Heart rate variability and apnea during sleep in Down's syndrome   总被引:1,自引:1,他引:0  
Autonomic system dysfunction has been reported to occur frequently in patients with Down's syndrome (DS) and is constituted mainly by an imbalance between the sympathetic and vagal systems. The analysis of heart rate variability (HRV) during sleep is a quantitative reliable method for studying such a mechanism, but it has not yet been extensively and adequately applied in DS. In this study, HRV during sleep was evaluated in seven DS patients and in six normal controls, by also controlling for the presence of sleep apnea or arousal. The main results were an increased sympathetic function (low-frequency component of HRV) and a decreased vagal activity (high-frequency component of HRV) in DS with respect to normal controls, during apnea-free periods. Moreover, the presence of apnea, in DS, induced a further significant increase in low-frequency and very low-frequency components of HRV during sleep Stage 2. This study provides additional evidence of a brainstem dysfunctioning in DS, responsible for the abnormal imbalance between the sympathetic and vagal systems and confirms the brainstem involvement already suggested in the literature in order to explain brainstem-auditory evoked potential abnormalities and central sleep apnea in these patients.  相似文献

OBJECTIVE: To investigate the role of substance P (SP) in chronic obstructive pulmonary disease (COPD) or asthma. METHOD: Plasma and sputum samples were obtained from 26 COPD patients and 20 asthmatic patients as well as 12 healthy subjects for measurement of SP content. RESULTS: Patients with COPD had significantly higher levels of SP in the plasma (7.9+/-2.6 pmol/L) and sputum (53.8+/-12.5 pmol/L) than the healthy subjects (3.6+/-1.7 pmol/L and 6.2+/-2.3 pmol/L, respectively, P<0.01). The asthmatic patients also had significantly higher SP levels (8.3+/-3.1 pmol/L and 46.9+/-10.2 pmol/L, respectively) than the healthy subjects, but there was no significant difference between COPD and asthmatic patients (P>0.05). CONCLUSION: SP may be involved in the airway inflammation process in COPD and asthma.  相似文献

应用双相气道内正压呼吸机(BiPAP)对9例慢性阻塞性肺疾病(COPD)康复期和呼吸衰竭稳定期进行临床观察发现,治疗后动脉血氧分压与治疗前相比显著升高(P<0.01);肺活量及用力肺活量轻度升高;1例3次发生急性左心衰、急性肺水肿致急性呼衰患者治疗后,病情很快改善;1例阻塞性夜间睡眠呼吸暂停综合征者应用效果满意。该机应用简便、安全,对心率血压无明显影响,为COPD康复期、急性左心衰兼肺水肿的治疗提供了新途径。  相似文献

目的探讨急性梗阻性化脓性胆管炎的诊断和治疗,提高治愈率,减少死亡率。方法对65例本院收治的急性梗阻性化脓性胆管炎病例进行回顾性分析。结果 62例行手术治疗,3例行内镜括约肌乳头切开取石术,结果55例治愈,7例好转,死亡3例。结论明确诊断后及早手术,合理选择术式,是提高治愈率、降低死亡率的关键。  相似文献

观察56例男性慢性阻塞性肺病(COPD)患者的体检营养指标,并分析其与生活质量的关系,实测体重占理想体重的百分比和生活质量测评表中的日常生活能力,社会活动情况,抑郁心理障碍等项评分有显著相关性(P〈0.05),上臂中点肌肉臂围与日常生活能力和社会活动情况的评分的相关也有统计学意义(P〈0.05)。提示改善COPD患者的营养状况对提高生活质量有重要意义。  相似文献

Abstract: A 63-year-old man was hospitalized because of jaundice and anorexia. An upper gastrointestinal series and hypotonic duodenography revealed circumferential sclerosis and stenosis of the duodenal wall. Endoscopic examination disclosed an ulcer, the upper margin of which was located at the papilla of Vater. The papilla was situated in the base of the ulcer. Endoscopic retrograde cholangiopancreatography disclosed mild dilatation of the common bile, intrahepatic bile and pancreatic ducts, but with neither severe stenosis nor occlusion. Nevertheless, there was some degree of circumferential compression and mild stenosis of the terminal portions of the bile and pancreatic ducts, as potential causes of obstructive jaundice in this patient. Computed tomographic examination of the abdomen revealed a tumorous lesion at the duodenal bulb. Because malignancy in the duodenum could not be ruled out, a pancreatoduodenectomy was performed. Histopathological examination showed a postbulbar duodenal ulcer, associated with inflammation of the papillary orifice and fibrosis of the region near the papilla. There was no evidence of a tumorous lesion. In this case, a postbulbar duodenal ulcer may have caused obstructive jaundice.  相似文献

Objective: We studied the bronchodilatory effect of tiquizium bromide [3-(di-2-thienylmethylene)-5-methyl-trans-quinolizidinium bromide; TQZ], an antimuscarinic agent, on airway smooth muscle in vitro, and also in patients with chronic obstructive pulmonary disease (COPD). Methods: In the first experiment, canine tracheal smooth muscle was used to measure the pA2 of TQZ in vitro. The selectivity of TQZ for muscarinic receptor subtypes was also examined with a radioligand binding assay. Results: The pA2 value of TQZ was 8.75. The pK i values of TQZ for M1, M2, and M3 were 8.70, 8.94, and 9.11, respectively. In an open pilot experiment, the effects of TQZ inhalation were studied in seven patients with COPD (seven men, mean age 68.5 years). TQZ significantly increased forced vital capacity (FVC) and forced expiratory volume in 1 s (FEV1) in a dose-dependent manner. The mean maximum increases in FVC and FEV1 caused by inhaled TQZ (2.0 mg) were 24% and 29%, respectively, and they were measured 1 h after the drug had been inhaled. The FVC and FEV1 were still significantly higher than the control values even 8 h after the drug had been inhaled. No adverse effects were observed after inhalation of TQZ. Conclusion: These data suggest that TQZ is an effective antimuscarinic agent, and that it causes significant bronchodilation in patients with COPD. 本文报告了手术治疗肝外恶性梗阻性黄疸212例。根据B超(BUS)检查结果与术中所见进行对照研究,认为BUS对肝外恶性梗阻性黄疸与内科黄疸的鉴别诊断(正确率99%)、梗阻部位的定位(准确率95.3%),提供了可靠的依据;对恶性梗阻性黄疸的病因诊断仍不理想(阳性率59.4%)。因此BUS检查应列为常规或首选方法,对于确诊或定性有困难者,再选用ERCP或PTC。  相似文献

慢性阻塞性肺疾病(COPD)是一种常见的慢性呼吸系统疾病,对人类健康造成极大威胁。研究发现,COPD患者与健康人群相比,肺部菌群及肠道菌群的组成结构发生了显著改变,导致黏膜屏障功能及机体免疫稳态被破坏,进而加重病情。积极调节肺部、肠道菌群平衡对于干预COPD的发生、发展有着至关重要的作用,但目前关于肺部、肠道菌群及其相互作用机制在COPD中作用的总结和认识还有待进一步阐明。本文就健康人群以及COPD患者肺部、肠道菌群的组成特点及可能的相互作用机制,以及现阶段基于肺部、肠道菌群及其相互作用防治COPD的最新研究成果进行综述,以期为COPD的发病机制、早期诊断、预防与治疗研究提供新思路。  相似文献

脂肪组织除了通过其力学特征或机械功能对周边组织器官产生影响以外,还可以凭借非物理性的内分泌功能通过脂肪因子输出信号方式有效地建立起与呼吸系统等远程器官的互作信息网络,进而参与多种肥胖相关疾病的发生与发展。作为内分泌领域的突破性进展之一,脂肪组织的内分泌功能受到生命科学领域的关注。近年来,多项流行病调查研究揭示了肥胖是造成慢性阻塞性肺疾病(COPD)等呼吸系统疾病肺功能损害重要的危险因素。基于此,本文将分别梳理脂联素、瘦素、内脂素等脂肪因子在COPD中的作用机制,从脂肪组织内分泌功能角度为COPD的防治研究提供新思路。  相似文献
