The Post Therapy Breast

Both systemic and local therapy, for conditions of the breast and unrelated to it, may produce manuno-graphic changes. Some of these are characteristic, such as the pattern of scarring seen in reduction mammo-plasty. In many other instances, however, the changes produced overlap features commonly seen in malignancy. A knowledge of the timing, natural history and spectrum of these changes will aid mammographic interpretation.     

血液流变学因素对缺血心肌冠脉流量的影响

选用长白猪８只于麻醉下开胸，于在体猪心上探讨心肌缺血时冠脉仍保留有部分扩张储备，而未被充分利用的机理。实验结果表明：降低左冠状动脉前降支（ＬＡＤ）内压至４．６７ｋＰａ时，由ＬＡＤ分别输入腺苷、山莨菪碱、生理盐水（２ｍｌ／ｍｉｎ），皆能诱发出明显的ＬＡＤ流量增加（Ｐ＜０．０５）。在维持ＬＡＤ内压不变，并持续分别输入上述三种溶液的同时，从ＬＡＤ远端取血测血液流变学指标，结果显示，输入生理盐水和山莨菪碱使红细胞压积降低，血浆粘度和全血粘度下降；心肌缺血时，冠脉内输注药物所诱发出的“冠脉扩张储备”部分是由于此种给药方式所引起的血液稀释及血液粘度下降的结果。它提示在冠脉狭窄时，血液粘度对狭窄远端的心肌血供可能是一个相当重要的因素，降低血液粘度可能并不亚于扩血管药物的作用。     

Referral pattern of patients with oral mucosal lesions in The Netherlands

The referral pattern of 140 Dutch patients with oral mucosal lesions, who had been referred to a Department of Oral & Maxillofacial Surgery and Oral Pathology, shows that patients with oral mucosal lesions consult the dentist as often as the family doctor as the first source of help or information. Furthermore, family doctors were much more used to refer patients with oral mucosal disease to medical specialists rather than to the dentist or the oral and maxillofacial surgeon.     

额颞除皱术辅以隆额隆颞术

对既有面部老化，又有因局部发育不良，面骨体积缩小，畸形等伴有“额小颞窄”和“两侧不对称”等症状的患者，采用额颞除皱术辅以隆额隆颞手术的方法，既可使额颞部皱纹消除，又可达到整形的目的。本组共４８例，其中应用硅橡胶隆颞２８例，额肌瓣隆额１０例，额肌瓣隆额加颞筋膜瓣隆颞６例，额肌瓣隆额加硅橡胶隆颞４例。随访最长５个月，疗效满意。因此，对伴有额颞部外形不满意及面部老化的患者，应以此术为首选方法。     

Development of tyrosine hydroxylase-, dopamine- and dopamine β-hydroxylase-immunoreactive neurons in a teleost, the three-spined stickleback

The development of catecholaminergic neuronal systems in the brain of a teleost, the three-spined stickleback, was studied through embryonic to early larval stages by immunocytochemistry using specific antibodies against dopamine, tyrosine hydroxylase and dopamine β-hydroxylase. By analysing the spatiotemporal patterns of development for the catecholaminergic nuclei, possible homologies with nuclei in amniote brains have been identified.

The noradrenergic neurons in the isthmus region of the rostral rhombencephalon originate in the same manner as the A4–A7 + subcoeruleus group in mammals. Their developmental characteristics show the largest similarities with the subcoeruleus group of birds and mammals, although some features are shared with developing A6 (locus coeruleus) neurons.

Catecholaminergic neurons never appear during development in the ventral mesencephalon of the three-spined stickleback. A group of large dopaminergic neurons that accompany the cerebrospinal fluid (CSF)-contacting neurons follows the border between the hypothalamus and the ventral thalamus into the caudal hypothalamus, where they are continuous with the dopaminergic neurons in the posterior tuberculum. They are thus topologically comparable with the dopaminergic neurons of the zona incerta in mammals.

The dopaminergic CSF-contacting neurons that line the median, lateral and posterior recesses of the third ventricle do not contain tyrosine hydroxylase-immunoreactivity at any developmental stage. This indicates that they take up and accumulate exogenous dopamine or -dihydroxyphenylalanine, and do not synthesize dopamine from tyrosine at any developmental stage. Tyrosine hydroxylase-immunoreactive neurons appear in the pineal organ on the day of hatching (120 h post-fertilization). They were still observed in 240-h-old larvae, but are absent in the pineal organ of adult sticklebacks.

The initial appearance and subsequent differentiation of catecholaminergic neurons in the stickle-back embryo follow essentially the same spatial and temporal pattern as in amphibian, avian and mammalian embryos. This observation supports the hypothesis that morphologically, topologically and chemically similar monoaminergic neurons in different vertebrate classes are homologous.     

胰腺分裂与慢性胰腺炎的病因学关系及发病机制

目的:探讨胰腺分裂(PD)在慢性胰腺炎病因学中的作用及其发病机制。方法:将32只犬随机分为4组(每组8只)。Ⅰ组:部分结扎背-腹胰管间的交通支。Ⅱa组:切断并结扎交通支。Ⅱb组:在背胰管注入小乳头前2mm处将其切断并结扎断端。Ⅲ组:假手术对照组,除不结扎交通支外,其余操作同上。于手术前检测各组犬血清磷脂酶(PLA2)和淀粉酶(Ams)活性及背、腹胰管基础压并在胰泌素激发后15、30、45、60和90分钟各测压一次。于术后180天再行胰胆管测压和造影并观察背、腹胰和十二指肠乳头的病理改变。结果:⑴Ⅰ、Ⅱa组和Ⅱb组术后5-80天血清PLA2和Ams活性显著升高。⑵处死时Ⅰ、Ⅱa组腹胰管和Ⅱb组背、腹胰管压力在激发后30-60分钟显著高于注药前(P<0.01);在60分钟后,前两组压力恢复到术前水平(P>0.05),而Ⅱb组仍较高(P<0.05),该组压力在90分钟后恢复正常。⑶光镜:Ⅱb组犬背、腹胰小叶间或/和胰管周围显著纤维组织增生,腺细胞结构破坏和炎细胞浸润。Ⅰ和Ⅱa组腹胰见轻度上述改变。⑷电镜:Ⅰ、Ⅱa组腹胰和Ⅱb组背、腹胰腺细胞粗面内质网脱颗粒、融合和扩张。酶原颗粒减少,线粒体肿张。Ⅰ和Ⅱa组背胰及Ⅲ组背、腹胰未见异常。结论:PD是慢性胰腺炎的病因学因素之一,其引发胰腺炎的机制是在胰液分泌的高峰期副乳头功能性梗阻。     

我对子午流注阴阳盛衰转换的认识

阐述了时相与阴阳的关系，阴阳昼夜的盛衰转换与经脉循行、气血流注密切相关；对传统流注说和任督为主的阴阳循环说进行了分析，认为子午流注阴阳盛衰转换在妇科学中有重要意义，子午流注所指出的4个较重要的时期与月经周期中的4期(行经期、经后期、经间排卵期、经前期)相关。重视研究日相阴阳太极钟的变化，将能更加深入地认识月经周期中阴阳消长转化的规律变化，从而推动中医妇科学向前发展。     

小包装全氟丙烷气体动力学实验研究

为检验塑料小包装全氟丙烷气体（C3F8）在不同包装和储存方法时浓度变化，将装有5～7mlC3F8的聚氯乙烯小袋，根据不同储藏温度和外包装方法随机分成四组：（1）22℃聚乙烯外包装，（2）36℃聚乙烯外包装，（3）22℃铝箔真空外包装，（4）-29℃聚乙烯外包装；每一组C3F8小袋气体存放一定时间后，应用气相色谱分析方法进行浓度测量。结果：第3组C3F8浓度最高和稳定，第2组浓度随放置时间降低最明显，第4组是临床应用气体的储藏和包装方法，其30天样本浓度和第3组相等，但放置一年时浓度降低。结果显示：塑料小包装C3F8予以铝箔真空外包装是一种可行的方法，利于C3F8运输和普及；聚乙烯外包装的C3F8，应放在-29℃保存，时间不超过一年。     

造血系统恶性肿瘤患者血清sIL—2R检测的临床意义

使用ＥＬＩＳＡ双抗夹心法对临床４３例造血系统恶性肿瘤患者及２０例健康体检人员的血清ｓＩＬ－２Ｒ进行检测，并对ＡＬＬ和ＡＮＬＬ复发与未复发患者的血清ｓＩＬ－２Ｒ水平进行了比较分析。实验结果表明，血清ｓＩＬ－２Ｒ水平的升高是造血系统恶性肿瘤的重要标志之一；测定血清ｓＩＬ－２Ｒ水平对造血系统恶性肿瘤的辅助诊断、疗效观察及预后估计具有重要意义。
