IntroductionConcerns were raised over an increase in Bell's palsy, herpes simplex and herpes zoster after BNT162b2 vaccination, all are manifestations of herpesviruses reactivation. As herpesviruses commonly reactivate in the oropharynx, we have hypothesized that oropharyngeal shedding of herpesviruses will increase after vaccination.MethodsImmune-competent Adults, excluding those using topical steroids or manifesting symptomatic herpesvirus infection, were sampled before BNT162b2 vaccination and one week after. Herpesviruses 1–7 shedding was tested with a multiplexed PCR.ResultsIn 103 paired samples the prevalence of herpesviruses was similar before and after vaccination: HSV1, 3.9% vs. 5.8% (p = 0.75); HSV2, 0% vs. 1% (p = not applicable, NA); VZV, 0% vs. 0% (p = NA); EBV, 14.6% vs. 17.5% (p = 0.63); CMV, 0% vs. 0% (p = NA); HHV6, 4.9% vs. 7.8% (p = 0.55); HHV7, 71.8% vs. 72.8% (p = 1); any herpesvirus, 73.8% vs. 74.8% (p = 1).DiscussionWe did not find evidence for increased oropharyngeal reactivation of herpesviruses one week after BNT162b2.  相似文献

BackgroundPeriodontitis is associated with the pathogenesis of atherosclerotic plaque, and hypersensitive C reactive protein (hs-CRP) and lipoprotein-associated phospholipase A2 (Lp-PLA2) are the serum biomarkers of the stability of atherosclerotic plaque. Whether periodontitis is associated with the serum level of hs-CRP and Lp-PLA2 of acute ischemic stroke remains unclear.Material and MethodsWe recruited 103 cases with acute ischemic stroke within 7 days after stroke onset. Pocket depth and clinical attachment loss were assessed by oral examination to define the severe periodontitis. Demographic information including gender, age and body weight index, income level, education level, past medical history include smoking history, drinking history, ischemic stroke history, coronary heart disease, hypertension, diabetes and hyperlipidemia were collected, and serum biomarkers including white blood cell (WBC), fibrinogen, total cholesterol (TC), triglyceride (TG), lower density lipoprotein (LDL-C), high density lipoprotein (HDL-C), hs-CRP, HemoglobinA1c (HbAlc), Homocysteine (HCY) and Lp-PLA2 were tested.Results65 (63.1%) cases were diagnosed as severe periodontitis. Severe periodontitis group showed more male, age, drinking history, higher levels of hs-CRP and Lp-PLA2. Multivariate logistic regression showed that severe periodontitis was were significantly associated with hs-CRP (OR = 2.367, 95%CI: 1.182–4.738; P = .015) and Lp-PLA2 (OR = 2.577, 95% CI: 1.010–6.574; P = .048).ConclusionsSevere periodontitis is independently associated with the serum Level of hs-CRP and Lp-PLA2 in patients with acute ischemic stroke. Whether the improvement of periodontitis could decrease the occurrence and re-occurrence of ischemic stroke by stablizating atherosclerotic plaque need be further studied in future.  相似文献

The genetic and antigenic drift associated with the high pathogenicity avian influenza (HPAI) viruses of Goose/Guangdong (Gs/GD) lineage and the emergence of vaccine-resistant field viruses underscores the need for a broadly protective H5 influenza A vaccine. Here, we tested experimental vector herpesvirus of turkey (vHVT)-H5 vaccines containing either wild-type clade 2.3.4.4A-derived H5 inserts or computationally optimized broadly reactive antigen (COBRA) inserts with challenge by homologous and genetically divergent H5 HPAI Gs/GD lineage viruses in chickens. Direct assessment of protection was confirmed for all the tested constructs, which provided clinical protection against the homologous and heterologous H5 HPAI Gs/GD challenge viruses and significantly decreased oropharyngeal shedding titers compared to the sham vaccine. The cross reactivity was assessed by hemagglutinin inhibition (HI) and focus reduction assay against a panel of phylogenetically and antigenically diverse H5 strains. The COBRA-derived H5 inserts elicited antibody responses against antigenically diverse strains, while the wild-type-derived H5 vaccines elicited protection mostly against close antigenically related clades 2.3.4.4A and 2.3.4.4D viruses. In conclusion, the HVT vector, a widely used replicating vaccine platform in poultry, with H5 insert provides clinical protection and significant reduction of viral shedding against homologous and heterologous challenge. In addition, the COBRA-derived inserts have the potential to be used against antigenically distinct co-circulating viruses and future drift variants.  相似文献

Background/objectiveObstructive sleep apnea (OSA) is independently associated with dyslipidemia, a surrogate marker of atherosclerosis. Low-density lipoprotein (LDL)-cholesterol is accepted as a major independent risk factor for cardiovascular disease. However, non-high-density lipoprotein (HDL)-cholesterol is a better marker of atherogenic dyslipidemia and recommended as a target of lipid lowering therapy. We aimed to assess the prevalence of atherogenic dyslipidemia, and relationship between OSA severity and serum LDL-cholesterol and non-HDL cholesterol levels in OSA patients.MethodsWe retrospectively evaluated treatment naïve 2361 subjects admitted to the sleep laboratory of a university hospital for polysomnography. All subjects’ lipid profile including total cholesterol, LDL-cholesterol, HDL-cholesterol, triglycerides, and non-HDL-cholesterol were measured.ResultsOut of 2361 patients (mean age 49.6 ± 11.9 years; 68.9% male, apnea-hypopnea index 36.6 ± 28.4/h), 185 (7.8%) had no OSA and 2176 (92.2%) had OSA. Atherogenic dyslipidemia prevalence was high (57–66%) in OSA patients, and especially increased in severe OSA compared to other groups (p < 0.05). Though total and LDL-cholesterol did not differ between those with and without OSA, non-HDL-cholesterol (p = 0.020), and triglycerides (p = 0.001) were higher and HDL-cholesterol levels (p = 0.018) were lower in OSA patients than non-OSA. Non-HDL-cholesterol was significantly correlated with OSA severity (p < 0.001) and hypoxia parameters (p < 0.01), whereas LDL-cholesterol showed no correlation.ConclusionsAtherogenic dyslipidemia is highly prevalent and non-HDL-cholesterol levels are significantly increased, predominantly in severe OSA patients. Non-HDL-cholesterol but not LDL-cholesterol, is significantly correlated with OSA severity and hypoxia parameters. Therefore, it could be better to use non-HDL-cholesterol, which is a guideline recommended target of lipid therapy, as a marker of atherosclerotic cardiovascular risk in OSA patients.  相似文献

目的探讨细胞色素P450药物代谢酶(CYP)2C19基因分型与氯吡格雷治疗的急性缺血性脑卒中患者再发脑卒中的关系。方法选取2018年5月—2018年12月常州市第一人民医院收治接受氯吡格雷治疗的159例急性缺血性脑卒中患者,检测患者入院后空腹外周血中CYP2C19*2,CYP2C19*3基因分型。对患者进行随访,随访截至2020年2月,观察再发脑卒中的情况,并分析CYP2C19*2,CYP2C19*3基因分型与再发脑卒中关系。结果随访时间14~22个月,平均随访时间为(18.2±1.5)个月,共7例患者失访,共20例(13.2%)患者复发缺血性脑卒中。再发脑卒中患者中CYP2C19*2 GG型及CYP2C19*3 GG型均低于无复发患者(P<0.05),CYP2C19*2 GA型,CYP2C19*2 AA型CYP2C19*3 GA型,CYP2C19*3AA型均高于无复发患者(P<0.05)。Kaplan-Meier法并Log-rank检验结果显示,CYP2C19*2 GG型无复发时间长于AA型,差异有统计学意义(Log-rank=6.759,P=0.034)。CYP2C19*3GG型无复发时间长于AA型,差异有统计学意义(Log-rank x2=8.660,P=0.013)。多因素Cox分析结果显示,糖尿病、氯吡格雷抵抗,CYP2C19*2及CYP2C19*3基因型是再发脑卒中影响因素(P<0.05)。结论在接受氯吡格雷治疗急性缺血性脑卒中患者中,CYP2C19*2及CYP2C19*3突变型再发脑卒中的风险明显增高。  相似文献

背景 随着血糖监测技术的发展,近些年来人们开始使用扫描式葡萄糖监测系统(FGMS)"全景式"地观察2型糖尿病(T2DM)患者的血糖水平,明确FGMS指标与T2DM并发症之间的关系有助于提高其临床应用价值,但目前相关研究较少。 目的 探究佩戴FGMS的T2DM患者葡萄糖在目标范围内时间(TIR)等指标与尿白蛋白/肌酐比值(UACR)的相关性。 方法 选取2019年1月至2021年10月于北京大学人民医院老年科就诊并佩戴FGMS的T2DM患者79例,以尿液检查中UACR是否<30 mg/g将患者分为无白蛋白尿组(n=50)和白蛋白尿组(n=29)。比较两组患者的临床特征、实验室检查指标及FGMS指标等。采用Pearson相关、Spearman秩相关分析探讨TIR、高血糖时间(TAR)与糖化血红蛋白(HbA1c)的相关性。分别采用Pearson相关、Spearman秩相关、偏相关分析探讨FGMS指标与lnUACR的相关性。使用多因素Logistic回归分析探究T2DM患者发生白蛋白尿的影响因素,采用受试者工作特征(ROC)曲线评估TIR对白蛋白尿的预测价值。 结果 白蛋白尿组T2DM病程长于无白蛋白尿组,三酰甘油(TG)、HbA1c、平均血糖(MBG)、TAR、平均血糖标准差(SDBG)、最大葡萄糖波动幅度(LAGE)、平均葡萄糖波动幅度(MAGE)、连续每隔2 h血糖净作用(CONGA2)高于无白蛋白尿组,TIR低于无白蛋白尿组(P<0.05)。Pearson相关、Spearman秩相关分析结果显示,TIR与HbA1c呈负相关(P<0.001),TAR与HbA1c呈正相关(P<0.001)。Pearson相关、Spearman秩相关、偏相关分析结果均表明,TIR与lnUACR呈负相关(P<0.001),MBG、TAR、SDBG、LAGE、MAGE、CONGA2与lnUACR呈正相关(P<0.001)。多因素Logistic回归分析结果显示,TIR>70%〔OR=0.038,95%CI(0.003,0.467)〕是T2DM患者出现白蛋白尿的保护因素(P<0.05),TAR升高〔OR=1.046,95%CI(1.000,1.094)〕是T2DM患者出现白蛋白尿的危险因素(P<0.05)。TIR预测T2DM患者出现白蛋白尿的ROC曲线下面积(AUC)为0.784〔95%CI(0.674,0.894)〕(P=0.003),灵敏度为78%,特异度为83%,最佳切点为69.71%。 结论 在FGMS指标中,TIR>70%是T2DM患者出现白蛋白尿的保护因素,TAR升高是T2DM患者出现白蛋白尿的危险因素。同时,SDBG、LAGE、MAGE、CONGA2等多种反映血糖波动的指标也与UACR密切相关。对TIR水平较低及TAR、SDBG、LAGE、MAGE、CONGA2水平较高的T2DM患者进行FGMS筛查有助于早期识别及预防白蛋白尿的发生、发展。  相似文献

In the present study, effect of exposure of bisphenol A (BPA) and combined exposure of BPA + HSD has been investigated on the glucose homeostasis and associated renal complications in Drosophila. Exposure of 1.0 mM BPA alone induced type 2 diabetes like condition (T2D) in adult male D. melanogaster via oxidative stress. Elevated TGF-β signaling was evident by increased expression of baboon (babo) in BPA exposed organism that stimulated the modulation of extracellular matrix (ECM) component collagen IV resulting in the fibrosis of the Malpighian tubules (MTs). Combined exposure of BPA + HSD (high sucrose diet) resulted in the increased magnitude of T2D and MTs dysfunction parameters. Taken together, the study illustrates that BPA has diabetogenic potential in exposed Drosophila that caused adverse effects on their MTs and combined exposure with BPA and HSD could aggravate the renal tubular dysfunction. The study further suggests the use of Drosophila model to study the environmental chemicals induced diabetes mediated renal dysfunction.  相似文献
