PurposeTo prospectively determine the rate of radial artery occlusion (RAO) in patients undergoing transradial access for intra-arterial interventions.Materials and MethodsSeventy-seven patients undergoing transradial access from August 2019 to March 2021 for 120 intra-arterial procedures (yttrium-90 mapping [n = 39] and radioembolization [n = 38], uterine artery embolization [n = 19], transarterial chemoembolization [n = 10], active bleed embolization [n = 8], angiomyolipoma embolization [n = 4], and other [n = 2]) were enrolled. The average patient age was 59 years ± 13.1 (range, 30–90 years), and 43 (55.8%) of the 77 patients were men. The patients underwent radial artery (RA) palpation, ultrasound evaluation, the Barbeau test, and the reverse Barbeau test prior to and following the intervention. Verapamil, nitroglycerin, and heparin were administered in a total of 114 (95%) of the 120 procedures prior to starting the procedure. The incidence of RAO and radial artery spasm (RAS) was calculated, and univariate logistic regression was performed to analyze the predictors of RAS.ResultsThe preprocedural RA diameter (3.0 mm ± 0.67) was not significantly different from the postprocedural RA diameter (3.0 mm ± 0.65, P = .904). The RAO rate was determined to be 0.8% (1/120), and this artery recanalized within 1 week. Due to the small number of occlusions, statistical analysis of predictors of RAO was not performed. The rate of RAS was 22.7% (27/119). None of the variables tested—including age, sex, RA diameter, initial versus repeat access, operator experience, and artery puncture technique—showed significant prediction for RAS. Patients were seen for follow-up after 111 (92.5%) of the 120 procedures.ConclusionsTransradial access resulted in a <1% rate of RAO. 相似文献

目的 观察超声引导下关节腔内注射重组人Ⅱ型肿瘤坏死因子受体-抗体融合蛋白(益赛普)治疗血友病性关节病(HA)的价值。方法 回顾性分析32例接受超声引导下穿刺关节腔注射益赛普的HA患者,对比观察治疗前及治疗后1个月血友病关节健康评分(HJHS)、视觉模拟评分(VAS),以及超声所示目标关节增生滑膜厚度、血流信号、Melchiorre及中国早期血友病性关节病超声检测(HEAD-US-C)评分,评估其治疗价值。结果 对32例均成功完成超声引导下穿刺关节腔及腔内注射益赛普,共对18例膝关节、7例肘关节及7例踝关节进行治疗。术后未出现感染、出血等并发症。治疗后1个月,目标关节HJHS、VAS、Melchiorre评分、HEAD-US-C评分及增生滑膜最大厚度、平均厚度、血流信号均低于治疗前(P均<0.01)。结论 超声引导下关节腔内注射益赛普治疗HA安全、有效。 相似文献

目的:探讨基于跨理论模型(TTM)协同护理模式在急性心肌梗死(AMI)介入治疗病人中的应用效果。方法:根据随机数字表法将2018年6月—2019年6月收治的88例AMI介入治疗病人分为观察组及对照组各44例,对照组实施常规护理,观察组实施基于TTM的协同护理模式。比较两组干预前及干预6个月后自我效能、疾病管理能力及生活质量改善情况,记录两组不良心脏事件发生率、再入院率及治疗依从率。结果:两组病人干预后自我效能、疾病管理能力、西雅图心绞痛量表(SAQ)总评分比较差异有统计学意义(P<0.05)。观察组病人不良心脏事件发生率(2.27%)、再入院率(4.54%)低于对照组的18.18%、20.45%,差异有统计学意义(P<0.05);观察组病人用药依从率、生活方式管理依从率分别为97.73%和95.45%,高于对照组的75.00%和72.73%,差异有统计学意义(P<0.05)。结论:基于TTM协同护理模式能有效改善AMI介入治疗病人自我效能,提高病人遵医行为及疾病管理能力,减少不良心脏事件的发生,改善病人生活质量。 相似文献

Acute gastrointestinal bleeding is a common medical emergency, accounting for approximately 85,000 admissions in the United Kingdom per annum. It is associated with significant morbidity and mortality. GI haemorrhage is commonly categorized according to source of blood loss; either upper GI (above the ligament of Treitz) or lower GI (below the ligament of Treitz). Rapid assessment, resuscitation and correction of coagulopathy should be undertaken to stabilize the haemodynamically compromised patient and definitive intervention should not be delayed. Clinicians may use of a range of treatment modalities, including endoscopic and interventional radiological techniques in order to get control of haemorrhage, which should be tailored to the site of bleeding and pathology. Where control is not achieved the clinician should consider either repeat intervention, use of alternative haemostatic techniques or different modalities to achieve haemostasis. Rarely is surgery the chosen treatment modality and surgical intervention should only be undertaken where all other measures to control haemorrhage have failed. 相似文献

PurposeTo describe the range of occupational badge dose readings and annualized dose records among physicians performing fluoroscopically guided interventional (FGI) procedures using job title information provided by the same 3 major medical institutions in 2009, 2012, and 2015.Materials and MethodsThe Radiation Safety Office of selected hospitals was contacted to request assistance with identifying physicians in a large commercial dosimetry database. All entries judged to be uninformative of occupational doses to FGI procedure staff were excluded. Monthly and annualized doses were described with univariate statistics and box-and-whisker plots.ResultsThe dosimetry data set of interventional radiology staff contained 169 annual dose records from 77 different physicians and 698 annual dose records from 455 nonphysicians. The median annualized lens dose equivalent values among physicians (11.9 mSv; interquartile range [IQR], 6.9–20.0 mSv) was nearly 3-fold higher than those among nonphysician medical staff assisting with FGI procedures (4.0 mSv; IQR, 1.8–6.7 mSv) (P < .001). During the study period, without eye protection, 25% (23 of 93) of the physician annualized lens dose equivalent values may have exceeded 20 mSv; for nonphysician medical staff, this value may have been exceeded 3.5% (6 of 173) of the time. However, these values did not account for eye protection.ConclusionsThe findings from this study highlight the importance of mitigating occupational dose to the eyes of medical staff, particularly physicians, performing or assisting with FGI procedures. Training on radiation protection principles, the use of personal protective equipment, and patient radiation dose management can all help ensure that the occupational radiation dose is adequately controlled. 相似文献
