BackgroundHamstring strain injuries are the most common type of injury in elite football and are associated with a high risk of reinjury, particularly those involving the intramuscular tendon (IMT). Limited information is available regarding the rehabilitation and return to sport (RTS) processes following such injuries. This case study describes the clinical presentation of an elite football player following IMT hamstring injury, their on- and off-pitch rehabilitation alongside performance monitoring throughout RTS and beyond.Case scenarioAn elite football player suffered a grade 2c hamstring injury during an English Premier League (EPL) match. The player underwent early post-injury management, alongside progressive off-pitch physical preparation. The ‘control-chaos continuum’ was used as a framework for on-pitch rehabilitation to prepare the player for a return to full team training and competition. Objective and subjective markers of the player's response to progressive on- and off-pitch loading were monitored throughout RTS and beyond.OutcomesThe player returned to on-pitch rehabilitation after 11 days, to full team training having achieved weekly pre-injury chronic running load outputs after 35 days and played in the EPL 40 days post-injury. The player did not suffer reinjury for the rest of the EPL season.ConclusionAn understanding the unique structural and mechanical properties of the IMT, alongside expected RTS timeframes are important to inform rehabilitation and decision-making processes post-injury. Performance and frequent load-response monitoring throughout RTS and beyond, in conjunction with practitioner experience and effective communication are critical in facilitating effective RTS and reduce risk of reinjury following IMT injury. 相似文献

Background and objectives
A prompt and effective management of trauma patient is necessary. The aim of this case report is to highlight the importance of intraoperative echocardiography as a useful tool in patients suffering from refractory hemodynamic instability no otherwise explained.Case report
A 41 year‐old woman suffered a car accident. At the emergency department, no abnormalities were found in ECG or chest X‐ray. Abdominal ultrasound revealed the presence of abdominal free liquid and the patient was submitted to urgent exploratory laparotomy. Nevertheless, she persisted suffering arterial hypotension and metabolic acidosis. Looking for the reason of her hemodynamic instability, intraoperative transthoracic echocardiography was performed, finding out the presence of pericardial effusion. Once the cardiac surgeon extracted pericardial clots, patient's situation improved clinically and analytically.Conclusion
Every anesthesiologist should be able to use the intraoperative echocardiography as an effective tool in order to establish the appropriate measures to promote the survival of patients suffering severe trauma. 相似文献4.

目的探讨经鼻间歇正压通气(NIPPV)在防治早产儿呼吸窘迫综合征(RDS)中的应用价值。方法选择2017年6月至2018年12月梧州市人民医院RDS早产儿90例,其中男性48例,女性42例;胎龄(29.03±0.58)周;出生体质量(996.91±98.52)g;病程(3.48±0.56)h;临床分级Ⅰ级58例,Ⅱ级32例;Apgar评分(6.85±1.06)分。依据随机数字表分为NIPPV组和持续气道正压通气(NCPAP)组,每组45例。NIPPV组给予NIPPV治疗,NCPAP组给予NCPAP治疗,若两组治疗后不能维持患儿生命体征则使用肺表面活性物质(PS)或行有创机械通气。结果NIPPV组和NCPAP组治疗12、24 h后和治疗结束时动脉血氧分压(PaO2)、氧合指数(OI)明显高于治疗前。NIPPV组治疗12、24 h后PaO2、OI明显高于NCPAP组,差异有统计学意义(P<0.05)。NIPPV组和NCPAP组治疗结束时PaO2、OI比较,差异无统计学意义(P>0.05);NIPPV组PS使用率(22.22%vs 44.44%)、有创通气率(17.78%vs 40.00%)、氧疗时间[(71.42±7.62)h vs(85.62±9.24)h]、有创通气时间[(46.78±5.32)h vs(55.27±6.14)h]、住院时间[(30.42±3.65)d vs(35.62±3.89)d]、并发症率(31.11%vs 53.33%)明显低于NCPAP组,差异有统计学意义(P<0.05)。结论NIPPV可有效改善RDS早产儿通气功能,有利于减少PS使用、有创通气及并发症,值得临床推广。 相似文献

Premature infants are physiologically immature and are particularly vulnerable to systemic illnesses. Instability of vital signs related to immaturity of organ systems are common but may also represent early manifestations of serious illness. While vital signs are frequently or continuously monitored in critically unwell neonates, changes in these parameters are subtle in the early phase of illness such as sepsis and difficult to interpret using traditional neonatal monitoring tools. Recent advances in identifying trends and patterns of vital signs in the pre-clinical phase of the illness, particularly in the field of heart rate characteristics monitoring has opened up the potential to improve outcomes through rapid and timely investigations and early intervention. ObjectiveWe aimed to establish an objective neurophysiological test protocol that can be used to assess the somatosensory nervous system.MethodsIn order to assess most fiber subtypes of the somatosensory nervous system, repetitive stimuli of seven different modalities (touch, vibration, pinprick, cold, contact heat, laser, and warmth) were synchronized with the electroencephalogram (EEG) and applied on the cheek and dorsum of the hand and dorsum of the foot in 21 healthy subjects and three polyneuropathy (PNP) patients. Latencies and amplitudes of the modalities were assessed and compared. Patients received quantitative sensory testing (QST) as reference.ResultsWe found reproducible evoked potentials recordings for touch, vibration, pinprick, contact-heat, and laser stimuli. The recording of warm-evoked potentials was challenging in young healthy subjects and not applicable in patients. Latencies were shortest within Aβ-fiber-mediated signals and longest within C-fibers. The test protocol detected function loss within the Aβ-fiber and Aδ-fiber-range in PNP patients. This function loss corresponded with QST findings.ConclusionIn this pilot study, we developed a neurophysiological test protocol that can specifically assess most of the somatosensory modalities. Despite technical challenges, initial patient data appear promising regarding a possible future clinical application.SignificanceEstablished and custom-made stimulators were combined to assess different fiber subtypes of the somatosensory nervous system using modality-specific evoked potentials. 相似文献

目的:比较经鼻导管高流量吸氧(HFNC)与经鼻气道正压通气(nCPAP)在重症毛细支气管炎呼吸支持中的应用价值,为临床治疗方案的选择提供参考。方法:选取2016年12月至2018年12月我院儿科收治的重症毛细支气管炎患儿90例,采用随机数字表法分为观察组和对照组各45例。两组患儿入院后均给予常规综合治疗以保证呼吸道通畅,在此基础上观察组采用HFNC治疗,对照组采用nCPAP治疗,比较两组患儿治疗前和治疗24 h后呼吸频率、经皮血氧饱和度(TcSO2)、呼吸窘迫评分体系(CSS)评分、动脉血氧分压(PaO2)等呼吸相关指标及治疗前后临床症状体征改善情况。结果:两组患儿治疗24 h后呼吸频率、CSS评分均降低,且观察组降低程度更大,TcSO2、PaO2于治疗24 h后升高,观察组升高幅度较对照组明显;治疗后两组患儿咳嗽及肺部湿啰音、肺部炎症情况均改善,观察组症状体征消失时间早于对照组,差异均有统计学意义(P<0.05)。结论:重症毛细支气管炎患儿采用HFNC治疗可明显改善通气功能和临床症状,治疗效果优于nCPAP治疗,可扩大样本量进一步观察。 相似文献
