or serological findings suggested systemic inflammation or vasculitis. Brain angiography was unremarkable. Brain biopsy revealed thickened and hyalinized small vessels, with intramural infiltration of inflammatory cells, which confirmed the diagnosis of small-vessel PACNS. Because the patient developed surgical site infection following biopsy, the administration of monthly IVIG (2 g/kg) was prescribed, instead of immunosuppressive agents. After IVIG therapy, the patient remained stable, except for a single episode of mild radiological exacerbation at 16 months, which occurred when the IVIG interval was expanded. Oral prednisone was added and gradually tapered. At 50 months, his intellectual abilities and motor functions were normal, although he showed residual upper-left homonymous quadrantanopia and post-exercise headache. A temporary headache, associated with the immunoglobulin infusion, was resolved by slowing the infusion rate. PACNS should be treated aggressively to improve prognosis. However, when immunosuppressants are contraindicated, IVIG may be an alternative therapeutic option. 相似文献
