Die chirurgische Behandlung des Magenfrühcarcinoms

Zusammenfassung An der Chirurgischen Klinik Köln-Lindenthal wurden zwischen 1968 and 1983 94 Patienten, an der Klinik and Poliklinik für Allgemein- and Abdominalchirurgie Mainz zwischen 1972 and 1986 74 Patienten wegen eines Magenfrühcarcinoms operiert. Überlebensraten und Rezidivverhalten wurden abhängig von der operativen Therapie untersucht. In beiden Kollektiven war die Prognose des Mucosacarcinoms unabhängig von der durchgeführten Therapie günstig. Beim Submucosacarcinom war nach radikalchirurgischem Vorgehen die Häufigkeit von Rezidiven geringer und die Überlebensraten signifikant günstiger als nach alleiniger 2/3-Resektion. Die Ergebnisse sprechen für die Notwendigkeit eines radikalchirurgischen Vorgehens mit Dissektion der N2-Lymphknoten zumindest beim Submucosacarcinom des Magens.

Surgical treatment of early gastric carcinoma

Summary At the department of surgery at the university hospital in Cologne 94 patients suffering from early gastric cancer of the stomach were operated in the years between 1968 and 1983. From 1972 till 1987 the department of surgery at the university hospital Mainz has had 74 patients. Survival rates and times without recurrence were calculated dependent on the surgical treatment carried out. In both collectives the prognosis for the patients with mucosal carcinoma was good no matter which operation was carried out. In the case of submucosal infiltration the frequency of recurrence was smaller and the survival rates significantly higher in those patients who subjected to a cancer operation than in those on whom only a resection of two third of the stomach was performed. The results indicate a radical approach in the treatment of early gastric cancer. At least in the case of submucosal carcinoma a dissection of the N2 lymph nodes is advisable.

     

后房型人工晶体植入术后早期并发症临床分析及处理

报告468例500眼后房型人工晶体植入术后2周内出现的并发症发生率,其中角膜水肿73.6％,色素膜炎症反应88.4％,瞳孔变形12.8％,人工晶体表面渗出膜3.8％,后囊混浊13.8％,短暂性高眼压5.2％,晶体皮质残留5.8％,人工晶体偏位2.6％,前房出血3％。提高术后视力的关键是提高手术技巧以减少并发症的发生。     

Inhibition of Complement Regulation Is Key to the Pathogenesis of Active Heymann Nephritis

Crry (complement receptor 1–related protein/gene y) is a key cellular complement regulator in rodents. It is also present in Fx1A, the renal tubular preparation used to immunize rats to induce active Heymann nephritis (HN), a model of membranous nephropathy. We hypothesized that rats immunized with anti-Fx1A develop autoantibodies (auto-Abs) to Crry as well as to the megalin-containing HN antigenic complex, and that anti-Crry Abs promote the development of injury in HN by neutralizing the complement regulatory activity of Crry. Rats immunized with Fx1A lacking Crry remained free of proteinuria and glomerular deposits of C3 during a 10-wk follow-up despite typical granular immunoglobulin (Ig)G deposits in glomeruli. Anti-Fx1A auto-Abs were present in their sera at levels that were not different from sera pooled from proteinuric rats with HN induced with nephritogenic Fx1A. Passive administration of sheep anti-Crry Abs to rats immunized with Crry-deficient Fx1A led to proteinuria and glomerular C3 deposition, which were not seen in such rats injected with preimmune IgG, nor in rats with collagen-induced arthritis injected with anti-Crry IgG. To directly examine the role of Crry in HN, rats were immunized with Crry-deficient Fx1A reconstituted with rCrry. This led to typical HN, with 8 out of 15 rats developing proteinuria within 14 wk. Moreover, the extent of glomerular C3 deposition correlated with proteinuria, and anti-Crry Abs were present in glomerular eluates. Thus, Crry is a key nephritogenic immunogen in Fx1A. Formation of neutralizing auto-Abs to Crry impairs its function, leading to unrestricted complement activation by Abs reactive with the HN antigenic complex on the epithelial cell surface.     

大鼠烧伤后急性期蛋白mRNA的表达

分析了不同烧伤程度大鼠肝、脾、肾和骨骼骨内４种急性期蛋白，即ａ１－酸性糖蛋白、ａ１－抗胰蛋白酶、白蛋白和转铁蛋白（Ｔｒｆ）ｍＲＮＡ水平的动态变化特征，结果表明：（１）肝脏是合成各种ＡＰＲ的主要场所；（２）即使是小面积的烧伤也可以诱发急性期反应。（３）烧伤后血清Ａｌｂ水平下降是急性期反应的特征，但在ｍＲＮＡ水平上并不下降而是增加。

Flt3L免疫扩增恒河猴外周血树突状细胞前体(pDC1/pDC2)的研究

目的 探讨Flt3L对恒河猴外周血树突状细胞前体(pDC1和pDC2)的免疫扩增作用。方法 健康猴艾滋病毒SIV阴性的恒河猴每日给予rhFlt3L，100μg／kg体重皮下注射，于第11天在氯胺硐全麻下采集静脉血，用Ficoll-Hypaque梯度离心法提取外周血单核细胞，利用与恒河猴有交叉反应的人单克隆抗体以及三色流式细胞仪分离pDC1和pDC2,并与未经Flt3L作用的pDC1和pDC2在数量和细胞表现型上进行对比。结果 经Flt3L扩增的恒河猴外周血pDC1和pDC2数量分别增加7倍和4倍，且细胞表现型和形态不发生改变。结论Flt3L对恒河猴外周血pDCl和pDC2具有免疫扩增作用。     

Gonadotrophin dynamics during reproductive life.

OBJECTIVE: To evaluate the follicle-stimulating hormone (FSH) and luteinizing hormone (LH) levels in early follicular phase throughout the reproductive years. METHOD: FSH and LH concentrations were determined by radioimmunoassay (RIA). Linear and polynomial regressions were carried out considering basal FSH as the dependent and age as the independent variable. RESULTS: FSH levels increased throughout the reproductive years (P<0.025). A positive correlation between age and basal FSH levels was detected (P<0.05). The Pearson squared coefficient of r(2)=0.889 was obtained. Using polynomial regression, the inclination of the parabole (Y=7.97-0.009x+0.057x(2)) was 0.359 and the generalized correlation coefficient was r=0.795. The goodness of fit analysis showed that the parabole may better represent the phenomenon (F=4.7; P<0.05). The LH levels remained constant, increasing only beyond 40 years of age. CONCLUSION: The FSH levels rose in a nonlinear way during the reproductive life and the LH concentrations increased discreetly only in patients over 40 years of age.     

急性心肌梗塞错误溶栓治疗12例分析

自1993年12月至1995年8月，对155例急性心肌梗塞（AMI）病人进行溶栓治疗，12例发生误溶(7．7％）。其中早期复极综合征4例，室壁瘤2例，心肌炎1例，心肌病1例，完全性左束支阻滞1例，间歇性左前分支阻滞互例，其它2例。本文分析了误溶病例与AMI的鉴别要点，并提出了减少误溶，提高诊疗水平的措施。
