椎弓根钉内固定治疗胸腰椎骨折   总被引:2,自引:0,他引:2  
目的:探讨脊柱胸腰段骨折椎弓根钉内固定治疗效果。方法:临床收治脊柱胸腰段骨折72例,均采用后路减压,Dick或RF系统钉内固定手术治疗。比较手术前后骨折复位及神经功能恢复情况。结果:术后椎体高度完全或基本恢复占95.8%,神经功能有Frankel一级以上改善,不完全截瘫占90%,完全截瘫占53.1%。随访3—38个月,无螺钉折断、后退及脊柱矫正度丢失。结论:开放复位后路椎弓根钉内固定是治疗胸腰椎骨折的有效方法。  相似文献

目的 通过107例胸腰椎骨折伴脊髓损伤病例的治疗,探讨前、后路手术适应证的选择。方法 31例经前路减压植骨,采用Z-Plate等内固定。76例经后路采用短节段椎弓根钉内固定。结果 两组均获得了满意的疗效。结论 选择前或后路手术,应综合考虑脊髓损伤程度、手术时机与技巧、骨折的部位、骨折类型和影像结果等,才能确实做到减压、恢复脊柱序列和坚强固定的目的。  相似文献

HLA-B27相关的前葡萄膜炎眼后节并发症的临床特点   总被引:1,自引:0,他引:1  
目的探讨HLA—B27阳性的前葡萄膜炎眼后节并发症的临床特点及治疗。方法2002年1月至2005年10月就诊的前葡萄膜炎患者采用流式细胞术进行外周HLA-B27的检测,筛选出71-例82眼HLA-B27阳性前葡萄膜炎患者,分为两组:实验组为眼后节受累的患者24例31眼,对照组为眼后节未受累的患者47例51眼。另有HLA—B27阴性前葡萄膜炎患者74例116眼作参考。所有患者均进行裂隙灯、眼底镜等常规眼科查体,部分行眼底荧光血管造影。分析了HLA—B27相关的前葡萄膜炎眼后节并发症的临床特点。采用x^2检验及t检验进行统计学分析。结果71例82眼HLA—B27阳性前葡萄膜炎中有24例31眼占37.8%出现眼后节并发症者,包括:黄斑囊样水肿12例15眼占48.4%,视乳头水肿7例8眼占25.8%,而玻璃体炎最多见为21例27眼占87.1%,8例10眼占32.3%患者存在两种或两种以上的眼后节表现。74例116眼HLA—B27阴性者有8例12眼占10.3%有眼后节表现。实验组中发生前房成型纤维素渗出31眼中18眼占58.06%和积脓31眼中9眼占29.03%明显高于对照组(前房纤维素渗出51眼中3眼,5.88%,X^2=27.56,P〈0.01;前房积脓51眼中1眼,1.96%,X^2=13.20,P〈0.01);实验组中伴发相关全身性疾病24例中有16例占66.67%的几率高于对照组(47例中有17例,31.66%,X^2=5.94,P〈0.05)。出现眼后节并发症的患者经全身及局部应用糖皮质激素治疗,31眼中有11眼最佳矫正视力≥1.0。结论HLA-B27阳性前葡萄膜炎易出现眼后节并发症,HLA—B27相关的前葡萄膜炎眼后节受累的患者发生相关全身性疾病和前房纤维素渗出与积脓的几率较高。  相似文献

Day care is generally accepted in anterior segment eye-surgery. In the Rotterdam Eye Hospital this option was also considered for posterior segment surgery. We were interested in the opinion, of patients on this matter and therefore asked patients, who were admitted for posterior segment eye-surgery, to answer a questionnaire. The major question was: ‘If your physician had given his permission, do you think it would have been possible for you to go home on the evening after surgery?’. Other questions evaluated problems in organising assistance at home and transportation to the out-patient clinic as well as circumstances after the operation, such as pain, nausea, dizziness and anxiety. Eighty-one out of 87 patients responded: 56% answered ‘eyes’ and 44% ‘no’ to the major question. Relating the answer to the major question to medical data and to answers to the other questions, we found organizational problems at home and anxiety to have a statistical significant relation with a negative answer. Clinical factors like age, American Society of Anesthesiologists (ASA)-class, diabetes mellitus (including insulin-dependant), type of anesthesia, time of the day the surgery was finished, duration of surgery, pain, nausea or dizziness were not signficantly related. The number of patients involved in this study, however, is too small to draw conclusions on specific subgroups of patients.  相似文献

Electron microscopy and immunocytochemistry with a monoclonal antibody against parvalbumin (PV) were combined to analyze the distribution and morphology of PV-immunoreactive (PV-IR) neurons and the synaptology of PV-IR processes in the principal sulcus of the macaque prefrontal cortex. Parvalbumin-IR neurons are present in layers II-VI of the macaque principal sulcus (Walker's area 46) and are concentrated in a band centered around layer IV. PV-IR cells are exclusively non-pyramidal in shape and are morphologically heterogeneous with soma sizes ranging from less than 10 microns to greater than 20 microns. Well-labeled neurons that could be classified on the basis of soma size and dendritic configuration resembled large basket and chandelier cells. A novel finding is that supragranular PV-IR neurons exhibit dendritic patterns with predominantly vertical orientations, whereas infragranular cells exhibit mostly horizontal or oblique dendritic orientations. PV-IR cells within layer IV exhibit a mixture of dendritic arrangements. Vertical rows of PV-IR puncta, 15-30 microns in length, resembling the "cartridges" of chandelier cell axons were most dense in layers II, superficial III, and the granular layer IV but were not observed in the infragranular layers. Cartridges were often present beneath unlabeled, presumed pyramidal cells. PV-IR puncta also formed pericellular nests around pyramidal cell somata and proximal dendrites, suggestive of basket cell innervation. PV-IR axons were occasionally observed in the white matter underlying the principal sulcus. Electron microscopic analysis revealed that PV-IR somata and dendrites are densely innervated by nonimmunoreactive terminals forming asymmetric (Gray type I) synapses as well as by fewer terminals forming symmetric (Gray type II) synapses. The majority of terminals forming symmetric synapses with PV-IR post-synaptic structures were not immunolabeled; however, some of these boutons did contain PV-immunoreactivity. PV-IR boutons exclusively form symmetric synapses and heavily innervate layer II/III pyramidal cells. PV-IR axon cartridges formed numerous axo-axonic synapses with the axon initial segments of pyramidal cells 15-20 microns beneath the axon hillock and also terminated on large axonal spines of the initial segment. Furthermore, we failed to observe a mixture of PV-immunoreactive and non-immunoreactive boutons composing a single axon cartridge. Pyramidal cell somata and proximal dendrites were also heavily innervated by PV-IR boutons forming symmetric synapses, again, consistent with basket cell innervation. In addition, PV-IR axon terminals frequently formed symmetric synapses with dendritic shafts and spines of unidentified neurons.(ABSTRACT TRUNCATED AT 400 WORDS)  相似文献

目的 对双凤尾钢板治疗胸腰椎爆裂骨折进行生物力学评价和临床应用观察。方法 取成年男性尸体脊柱标本(T12-L2),制成8具L1椎体爆裂骨折模型。按实际手术方法放置双凤尾钢板。对试件分别进行轴向和弯曲扭转加载测试。临床观察应用双凤尾钢板治疗胸腰椎爆裂骨折的效果。结果 压缩实验和弯曲扭转实验中,各点应变值与载荷均呈线性关系。当轴向载荷达到600N或扭矩达到600N·cm时,这种线性关系未改变。弯扭矩与模型两端之间相对扭转角的增加呈线性关系。弯扭矩达到600N·cm时,扭转角只有6.26°。临床观察表明双凤尾钢板固定可靠。结论 双凤尾钢板固定具有良好的稳定性,表现出高弹性,符合生物学固定的原则。值得临床推广应用。  相似文献

We sequenced the complete S segments of hantaviruses detected from 12 HPS patients living in southern of Brazil. Samples were obtained from patients diagnosed in different years, in distinct areas, and with a broad spectrum of clinical signs. Despite these differences, all the S proteins of hantavirus from Paraná were identical, except for one amino acid substitution. Phylogenetic analyses of the complete S segment nucleotide and amino acid sequences indicated that hantaviruses from Paraná form a distinct clade from those circulating in South and North America. Other hantaviruses from Brazil were not placed in the same clade. The Oligoryzomys nigripes-associated strains ITA37 and ITA38 from Paraguay were found to belong to the same clade as the hantaviruses from Paraná. Paraguay and Paraná state are located at the same latitude and some ecosystems are similar in both places. The geographic position and common rodent hosts could explain this phylogenetic relationship.  相似文献

目的:从生物力学角度了解融合内固定对腰椎邻近节段的影响。方法:9具新鲜腰1~骶椎尸体标本分别近、远端固定,在脊柱三维运动实验机上施以8Nm纯力偶矩模拟人体行屈伸、左右侧弯及旋转活动,观察L3~4、L4~5、L5~Sl节段运动范围(ROM);随后进行各种模拟手术并安装内固定,依次测定CD内固定(CD)、CD加椎体间植骨(CD-骨块)、CD加TFC(CD—TFC)状态下各节段ROM值并进行统计学分析。结果:①各融合术式均可使固定节段达到即刻稳定性,L4~5节段ROM值明显减小;②L3~4节段在屈伸、侧弯和旋转时有明显位移增加,L5S1节段在旋转时ROM值明显增加,屈伸和侧弯时ROM值改变无统计学意义。结论:脊柱融合内固定术后,相邻节段位移增加、运动模式改变,易继发不稳和退变。  相似文献

This study tested the hypothesis that, as compared to whole-body bioelectrical impedance (BI) analysis, segmental BI analysis can estimate lean body mass (LBM) more accurately in a population with a large difference in muscularity. In addition to whole-body BI, which determines impedance (Z) between the wrist and ankle, two segmental BI analyses which determine the Z value of every body segment in each of (1) the arms, legs and trunk (distal BI) and (2) the upper arms, upper legs and trunk (proximal BI) were applied to a group of 125 male athletes and 75 non-athletes. The subjects were divided into validation and cross-validation groups. Simple and multiple regression analyses were applied to (length)2/Z (BI index) values for the whole-body and each body segment, to develop the prediction equations of LBM measured using air-displacement plethysmography. In the validation group, the SE of estimation was similar in the whole-body (3.4 kg, 5.4%), distal (3.4 kg, 5.5%) and proximal BI (3.3 kg, 5.2%) analyses. However, the whole-body and distal BI analyses produced systematical errors in the estimates of LBM. Moreover, the residuals in the two methods significantly (P<0.05) correlated with the ratios of BI indices of the upper arms and upper legs to those of the arms and legs, respectively, calculated as variables approximating the relative development of lean tissues at the proximal area of limbs. On the other hand, the proximal BI analysis was validated and cross-validated. Thus, the accuracy of estimating LBM was similar in the whole-body and the two segmental BI analyses. However, the prediction equations derived from the use of the whole-body BI index and a combination of the arms, legs and trunk BI indices produced a systematical error relating to the difference between the limb segments in lean tissue development.  相似文献

目的:明确脊柱胸腰段冠状、矢状和横断面解剖学特点,探索薄层断层切片在观测各髓节及脊神经根走行过程的临床应用价值。方法:采用40具成人脊柱胸腰段标本经改良火棉胶包埋法处理,制作16具冠状、8具矢状和16具横位的0.25mm厚连续切片,观察脊神经根走行于侧椎管和椎间管不同区段的解剖关系和椎间孔韧带分布特征,并直接测量相关结构参数。结果:观测了脊柱胸腰段A、B、C、D经4区横断层面、经椎间管内口矢状层面、经两侧椎弓根中心冠状层面的形态和相关参数。结论:三种方位断层切片可较好显示诸多结构位置关系,对脊髓或脊神经源性疾病诊疗和正确辨认手术视野所见具有重要价值。  相似文献
