The regulation of food intake is a complex mechanism, and the hypothalamus is the main central structure implicated. In particular, the arcuate nucleus appears to be the most critical area in the integration of multiple peripheral signals.Among these signals, those originating from the white adipose tissue and the gastrointestinal tract are known to be involved in the regulation of food intake.The present paper focuses on adiponectin, an adipokine secreted by white adipose tissue, which is reported to have a role in the control of feeding by acting centrally. The recent observation that adiponectin is also able to influence gastric motility raises the question of whether this action represents an additional peripheral mechanism that concurs with the central effects of the hormone on food intake. This possibility, which represents an emerging aspect correlating the central and peripheral effects of adiponectin in the hunger-satiety cycle, is discussed in the present paper.  相似文献

Objective

To assay peripheral inter-ictal cytokine serum levels and possible relations with non-invasive vagus nerve stimulation (nVNS) responsiveness in migraineurs.

Methods

This double-blinded, sham-controlled study enrolled 48 subjects and measured headache severity, frequency [headache days/month, number of total and mild/moderate/severe classified attacks/month], functional state [sleep, mood, body weight, migraine-associated disability] and serum levels of inflammatory markers [inter-ictal] using enzyme-linked immunoassays at baseline and after 2 months of adjunctive nVNS compared to sham stimulation and suitably matched controls.

Results

No significant differences were observed at baseline and after 2 months for headache severity, total attacks/month, headache days/month and functional outcome [sleep, mood, disability] between verum and sham nVNS. However, the number of severe attacks/month significantly decreased in the verum nVNS group and circulating pro-inflammatory IL-1β was elevated significantly in the sham group compared to nVNS. Levels of anti-inflammatory IL-10 were significantly higher at baseline in both groups compared to healthy controls, but not at 2 months follow-up [p?<?0.05]. Concentrations of high-mobility group box-1 (HMGB-1), IL-6, tumor-necrosis factor-α (TNF-α), leptin, adiponectin, ghrelin remained unchanged [p?>?0.05]. No severe device-/stimulation-related adverse events occurred.

Conclusion

2 months of adjunctive cervical nVNS significantly declined the number of severe attacks/month. Pro-inflammatory IL-1β plasma levels [inter-ictal] were higher in sham-treated migraine patients compared to verum nVNS. However, pro- [IL-6, HMGB-1, TNF-α, leptin] and anti-inflammatory [IL-10, adiponectin, ghrelin] mediators did not differ statistically. Profiling of neuroinflammatory circuits in migraine to predict nVNS responsiveness remains an experimental approach, which may be biased by pre-analytic variables warranting large-scale biobank-based systematic investigations [omics].  相似文献

Research on the adipose tissue secretions has undergone a major inflection for a few years. The investigators became aware that the biology of the various cells of the stroma-vascular fraction of adipose tissue was to be taken into account to include/understand the diversity of the secretions. The adipose tissue is a site of production of adipokines (strictly synthesized and secreted by the adipocyte), of pro- or anti-inflammatory cytokines and of acute phase reactant proteins. Cells of the stroma-vascular fraction (monocytes/macrophages, microvascular endothelial cells…) have a secretory importance neglected up to now. The action of both major adipokines (leptin and adiponectin) will be more particularly evoked while considering the diversity of action of the other most current factors. Finally some secreted products more recently discovered (apelin, autotaxin, visfatin.), whose properties are still in the course of exploration, will be also mentioned.  相似文献

Obesity may be described as the new worldwide epidemic, and its serious impact on morbidity and mortality are well known. As more and more women become obese, the reproductive problems associated with obesity present an ever-growing challenge to physicians involved in their fertility care. The spectrum of reproductive problems associated with obesity encompasses a wide range of disorders including infertility problems, miscarriage and pregnancy complications. In this review, we aim to discuss the impact of obesity on the various aspects of female reproductive function with focus on the clinical aspects of fertility problems in obese women. 背景 妊娠期糖尿病(GDM)的发病率居高不下,且在全球普遍流行,在中国GDM发生率高达14.8%,妊娠期高血糖严重威胁母婴健康。目的 探讨GDM孕妇采用基于正念的补充替代疗法的干预效果。方法2021年3—12月采用便利抽样法选取在长沙市中心医院门诊进行产前检查的64例GDM孕妇,采用随机数字表法分为试验组(n=31)和对照组(n=33)。对照组采取常规心理护理、饮食宣教、运动指导,试验组在常规心理护理基础上采用8周基于正念的补充替代疗法。收集患者基线资料,比较两组孕妇干预前后血糖水平及炎性因子、丝氨酸蛋白抑制剂(vaspin)水平,随访新生儿结局指标(出生体质量、随机血糖)。结果 干预后试验组孕妇空腹血糖、餐后1 h血糖、餐后2h血糖、白介素6、白介素8、肿瘤坏死因子α、vaspin水平低于对照组(P<0.05);干预后试验组空腹血糖、餐后1h血糖、餐后2 h血糖、白介素6、白介素8、肿瘤坏死因子α、vaspin低于干预前(P<0.05);试验组GDM孕妇分娩时体质量、分娩时体质指数、妊娠期增重、分娩前糖化血红蛋白,新生儿出生体质量均低于对照组(P<0.05),试验组...  相似文献

妊娠期糖尿病(gestational diabetes mellitus,GDM)是妊娠期的主要并发症之一,随着我国二孩政策的实施、生活水平的提高、婚姻观念的转变,在肥胖、高龄和遗传易感性等高危因素的影响下,GDM的发病率呈逐年上升趋势。目前GDM的具体发病机制尚不明确,可能与胰岛素抵抗及胰岛β细胞功能缺陷有关。GDM孕妇体内糖代谢的紊乱会进一步导致脂质代谢的异常,从而引发不良妊娠结局。其中脂肪因子,如促代谢因子(betatrophin)、趋化素(chemerin)、C1q/肿瘤坏死因子相关蛋白(CTRP9)和瘦素(leptin)等在GDM的相关发病机制中可能起到一定作用。综述脂肪因子在调节糖脂代谢中的作用,及其在GDM病理发生中的作用。  相似文献

Background and aims

Leptin (LPT) is associated with unfavourable cardio-metabolic risk profile. Although a number of studies have found a positive association between LPT and insulin resistance (IR), no observational study has evaluated a prospective association to detect a predictive role of LPT in IR. Therefore, the aim of this study was to estimate the role of LPT on the incidence of IR in an 8-year follow-up of a sample of adult men (The Olivetti Heart Study).

Methods and results

The study included 527 not diabetic men without IR (homeostasis model assessment - HOMA index < 2.77 UI) at baseline. Baseline LPT was significantly and positively associated with HOMA index, body mass index (BMI), waist circumference and blood pressure. At the end of the 8-year follow-up period, a positive and significant association was detected between baseline LPT and changes in HOMA index (r = 0.25, p < 0.01) and incidence of IR (OR: 2.6, 95%CI: 1.9–3.4). This trend was also confirmed after adjustment for potential confounders. In addition, the predictive value of LPT was found in subjects who had not experienced any weight increase over the years, and for normal weight and excess body weight participants, separately.

Conclusions

The results of this prospective study suggest a predictive role of circulating LPT levels on a reduction of insulin sensitivity over time, independently of main potential confounders, in non-diabetic men without IR at baseline. In addition, in normal weight individuals, LPT levels were associated with development of IR.  相似文献
