薄层扫描法测定通痹灵片中士的宁的含量

目的：建立通痹灵片中士的宁的含量测定方法。方法：测定波长λ5=254nm，参比波长λR=325nm；展开剂为甲苯-丙酮-乙醇-浓氨试液(16：12：1：4)的上层液。结果：士的宁的平均回收率为97．53％；RSD为1．21％。结论：该方法简便、准确，可对产品质量进行控制。     

Successful Treatment of Erdheim-Chester Disease With Coronary Artery Involvement

Erdheim-Chester disease (ECD) is a rare non-Langerhans histiocytosis and inflammatory myeloid neoplasm with poor prognosis. Symmetric long bone osteosclerosis occurs in nearly all patients, but other organs are often involved. Coronary artery involvement is rare, but was encountered in a patient who experienced angina. Radiologic presentation and histologic findings were consistent with diagnosis of ECD. A soft-tissue mass was found surrounding the right atrium, ascending aorta, and all branches of coronary artery. Interferon-alfa treatment was successful. In conclusion, we recommend coronary artery computed tomography angiography for cardiovascular evaluation of ECD and interferon-alfa to treat ECD.     

仝小林运用茯苓、生白术、泽泻利脾湿经验

茯苓、生白术、泽泻是仝小林院士临床常用的利脾湿三味小方。仝小林认为,现今多种疾病以脾虚湿困为基本病机。茯苓可利水渗湿,常用剂量为9~120 g;生白术可健脾燥湿,常用剂量为9~90 g;泽泻渗湿化浊,常用剂量为15~30 g。利脾湿三味小方用于代谢性疾病、内分泌、消化系统疾病等有脾湿之证。     

Techniques of Yang’s arthroscopic discopexy for temporomandibular joint rotational anterior disc displacement

Disc displacement is a common disorder affecting the temporomandibular joint. According to previous publications, the displaced disc can be categorized into pure anterior displacement and rotational displacement (anteromedial and anterolateral). However, the technique of arthroscopy treatment has only been reported for patients with pure anterior disc displacement. In this study, an arthroscopic discopexy for rotational anterior disc displacement was developed and its effectiveness evaluated over 24 months of follow-up. A total of 532 patients (749 joints) with rotational anterior disc displacement, admitted to Shanghai Ninth People’s Hospital between January 2011 and December 2015, were included. The success rate was based on clinical parameters (visual analogue scale (VAS) for pain, maximum inter-incisal opening (MIO), and complications) and radiographic data. The clinical and radiographic data were collected preoperatively and at 1, 6, 12, and 24 months postoperative. The VAS score decreased to 0.73 ± 1.43 following surgery (P < 0.001). A significant improvement in MIO (34.73 ± 6.28 mm) was also detected (P < 0.001). Magnetic resonance imaging showed discs repositioned in both sagittal and coronal images for 714 of the 749 joints, giving a success rate of 95.3%. This study reports an effective and predictable technique of arthroscopic discopexy for rotational anterior disc displacement.     

健脾通腑方序贯联合生长抑素治疗急性胰腺炎对TREM-1水平和胰腺功能的影响分析

【】目的 分析健脾通腑方序贯联合生长抑素治疗急性胰腺炎对髓样细胞触发受体-1(TREM-1)水平和胰腺功能的影响。方法 回顾性选取2015年12月至2018年1月我院收治的急性胰腺炎患者106例,根据治疗方法不同分为对照组和观察组。对照组患者采用常规治疗联合生长抑素治疗,观察组患者加用健脾通腑方序贯治疗,分析两组患者治疗后的临床效果。结果 治疗前,两组患者中医症状积分水平比较,差异无统计学意义(P>0.05)。治疗后,观察组患者中医症状积分水平低于对照组,差异有统计学意义(P<0.05)。治疗前,两组患者TREM-1、内皮素(ET)、C反应蛋白(CRP)、胰腺功能指标水平比较,差异无统计学意义(P>0.05)。治疗后,观察组患者正铜蓝蛋白(CP)、胰岛素(INS)水平高于对照组,TREM-1、ET、CRP、胰蛋白酶原-2(TPS-2)、脂肪酶(LIP)水平低于对照组,差异有统计学意义(P<0.05)。观察组患者中转手术率低于对照组,血淀粉酶恢复时间、腹痛缓解时间及住院时间短于对照组,差异有统计学意义(P<0.05)。结论 健脾通腑方序贯联合生长抑素治疗急性胰腺炎可降低患者TREM-1、ET、CRP水平,改善患者胰腺功能,缩短住院时间。     

降脂通络软胶囊联合瑞舒伐他汀对2型糖尿病合并高脂血症患者效果观察?

目的：观察降脂通络软胶囊联合瑞舒伐他汀治疗2型糖尿病合并高脂血症的临床疗效,并探讨其可能的机制。方法80例2型糖尿病合并高脂血症患者分为联合组与对照组(n=40)。在常规治疗基础上,联合组给予口服瑞舒伐他汀10 mg,1次/天,降脂通络软胶囊100 mg,3次/天口服,对照组予瑞舒伐他汀10 mg,1次/天,口服,连续用药12周。治疗前后分别检测肝功能、肾功能、肌酸激酶,各项血脂指标。结果治疗12周后,联合组在降低血清胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)及超敏 C-反应蛋白(hs-CRP)方面和升高高密度脂蛋白胆固醇(HDL-C)明显优于对照组,差异有统计学意义(P <0.05)。结论瑞舒伐他汀能有效治疗2型糖尿病高脂血症,联合降脂通络软胶囊应用疗效更佳,并且安全性良好。
