Reconstruction with distally based dorsalis pedis fasciocutaneous flap for the coverage of distal toe-plantar defects

Reconstruction of small defects in the distal portion of the foot has always been a difficult problem. A case involving a young man with a deep electric burn of the distal lateral side of the big toe, successfully treated with a distally based dorsalis pedis fasciocutaneous flap, is presented. The donor site area of the dorsum of the foot was grafted, and deambulation was reassumed three weeks later. Advantages, limits and anatomical consideration regarding the viability of the flap are also discussed.     

神经核团的立体定位与立体构形的实验研究

对树中缝背核（ＮｕｃｌｅｕｓＲａｐｈｅＤｏｒｓａｌｉｓ，ＮＲＤ）的立体定位、立体构形和细胞构筑进行研究，结果：ＮＲＤ在ＡＰＯ平面前（０．２７±０．０８）ｍｍ出现，至ＡＰＯ平面后（１．１２±０．０９）ｍｍ消失；ＮＲＤ背侧顶点在ＨＯ平面上（１．６８±０．０６）ｍｍ，腹侧低点在ＨＯ平面下（０．９７±０．０５）ｍｍ；ＮＲＤ左右侧最宽处在ＭＳ平面外（０．５０±０．０４）ｍｍ。根据树ＮＲＤ的形态变化，重塑了它的立体构形。树ＮＲＤ的细胞构筑与其它哺乳类动物的ＮＲＤ细胞构筑大体相似。     

拇指背动脉逆行岛状皮瓣的改进及应用

自１９９２年以来，采用拇指背动脉为蒂的逆行岛状皮瓣修复指端缺损６例，均获成功。皮瓣为１．５ｃｍ×２ｃｍ～４ｃｍ×３．５ｃｍ。经随访，拇指外形好，关节活动不受限，有痛觉恢复。手术的改进要点是：供区为第１、２掌骨背侧，不需切断拇短伸肌健。操作更简单，术后功能恢复快。     

大鼠延髓网状背侧亚核传出投射的PHA-L顺行追踪法研究

目的：显示大鼠延髓网状背侧亚核向内源性镇痛系统的直接传出纤维探讨。方法：将顺行追踪剂PHA－L注入大鼠单侧延髓网状背侧亚核，术后存活20天后取脑，连续冠状切片，继而对切片进行PHA－L免疫组织化学染色。结果：发现中缝大核，蓝斑核，巨细胞网状核，中缝背核，中脑导水管周围灰质以及丘脑室旁核等均有阳性终末。结论：延髓网状背侧亚核与内源性镇痛系统之间有广泛的纤维联系，提示该核与痛觉调节活动有关。     

迷走神经背核至中枢及周围部的分支投射-荧光素双标记法研究

本文利用荧光素双标记法研究了迷走神经背核至中枢及周围部的分支投射。将荧光素Fast Blue(FB)和Nuclear Yellow(NY)分别注入左侧颈段迷走神经干和同侧颈、胸、腰或骶尾段脊髓。结果在同侧迷走神经背核可见到大量的FB标记细胞,极少NY标记细胞和少量FB-NY双标记细胞。提示:迷走神经背核有双重投射,即至中枢及周围部的投射。这种双重投射可分别来自述走神经背核的不同神经元的分支投射或来自同一神经元的侧支投射。     

Incidence of occult athlete's foot in swimmers

Athlete's foot is a dermatophytic infection frequently found in adults. This disease is, in some cases, asymptomatic. In order to evaluate the incidence of subelinical infection, we obtained 300 toe-web samples from the fourth interdigital space of 150 regular swimmers. More over, 66 specimens from the pool area were analysed. The method used was the carpet technique described by Mariat et al. (10). The fungal isolates were identified according to standard methods. A list of epidemiological data was completed for every swimmer.In our results, 22 swimmers had positive cultures (15%), 8 of these cases had no lesions (36%). They included 7 infections with Trichophyton mentagrophytes (87.5%) and one with T. rubrum (12.5%). We observed one case with a dual infection. Only one sample from the inanimate environment was positive. This study showed a significant incidence of occult athlete's foot in swimmers. To control this endemic problem, adequate preventive measures must be taken.     

Dermatophytes in Sporting Activities*: Dermatophyten bei sportlichen Aktivitäten

Summary: Gymnastics and tbe practice of martial arts are very frequent nowadays. These activities involve a great risk of infections in shower rooms, gymnasium grounds, using training tatami judo mattress, etc. The problem of fungus contamination in these places is a serious one. In a study of samples collected in the Sport Club of the Escola Paulista de Medicina, Säo Pado Brasil by using the technique of the sterile square of carpet according to Mariat & Tapia 1966 we isolated dermatophytes from 37.2% of the samples. Epidermophyton floccosum was recovered from tatami and dressing rooms and Trichophyton mentagrophytes was fonnd in bathrooms, tatami and male dressing rooms. Based on present results we propose some prophylactic measures. Zusammenfassung. Gymnastik und andere Leibesübungen werden heute von weiten Bevölke-rungskreiisen betrieben. Bei diesen Aktivitäten besteht ein Infektionsrisiko insbesondere bei der Benutzung von Duschräumen, Turnhallen, Sportmatratzen etc. Das Problem der Pilzkontamination dieser Bereiche ist durchaus ernstzunehmen. In einer Untersuchung, die im Sportklub der Escola Paulista de Medicina, Säo Paulo Brasilien mit Hilfe der 1966 von Mariat & Tapia angegebenea Methode durchgeführt wurde, konnten in 37,2% der Proben Dermatophyten isoliert werden. Epidermophyton floccosum fand sich auf Übungsmatratzen und in Umkleideräumen. Trichophyton mentagrophytes wurde ebenfalls von Matratzen, aus Baderäumen und aus Umkleideräumen von Männern isoliert. Aufgrund dieser Ergebnisse werden prophy-laktische Maßnahmen vorgeschlagen.     

Microsurgical treatment of disorders of leg and foot — A report of 40 cases

Summary Since 1979, the authors have performed five types of microsurgical flap (including myocutaneous flap) graft to treat 40 patients with disorders of leg and foot. In the treatment of inflammatory diseases, good results were obtained owing to the following important points: 1) the operation was performed with inflammation well controlled or localized, particularly in chronic osteomyelitis; 2) indication for each type of microsurgical flap was made carefully; 3) isolation of flap was carried out gently, bleedings were thoroughly stopped by ligation.     

Distribution and quantification of 5-HT nerve cell bodies in the nucleus raphe dorsalis area of C57BL and BALBc mice. Relationship between anatomy and biochemistry

BALBc and C57BL mice have been shown to have a different 5-HT metabolism. The present study compares the number and the distribution of 5-HT cell bodies in the nucleus raphe dorsalis area (B7 + B6) of these strains. By using 5-HT immunohistochemistry, we found a higher number of 5-HT neurons in the most caudal part of NRD (B6) of BALBc mice compared to C57BL. This difference may be correlated with a higher level of endogenous 5-HT, a higher uptake capacity toward exogenous [³H]5-HT, and a lower release of the amine in this same area of BALBc mice compared to C57BL. It could also imply a significant participation of the nerve cell bodies in the regulation of 5-HT transmission inside 5-HT nuclei.
