Low-income parents’ adult interactions at childcare centres

Little is known about the extent and nature of low-income parents’ interactions with other parents and staff at childcare centres, despite the potential for these interactions to provide emotional, informational, and instrumental support. This study interviewed 51 parents at three childcare centres in low-income neighbourhoods in New York City. Twenty-six per cent of parents reported talking with other parents at drop-off and pick-up, and another 35% reported meeting with parents outside the centre in addition to talking with them at the centre. Parents’ extent of interaction was related to how long they spent at drop-off and pick-up, their participation in centre activities, and their sociability in general. All parents reported interacting with teachers and administrators, and described them more often than other parents as good sources of information and advice. We discuss the implications for parents and centre-based childcare providers.     

Together and alone a study of interactions between toddlers and childcare providers during mealtime in Norwegian childcare centres

The study investigated the quality of interactions between childcare providers and toddlers during a lunch in childcare centres. Meals in childcare centres are semi-structured adult-led situations where the children not only eat, but are also provided with opportunities for implicit learning and interactions. Participants were 13 toddlers aged about 18 months in 11 different childcare centres in a Norwegian municipality. Video recordings were analysed for organization and structure, relational climate and childcare provider–child interactions, weighting provider sensitivity, language support and facilitated exploration. Findings indicated good quality regarding the organization of meals and relational climate in about half of the centres. Sensitivity, language support and facilitated exploration showed low quality across the different centres and are a cause for concern. Implications of the findings are discussed.     

American Society of Nephrology Quiz and Questionnaire 2013: Transplantation

The nephrology quiz and questionnaire remains an extremely popular session for attendees of the Annual Meeting of the American Society of Nephrology. As in past years, the conference hall was overflowing with interested audience members. Topics covered by expert discussants included electrolyte and acid–base disorders, glomerular disease, ESRD/dialysis, and transplantation. Complex cases representing each of these categories along with single best answer questions were prepared by a panel of experts. Before the meeting, program directors of United States nephrology training programs answered questions through an Internet-based questionnaire. A new addition to the nephrology quiz and questionnaire was participation in the questionnaire by nephrology fellows. To review the process, members of the audience test their knowledge and judgment on a series of case-oriented questions prepared and discussed by experts. Their answers are compared in real time using audience response devices with the answers of nephrology fellows and training program directors. The correct and incorrect answers are then briefly discussed after the audience responds, and the results of the questionnaire are displayed. This article recapitulates the session and reproduces its educational value for the readers of CJASN. Enjoy the clinical cases and expert discussions.     

Carcinoma-associated fibroblasts: orchestrating the composition of malignancy

The tumor stroma is no longer seen solely as physical support for mutated epithelial cells but as an important modulator and even a driver of tumorigenicity. Within the tumor stromal milieu, heterogeneous populations of fibroblast-like cells, collectively termed carcinoma-associated fibroblasts (CAFs), are key players in the multicellular, stromal-dependent alterations that contribute to malignant initiation and progression. This review focuses on novel insights into the contributions of CAFs to disease progression, emergent events leading to the generation of CAFs, identification of CAF-specific biomarkers predictive of disease outcome, and recent therapeutic approaches aimed at blunting or reverting detrimental protumorigenic phenotypes associated with CAFs.
