数据挖掘法探讨周珉教授治疗原发性肝癌用药规律及经验总结

[目的]数据挖掘法分析周珉教授治疗原发性肝癌的中医用药规律,探讨相关病机并进行经验总结。[方法]收集2016年2月—2018年5月周珉教授门诊期间治疗原发性肝癌的方剂,运用"中医传承辅助系统(V2.50)"进行数据挖掘,并结合周珉教授临床经验,进行原发性肝癌病机探讨及用药规律分析。[结果]共收集治疗原发性肝癌方剂176首,涉及中药235种,列出方剂中的高频药物及组合规律。[结论]总结原发性肝癌以"湿热痰毒、气阴两伤"为基础病机,"清热化湿、健脾养阴"治则贯穿原发性肝癌治疗始末,同时,根据疾病不同发展阶段及治疗措施,权衡"扶正"与"祛邪"的主次分配,斟酌应用"攻毒散结""行气化瘀"等治法,以达到改善患者生活质量、延缓疾病进展的目的。     

Two cases of Klebsiella pneumoniae liver abscess necessitating liver resection for effective treatment

We report two British cases of liver abscess, due to Klebsiella pneumoniae and associated with synchronous infection elsewhere, which required liver resection for definitive treatment. They illustrate the geographic spread of aggressive K pneumoniae liver infection and demonstrate the importance of early aggressive treatment.     

断指再植手术对手指钝性离断伤的治疗效果研究

目的：探析手指钝性离断伤采用断指再植手术治疗的临床效果。方法整群选择2013年6月—2014年6月期间该院收治的手指钝性离断伤患者59例为研究对象，对其临床治疗资料进行回顾性分析。结果该组的59例患者中，术后断指再成活54例，占91.53%，5例失败，占8.47%，其中手指远端坏死3例，占5.08%，2例术后感染，占3.39%。结论临床上给予手指钝性离断伤患者断指再植手术治疗，可以获得较好的效果，有助于恢复患者的手指功能。     

Pediatric cardiac computed tomography angiography: Expert consensus from the Filiale de Cardiologie Pédiatrique et Congénitale (FCPC) and the Société Française d’Imagerie Cardiaque et Vasculaire diagnostique et interventionnelle (SFICV)

This article was designed to provide a pediatric cardiac computed tomography angiography (CCTA) expert panel consensus based on opinions of experts of the Société Française d’Imagerie Cardiaque et Vasculaire diagnostique et interventionnelle (SFICV) and of the Filiale de Cardiologie Pédiatrique Congénitale (FCPC). This expert panel consensus includes recommendations for indications, patient preparation, CTA radiation dose reduction techniques, and post-processing techniques. The consensus was based on data from available literature (original papers, reviews and guidelines) and on opinions of a group of specialists with extensive experience in the use of CT imaging in congenital heart disease. In order to reach high potential and avoid pitfalls, CCTA in children with congenital heart disease requires training and experience. Moreover, pediatric cardiac CCTA protocols should be standardized to acquire optimal images in this population with the lowest radiation dose possible to prevent unnecessary radiation exposure. We also provided a suggested structured report and a list of acquisition protocols and technical parameters in relation to specific vendors.     

浅谈职业病危害因素苯的识别与化学品安全说明书的关系

新型冠状病毒肺炎（新冠肺炎）对人民的生命健康造成了巨大的威胁。在新冠肺炎疫情下，如何应对肝胆外科肝癌患者的门诊筛查、病房管理以及安全地实施肝癌手术，都是对肝胆外科医师提出的新挑战。我们认为，该病的临床处置应在筛查新冠肺炎的前提下，遵循正规操作流程，做好充分防护。对于需要急诊治疗但无法排除新冠肺炎的肝癌患者，须综合考虑患者的病情严重程度、手术方式及手术室条件，慎重制定个体化治疗方案。整个诊疗过程必须遵从既保证患者的安全及疗效，又要降低医护人员感染风险的原则。
