人外周血CD68^＋单核细胞体外的分选及诱导分化为破骨细胞

OBJECTIVE: To establish a method for obtaining highly purified primary human osteoclast precursors for the biochemical and molecular biological research. METHODS: CD68(+) mono/macrophages were separated from peripheral blood mononuclear cells (PBMCs) of healthy donors by means of immunomagnetic cell sorting for subsequent analysis with flow cytometry. The isolated cells were incubated on coverslips or bone slices in the presence of dexamethasone(10(-8) mol/L), macrophage colony-stimulating factor (25 microg/L ) and soluble receptor activator of nuclear factor (NF)-kappaB ligand (s-RANKL, 16 microg/L). Calcitonin receptor (CR) immunocytochemistry and tartrate-resistant acid phosphatase (TRAP) histochemistry were employed. The bone slices were also studied by scanning electron microscopy (SEM). RESULTS: Fluorescence-activated cytometric analysis showed that 93.06%+/-0.61% n=4 of the selected cells were CD68(+) cells. After 7 days of culture of the CD68(+) cells, VR+, TRAP+ multinucleated giant cells appeared, and resorption lacunae could be observed by SEM. CONCLUSION: Highly purified CD68(+) cells can be obtained from human PBMCs as the osteoclast precursors, and mature osteoclasts can be induced from CD68(+) mono/macrophages by RANKL.

被覆自体微粒皮的辐照氟银猪皮移植治疗深度烧伤15例报告

OBJECTIVE: To study the feasibility of using irradiated porcine skin impregnated with fluorine and silver covered by autogenous skin granule graft in the treatment of severe burn injury. METHODS: Autogenous split-thick skin grafts were cut into small pieces (<1 mm3), which were transferred onto the sheet of porcine skin mounted with a layer of Xinnaxing ointment (2 mm in thickness). After eschar removal, the burn wound was covered with the prepared porcine skin, sutured and fixed with small holes at the lower site for drainage. The growth of autogenous granule skin was then observed and the criteria for therapeutic effect evaluation drafted. RESULTS: The layer of Xinnaxing ointment on the the porcine skin improved the adhesion of the autogenous small skin grafts, therefore full use could be made of the donor skin source. In the 15 patients receiving this treatment, the graft-covered area was (12.3+/-3.5) % that of the wound, with the donor-recipient area ratio of 1: (11.0+/-2.7) and subsequent healing time of 27.5+/-5.5 d. Primary healing was achieved in 9 cases, and secondary healing in 6, demonstrating similar clinical effect to that of allo-skin grafting. CONCLUSION: The method is easy and economic, which saves autogenous skin source and produces satisfactory clinical effect.     

外伤性肝破裂的救治

目的 分析创伤性肝破裂外科多种干预治疗的效果。方法 总结分析1982-2002年我院收治创伤性肝破裂76例的救治方法和病死率。其中Ⅲ级以上的严重肝破裂46例(60．53％)。手术治疗56例，手术方式包括单纯修补、清创性肝切除、规则性肝切除、肝周填塞止血；非手术治疗20例。结果 手术组：治愈44例，术后并发症17例均经保守治疗治愈，死亡12例，其中术中死亡2例；非手术组：治愈12例，好转6例，2例失访。全组治愈64例，死亡12例，病死率15．8％。结论 Ⅰ-Ⅱ级单纯性外伤性肝破裂可保守治疗；手术是治疗创伤性肝破裂的主要措施，正确的手术方式，积极处理合并伤，重视综合治疗可提高救治成功率。     

老年人肱骨近端二部分和三部分移位骨折19例临床分析

目的 探讨老年人肱骨近端二部分和三部分移位骨折的手术治疗方法。方法 回顾性分析19例老年肱骨近端二部分和三部分移位骨折患者的临床资料，其中二部分骨折13例、三部分骨折6例，采用切开复位，肱骨近端解剖钢板固定治疗。结果 二部分骨折术后优良率为76．9％，未出现骨折不愈合及肱骨头坏死；三部分骨折术后优良率为66．7％，肱骨头坏死率16．7％。结论 老年人肱骨近端二部分和三部分骨折应尽量采用有限切开复位，选择肱骨近端解剖钢板固定。     

Inhibitory Effect of Angiogenesis Inhibitor TNP—470 on Human ACHN Renal Cell Carcinoma

Promoting effect of the Chinese medicinal herb, wikstroemia chamaedaphne and tung oil extracts on carcinoma of the uterine cervix induced by HSV-2 or methylcholanthrene (MCA) in mice

The promoting effect of the Chinese medicinal herb, Wikstroemia chamaedaphne and Tung oil extracts (WC and HHPA) on carcinoma of uterine cervix induced by HSV-2 or MCA in mice was studied. The results showed that WC and HHPA extracts were not carcinogenic themselves. After carcinogen HSV-2 and MCA treatment, WC and HHPA were added separately. The inducing rates by HSV-2 increased from 7.4% to 21.1% and 26.3%, those by MCA increased from 56.5% to 82.8% and 84.4%. There was a significant difference between the combined groups and groups with HSV-2 or MCA only. The experimental results suggest that these two kinds of extracts play a promoting effect on carcinogenesis. The relation between the carcinogenesis of uterine cervix or nasopharynx and WC or HHPA extracts is discussed.     

JX-1对雄性小鼠生殖细胞致突变作用的研究

本文采用小鼠精子试验和小鼠睾丸DNA合成抑制试验,检测了JX-1对雄性生殖细胞的致突变作用。实验结果表明JX-1对小鼠精子总数、精子活动率及精子畸形率均无统计学意义的增加,与小鼠睾丸DNA合成抑制试验阴性反应一致。作者认为JX-1对雄性生殖细胞为一种非诱变剂。     

血虚证的本质初探

中医血虚证是由失血过多，或脾胃虚弱，或血液生化之源不足，或因瘀血阻滞新血不生等原因所导致的血液不足或血液营养功能低下，脏腑组织器官失养的病理状态。本文从现代医学“贫血”的常规研究对中医血虚量上的不足人手，对中医血液的功能低下所致血虚证进行深入探讨。     

循证护理在糖尿病皮肤损害临床实践中的应用

目的　应用循证护理 (EBN) ,探索糖尿病皮肤损害有效干预方法 ,减少残疾及死亡率 ,提高病人生活质量。方法　通过临床评估确定护理诊断 ,查阅文献 ,选择最佳护理证据 ,制定护理干预措施。结果　通过治疗及护理干预 ,2 2例糖尿病患者皮肤损害 ,显效 2 1例 ,有效 1例 ,转科 1例 ,出院时患者无并发症。结论　循证护理应用于临床 ,使传统护理经验走向护理科学发展 ;促进护士主动学习 ,提高护士综合素质 ;优质护理服务于个体化 ;解决患者现存的、潜在的护理问题 ,提高患者生存质量。
