多力视角下社会资本参与基层卫生服务的动力机制及优化策略研究

社会办医作为我国基层卫生服务的重要组成部分,可以弥补基层卫生机构发展总量与质量不足等问题,对于推进整个基层卫生服务高质量发展也是有着重大裨益。文章从物理学的"力"出发,运用支持力,推力,引力,阻力,摩擦力五种"力"探讨社会资本参与基层卫生服务的动力形成机制,并构建动力斜坡图,针对存在问题,提出加大支持力和引力,合理利用推力,减少摩擦力,消除阻力等优化建议,以期助力社会资本参与基层卫生服务建设平稳进行。     

医院离退休干部参与社会治理现状

通过文献研究、问卷调查、专家访谈、现地调研等方法,分析医院离退休干部参与社会治理的现状及影响因素,探讨社区对离退休干部参与社会治理的需求,提出通过党建引领医院离退休干部参与社会治理的“1235”模式,并给出具体的操作路径,以期为推动医院离退休干部积极参与社会治理提供参考。     

High-Frequency Irreversible Electroporation Using 5,000-V Waveforms to Create Reproducible 2- and 4-cm Ablation Zones—A Laboratory Investigation Using Mechanically Perfused Liver

PurposeTo investigate if high-frequency irreversible electroporation (H-FIRE) treatments can be delivered at higher voltages and with greater energy delivery rates than currently implemented in clinical irreversible electroporation protocols.Materials and MethodsTreatments using 3,000 V and 5,000 V were administered to mechanically perfused ex vivo porcine liver via a single applicator and grounding pad (A+GP) as well as a 4-applicator array (4AA). Integrated energized times (IET) 0.01–0.08 seconds and energy delivery rates 25–300 μs/s were investigated. Organs were preserved at 4°C for 10–15 hours before sectioning and gross analysis using a metabolic stain to identify the size and shape of ablation zones.ResultsA+GP ablations measured between 1.6 cm and 2.2 cm, which did not increase when IET was increased from 0.02 seconds to 0.08 seconds (P > .055; range, 1.9–2.1 cm). Changes in tissue color and texture consistent with thermal damage were observed for treatments with energy delivery rates 50–300 μs/s, but not for treatments delivered at 25 μs/s. Use of the 4AA with a 3-cm applicator spacing resulted in ablations measuring 4.4–4.9 cm with energy delivery times of 7–80 minutes.ConclusionsH-FIRE treatments can rapidly and reproducibly create 2-cm ablations using an A+GP configuration. Treatments without thermal injury were produced at the expense of extended treatment times. More rapid treatments resulted in ablations with varying degrees of thermal injury within the H-FIRE ablation zone. Production of 4-cm ablations is possible using a 4AA.     

北京地方志医学资料挖掘浅析

地方志作为"地方性的百科全书",因其全面性而往往为专业研究如医学研究所忽略,而其中却不乏丰富的医学资料。通过对北京地方志中的医学资料进行挖掘,发现涉及太医院、官办医药机构、社会福利机构、医家、流行病、土产药物、药王庙等内容,并对相关文献进行分析,以展现北京地方志医学资料情况,为后期进一步研究奠定基础。     

皮肤再生医疗技术治疗糖尿病足溃疡疗效分析

目的:分析、总结、探讨皮肤再生医疗技术在临床上治疗糖尿病足溃疡的可行性与有效性。方法:对32例糖尿病足溃疡住院病人采用皮肤再生医疗技术为主、必要时配合植皮的治疗方法进行局部创面修复。结果:32例糖尿病足溃疡应用皮肤再生医疗技术获得修复9例(占28.1％),皮肤再生医疗技术 皮片移植12例(占37．5％),皮肤再生医疗技术 皮瓣修复5例(占15．6％),进行截肢6例(占18．8％)。结论:及时就诊、及时采用皮肤再生医疗技术为主的治疗方法能有效修复溃疡创面、降低糖尿病足截肢率。     

四论音乐心理学在精神病医院中的地位

目的对综合的行为分析的理论和治疗的应用研究段计进行论证。方法音乐心理学是周用桓辩院模式的理论基础。“税书”是医疗、教育、工疗和娱疗的新四结合治疗的灵魂。结果音乐心理行为治疗促进了病房秩序，方便了譬疗工作的开展井有益于精神康复，提高了患者的自信心．强化了正常行为。结论音乐心理行为治疗是公认的行之有效的康复治疗的方法之一。可帮助慢性精神分裂症患者接受正常社会生活和唤醒他们潜在的正常精神功能。     

社区老年患者拔牙的临床特点分析

目的：通过对近两年社区卫生服务中心327例老年病人拔牙临床观察，探讨基层社区老年病人的特点。方法：通过心理、精神和技术方面的配合，安全顺利地完成拔牙术。结果：除5例病人术后感头晕外，其余均顺利完成且病人满意。结论：社区卫生服务中心除具备熟练的技术外，医患之间心理沟通是必不可少的。     

积极主动为贫困人口服务--上海市北站医院平价病房的案例调查

目的:对上海市一所国有民营医院的平价病房进行案例调查,了解运行状况、经验及存在问题。方法:相关人员访谈和现场观察。结果:该院设立了14张床位的平价病房,收治对象是低保户、特困户及经济困难的外来务工人员。对医保病人和自费病人设定不同的减免优惠方案,包括减免50.0%的床位费和10.0%的护理、检查、治疗及手术等劳务费用(药费除外)。以收治急性病为主,住院时间原则上不超过2周。出院病人总费用的实际减免率约为6.0%￣10.0%。结论:创办平价病房的方向应予肯定。是民营机构委托管理医院的一种公益性(非营利性)的表现,对其他公立医疗机构具有促进作用。     

Evidence-based Medicine

Summary:  Evidence-based medicine (EBM) has become a watchword for "the new" medical practice in the new century. Whether it represents a paradigm shift or simply a codification of the scientific method in medicine will be debated for years to come. Regardless of the place of EBM in medicine, this "movement" has served an important role in moving physician practice into the realm of becoming more scientific (albeit empirical) and transparent. There are still many problems to be addressed in the global application of the best evidence for medical practice. These include low-quality studies, delays in implementation of clear-cut improvements, outright fraud and deceit, and wariness on the part of physicians to change their practice. By working to understand the underpinnings of EBM (basic statistical concepts and critical thinking) we can advance the practice of medicine along the moral high road of science.
