目的 对1例骨关节尖端赛多孢菌与偶发分枝杆菌并发感染病例进行病原学诊断和治疗分析,为临床少见菌感染诊治提供思路。方法 将左大腿及左膝外伤后膝关节红肿骨科住院患者作为研究对象,采集其关节腔积液进行涂片染色、培养及质谱鉴定,根据病原学鉴定结果给予用药指导。结果 患者关节腔引流液外观为脓性乳糜状,涂片抗酸染色见大量白细胞和抗酸阳性呈柴捆状排列菌体;革兰染色见大量白细胞及真菌菌丝;荧光染色见团状菌丝。培养72h后血平板见丝状真菌菌落及细小、干燥的细菌样菌落,质谱仪鉴定为偶发分枝杆菌和尖端赛多孢菌混合感染。给予左氧氟沙星+伏立康唑+阿米卡星静脉滴注治疗14天,患者左膝关节肿胀明显减轻,左下肢肌力正常出院。出院医嘱改左氧氟沙星+多西环素+伏立康唑口服,因患者经济原因自行停服伏立康唑后复发再次入院,行关节积液培养再次分离出尖端赛多孢菌。临床清创并继续给予左氧氟沙星+伏立康唑+阿米卡星静脉滴注治疗,待症状改善后口服左氧氟沙星+多西环素+伏立康唑,期间肝功能检测有轻度损害,治疗方案改为口服左氧氟沙星+复方磺胺甲噁唑+伏立康唑直至出院,持续治疗6月后患者左膝关节肿胀消失,可自行站立行走,复查血细胞分析、C-反应蛋白、红细胞沉降率及肝肾功能均正常,逐渐停用所有药物。该患者停药后再无复发。结论 骨关节少见菌混合感染应重视实验室涂片检查和病原体鉴定,临床应该结合鉴定结果规范及足疗程治疗是患者治愈的关键。  相似文献

目的探讨不同类型尺骨近端骨折的临床特点、内固定选择、手术策略和临床疗效。 方法回顾性分析2014年6月至2017年6月收治的45例尺骨近端骨折患者资料,男26例、女19例;年龄19~65岁,平均40.8岁;致伤原因:摔伤25例,高处坠落伤12例,交通伤8例。所有患者均行手术治疗,根据骨折的特点和类型分别选择克氏针张力带、解剖钢板或两者联合的方法固定骨折,恢复骨性结构的稳定性。合并尺骨冠状突骨折采用袢钢板固定。合并桡骨头骨折采用微型钢板固定或金属桡骨头置换。合并韧带损伤时均采用带线锚钉缝合固定修补。 结果45例患者获15.6个月(12~24个月)随访。随访结果:所有患者均获骨性愈合,愈合时间平均为11.2周(8~16周)。末次随访:肘部屈曲活动80°~135°,平均121°;肘部伸直活动0~30°,平均8°。Mayo肘关节功能评分为50~100分,平均88.2分,其中优26例,良15例,可2例,差2例,优良率为91.1%。 结论尺骨近端骨折形态各异,应该根据受伤机制、骨折特点及患者情况进行骨折严重程度的评估和分型,选择合理的内固定方式,积极进行术后康复,恢复良好的肘关节功能。  相似文献

Background

Describing the morphologic features of the thoracic pedicle in patients with adolescent idiopathic scoliosis is necessary for placement of pedicle screws. Previous studies showed inadequate reliability owing to small sample size and heterogeneity of the patients surveyed.

Questions/Purposes

To use CT scans (1) to describe the morphologic features of 2718 thoracic pedicles from 60 female patients with Lenke Type 1 adolescent idiopathic scoliosis and 60 age-, sex-, and height-matched controls; and (2) to classify the pedicles in three types based on pedicle width and analyze the distribution of each type.

Methods

A total of 2718 pedicles from 60 female patients with Lenke Type 1 adolescent idiopathic scoliosis and 60 matched female controls were analyzed via CT. All patients surveyed were diagnosed with adolescent idiopathic scoliosis, Lenke Type 1, at the First Affiliated Hospital of Sun Yat-sen University, and all underwent pedicle screw fixation between January 2008 and December 2013 with preoperative radiographs and CT images on file. We routinely obtained CT scans before these procedures; all patients who underwent surgery during that period had CT scans, and all were available for analysis here. Control subjects had CT scans for other clinical indications and had no abnormal findings of the spine. The control subjects were chosen to match patients in terms of age (15 ± 2.6 years versus 15 ± 2.6 years) and sex. Height of the two groups also was matched (154 ± 9 cm versus 155 ± 10 cm; mean difference, −1.06 cm; 95% CI, −1.24 to −0.81 cm; p < 0.001). Pedicle width and length were measured from T1 to T12. The thoracic spine was classified in four regions: apical vertebra in the structural curve (AV-SC), nonapical vertebra in the structural curve (NAV-SC), apical vertebra in the nonstructural curve (AV-NSC), and nonapical vertebra in the nonstructural curve (NAV-NSC). Pedicles were classified in three types: pedicle width less than 2 mm as Type I, 2 mm to 4 mm as Type II, and greater than 4 mm as Type III. Types I and II were defined as dysplastic pedicles. Paired t test, independent samples t test, one-way ANOVA, followed by Bonferroni’s post hoc test and chi-square or Fisher’s exact tests were used for statistical comparisons between patients and controls, as appropriate.

Results

No difference was found between pedicle width on the convex side (PWv) and in controls (PWn), but pedicle width on the concave side (PWc) (4.99 ± 1.87 mm) was found to be narrower than PWv (6 ± 1.66 mm) and PWn (6 ± 1.45 mm). The variation degree of pedicle width (VDPW) was greatest in the AV-SC region (34% ± 37%), in comparison to AV-NSC (20% ± 25%) (mean difference, 14%; 95% CI, 1.15%–27%; p = 0.025), NAV-SC (17% ± 30%) (mean difference, 17%; 95% CI, 7%–27%; p < 0.001), and NAV-NSC (11% ± 24%) (mean difference, 24%; 95% CI, 13%–34%; p < 0.001). Dysplastic pedicles appeared more in patients with adolescent idiopathic scoliosis (22%; 293 of 1322) compared with controls (13%; 178 of 1396) (odds ratio [OR] = 0.51; 95% CI, 0.42–0.63; p < 0.001). In patients with adolescent idiopathic scoliosis, they commonly occurred on the concave side 34% (228 of 661) and on the AV-SC region (32%; 43 of 136).

Conclusions

Pedicle width on the concave side was narrower than pedicle width on the convex side and pedicle width in healthy control subjects. The apical vertebra in the structural curve was the most variegated region of the curve with the highest prevalence of dysplastic pedicles.

Clinical Relevance

Our study can help surgeons perform preoperative assessments in females with adolescent idiopathic scoliosis, and with preoperative and intraoperative management for difficult pedicle screw placement. In particular, our results suggest that surgeons should exercise increased vigilance when selecting pedicle screw dimensions, especially in the concave aspect of the mid-thoracic curve, to avoid cortical breeches. Future studies should evaluate other Lenke types of adolescent idiopathic scoliosis, and males with adolescent idiopathic scoliosis.

ContextVitamin D status and parathyroid hormone (PTH) levels influence the risk of accidental falls in older people, but the mechanisms underlying this effect remain unclear.ObjectiveInvestigate the relationship between circulating PTH and 25 hydroxyvitamin D (25-OHD) levels and clinical tests of gait stability and balance as physical fall risk factors. We hypothesized that high levels of PTH and low 25-OHD levels would be significantly associated with gait stability and decreased balance performance.DesignObservational cohort study.SettingAustralian community.Participants119 healthy, ambulatory female twin adults aged 47–80 years residing in Victoria, Australia.Outcome measuresSerum PTH and 25-OHD levels with clinical tests of gait stability [double support duration (DSD)] and dynamic balance (Step Test). Associations were investigated by regression analysis and by comparing groups divided by tertiles of PTH (<3.5, 3.5–4.9, >4.9 pmol/L) and 25-OHD (<53, 53–75, >75 nmol/L) using analysis of variance.ResultsSerum PTH was associated positively with DSD, with an increase of 10.6–15.7% when the mid and highest PTH tertiles were compared to the lowest tertile (p < 0.025) when 25-OHD was included in the regression analysis. 25-OHD was significantly associated with DSD (greater by 10.6–11.1% when lowest and mid-tertiles compared with the highest 25-OHD tertile) (p < 0.025) and dynamic balance (better performance by 12.6% in the highest compared with the lowest 25OHD tertile) (p < 0.025).ConclusionThese findings reveal an important new relationship between parathyroid hormone and gait stability parameters and add to understanding of the role of 25-OHD in motor control of gait and dynamic balance in community-dwelling women across a wide age span.  相似文献

OBJECTIVE

To evaluate the effect and safety of Zuogui pill and Yougui pill, classic Yin and Yang tonic formula (CYYTF), in the treatment of osteoporosis and the underlying mechanism.

METHODS

Participants aged 55 to 75 with osteoporosis and Kidney deficiency in Traditional Chinese Medicine (TCM) will be included and randomly allocated into two groups: treatment group and control group. Participants in the treatment group were treated with Zuogui pill or Yougui pill TCM formula granule, while the control group received placebo. Primary outcomes are the lumbar spine on bone mineral density (BMD) (L1-4) and femoral BMD. Secondary outcomes include pain intensity, health-related quality of life (HRQoL), bone turnover markers and safety.

RESULTS

Totally 200 patients were enrolled from December 2014 to April 2016 from four hospitals. There were no statistically significant differences between the two groups at baseline (P > 0.05) and it was good to comparability. Statistically significant differences between the two groups were observed for the lumbar BMD (L1-4), pain VAS scores and HRQoL at six months and twelve months and femoral BMD at twelve months (P < 0.05), but no significant differences for femoral BMD and bone turnover markers at six months (P > 0.05). Moreover, significant difference was observed at different time before and after treatment in terms of lumbar spine (L1-4) BMD, femoral BMD, pain VAS scores and health-related quality of life, and there was an crossover effect between the time and groups before and after treatment. In additional, in the treatment group, 8 patients lost to follow-up and 3 patients had adverse events (AEs) and in the control group, 10 patients lost to follow-up and 2 patients had AEs. No remarkable differences were observed between the two groups with regard to AEs, lost rate and safety (P > 0.05).

CONCLUSION

Zuogui pill or Yougui pill could improve BMD, ease pain, relieve Kidney deficiency syndrome, improve the quality of life osteoporosis patients, inhibit bone conversion and regulate the coupling balance of bone formation and bone resorption, but long-term efficacy should be confirmed by a longer term follow-up and larger of samples clinical randomized controlled trials.  相似文献

目的探讨持续多模态电生理监测责任血管对面神经出脑干区(REZ)不同程度的压迫在面神经显微血管减压(MVD)术中的临床意义。 方法回顾性分析西安交通大学附属红会医院功能神经外科自2017年3月至2019年3月收治的行乙状窦后入路行面神经MVD的648例面肌痉挛患者的临床资料,术中监测侧方扩散反应(LSR)、脑干听觉诱发电位(BAEP)。根据术中观察到的面神经根部受压程度将压迫类型分为接触压迫(血管与REZ显微镜下仅见有接触)、压迹压迫(血管在REZ形成压迹)、合并穿支血管压迫(有血管分支穿过面听神经之间)、椎动脉复合体压迫,统计其病例数。观察术中受压程度与LSR消失的关系和术中BAEP变化。 结果本组患者术前、术中均可监测到LSR,术中635例患者LSR消失,其中分离血管后LRS消失者199例,垫离血管后LRS消失者436例;LSR持续存在13例,发生术中BAEP报警者79例,其中术后发生听力障碍者41例。 结论多模态电生理监测能够帮助术者更好的判断责任血管,血管对面神经根部的不同压迫类型与减压后LSR消失与否关系密切,LSR消失时机影响预后,并且增加BAEP报警的几率。  相似文献

Background

Hyaluronic acid injection has been reported to decrease pain compared with baseline levels in knee joint osteoarthritis. Hylan G-F 20 is distinguished from the other products by its chemical structure and relatively higher molecular weight. Many trials have compared hylan G-F 20 and low molecular weight hyaluronic acids (LMWHAs); however, their relative efficacy and safety are still debated.

Objective

The aim was to compare the effectiveness and safety of intra-articular injection of hylan G-F 20 and LMWHA in the treatment of knee joint osteoarthritis.

Methods

A comprehensive search of the literature up to February 2016 was performed; multiple databases were searched with ‘Synvisc’ or ‘hylan’ or ‘hyaluronan’ as free word terms. The pain-related outcomes and treatment-related adverse events from intent-to-treat analyzed studies were pooled for meta-analysis; other functional outcomes were included in the qualitative analysis.

Results

Twenty trials with a total of 3034 patients and 3153 knees were included, with a pooled dropout rate of 7.2 %. The pooled pain-related outcomes at 2 to 3 months reached a statistically significant difference in favor of hylan G-F 20 (I 2 = 88 %; random effects; P = 0.02), and the significance still existed with exclusion (in order to eliminate heterogeneity) of the three studies that most favored hylan G-F 20 (I 2 = 51 %; fixed effect; P = 0.03). No significant difference was reached for other group and subgroup analyses. No significant difference was reached in comparing the patients with treatment-related adverse events (seven trials; 2025 patients; P = 0.13) or the treatment-related adverse events (six trials; 1633 patients; P = 0.14).

Conclusion

According to the current results, limited evidence showed a superior effect favoring hylan G-F 20 over LMWHA in the period from 2 to 3 months post-injection for pain-related outcomes. There was no evidence of increased risk of treatment-related adverse events for hylan G-F 20 injections.
