低抗凝肝素来源低分子肝素口服制剂的研究

以生产肝素的副产品——低抗凝肝素为原料,采用亚硝酸控制解聚法制得了低分子肝素,分子量为5300,抗凝活性为39.2u/mg,测定了理化指标。以对家兔实验性血栓形成的影响为药效学指标,确定低分子肝素口服制剂的处方组成为低分子肝素、油酸、牛胆盐。研究了所制备的低分子肝素胶囊对家兔血液流变学及血栓形成的影响,并试用于部分动脉粥样硬化症志愿者,表明该胶囊可有效地改善家免和动脉粥样硬化症志愿者血液流变性质,抑制血栓形成。     

Oral tolerance in EAE: reversal of tolerance by T helper cell cytokines

Oral administration of myelin basic protein (MBP) inhibits clinical and histopathological manifestations of experimental autoimmune encephalomyelitis (EAE), but only partially reduces serum anti-MBP antibody titers. We report here that orally administered MBP alters the isotypic distribution of anti-MBP antibody-forming cells (AFC) among various lymphoid tissues, with the most profound differences seen in mucosal tissues. We observed an isotype-selective reduction in anti-MBP IgA but not IgM AFC frequencies in Peyer's patches. The anti-MBP IgA AFC frequencies could be reconstituted by addition of interleukin 4 (IL-4) and interleukin 5 (IL-5). The cytokines did not appear to generate de novo responses since no increases in anti-MBP IgA AFC frequencies were observed in control cultures. These results indicate that decreased antibody production, as a result of oral antigen administration, can be reversed by exposure to the appropriate cytokines.     

颈阔肌肌皮瓣修复口腔癌术后组织缺损

目的探讨颈阔肌肌皮瓣修复口腔癌术后组织缺损的方法及可行性。方法2003年3月~2004年6月,应用颈阔肌肌皮瓣修复舌、口底、颊、咽侧壁及软腭缺损15例。其中男12例,女3例,年龄41~77岁。颊癌11例,舌癌1例,口底癌2例,咽侧壁及软腭癌1例。均行病变及颈淋巴根治性切除,缺损范围3.0 cm×3.5 cm~7.0 cm×4.0 cm,应用颈阔肌皮瓣修复软组织缺损,皮瓣范围7.0 cm×3.5 cm~12.0 cm×4.0 cm。结果术后15例皮瓣均成活,肌皮瓣色泽与皮肤相似,外形无异常,张口不受限。术后10~15 d出现颊部皮瘘8例,术后4周二期断蒂修补后完全愈合。随访6~12个月,无舌运动受限等并发症,肿瘤无复发,患者对术后形态及功能均较满意。结论颈阔肌肌皮瓣血运丰富,抗感染力较强,成活率高,是修复口腔组织缺损行之有效的方法,手术方法简便,值得推广。     

适应性反应对中医药研究的启示

适应性反应是近年医学研究的热点问题之一,利用适应性反应已作为疾病防治的新战略,其核心和精髓是充分调动和激发机体内源性保护机制,增加机体适应性和抗损伤能力,维持机体自稳态.适应性反应的研究表明了机体自身存在着很强的内源性保护机制,其本质是适应性反应诱导了内源性保护物质(保护性蛋白等)的产生,而用药物等途径则可模拟或增强适应性保护作用.适应性反应与中医学的天人相应、正气学说、扶正固本、藏象学说、治未病思想、适应原中药、针灸经络等理论有诸多联系,对中医药研究有一定的启示,对于揭示中医学的科学内涵和实质、发展和弘扬中医理论及促进中西医结合均具有一定参考意义.     

Continuous recording of urethral activity in healthy female volunteers

The pressure variations at the maximal urethral closure pressure (MUCP) were continuously recorded in healthy female volunteers by means of a two-point microtip transducer catheter for one hour. Before the investigation a normal voiding was assured objectively and bladder instability was excluded. All women showed pressure variations both at the MUCP and more distally. The pressure variations, from 3 to 66 cm H₂O, showed rhythmicity and three frequency ranges could be identified. Slow pressure waves with a frequency of one in eight to 19 minutes were observed. Relatively fast-pressure waves were observed (one every one to four minutes) and relatively fast-frequency pressure waves were observed (rate: one to eight per minute). The pressure variations of the urethra seem to be an aspect of normal urethral physiology possibly contributing to continence and urinary tract infection prevention.     

应重视口腔颌面部恶性肿瘤的综合序列治疗

口腔颌面部恶性肿瘤约占全身恶性肿瘤的3％，虽然所占比例不高，但l大l其解剖部位特殊．涉及美观和重要的生理功能，冈此对患者机体和心理的影响不容忽视。早期（Ⅰ、Ⅱ期）口腔颌面部恶性肿瘤无论采用手术和（或）放射治疗，均能取得良好效果。但遗憾的是，大多数患者就诊时，已属局部一区域晚期（Ⅲ、Ⅳ期）。由于受到局部解剖和维持功能的限制，单纯通过扩大手术切除范围和改进现有手术技术，除能改善患者生存质量外，已无法提高此类患者的生存率，延长其生存时间。     

Teratoma tongue: Case report and review of literature

Teratomas are true neoplasms composed of tissues from all three germinal layers and may exhibit variable levels of maturity. They have an unknown origin and eccentric microscopic appearance. Teratomas arising from the oral cavity are rare in the newborn; only 22 cases have been reported in the literature. We describe a female neonate with an oral teratoma originating from the tongue that was successfully treated with surgery.     

Reviewing the definition of "elderly"

Conventionally, "elderly" has been defined as a chronological age of 65 years old or older, while those from 65 through 74 years old are referred to as "early elderly" and those over 75 years old as "late elderly." However, the evidence on which this definition is based is unknown. We have attempted to review the definition of elderly by analyzing data from long-term longitudinal epidemiological studies, and clinical and pathological studies that have been accumulated at the Tokyo Metropolitan Geriatric Hospital and the Tokyo Metropolitan Institute of Gerontology. Our recommendation might be a starting point in developing a strategy for a successful society by reviewing the definition of elderly based on comprehensive evidence in all aspects of social, cultural and medical sciences.     

保肾口服液影响IgA肾病小鼠T细胞增殖及分泌IL-2的实验研究

观察保肾口服液对IgA肾病小鼠T细胞^3H-TdR掺入及其分泌IL-2水平的影响，结果表明低、中、高3种浓度的保肾口服液均能显著促进IgA肾病小鼠的自发性和ConA刺激的T细胞^3H-TdR掺入，促进T细胞合成，分泌IL-2，提前给药作用相同。提示保肾口服液有显著增强IgA肾病小鼠细胞免疫功能的作用。
