目的 调查河南省X射线摄影致成年受检者的入射体表剂量水平,为建立适合我国国民体质特征的放射诊断受检者剂量指导水平提供技术和数据支持。方法 采用非概率抽样方法选取河南省郑州、开封和信阳3个地市14家医院,用热释光剂量测量方法调查普通X射线摄影、计算机X射线摄影(CR)和直接数字化X射线摄影(DR)不同照射部位1 404名受检者入射体表剂量水平。结果 河南省普通X射线摄影、CR摄影和DR摄影致成年受检者入射体表剂量范围分别为0.20~47.71、0.16~6.89和0.10~10.41 mGy。腹部前后位(AP)、骨盆AP、头颅侧位(LAT)、头颅后前位(PA)、胸部LAT、胸部PA、胸椎AP、胸椎LAT、腰椎AP、腰椎LAT摄影入射体表剂量范围分别为0.16~10.05、0.20~10.36、0.11~2.13、0.10~2.92、0.39~5.85、0.12~1.82、0.16~11.67、0.36~29.37、0.25~14.49和1.18~47.71 mGy。普通X射线摄影致受检者入射体表剂量高于CR和DR摄影,差异有统计学意义(Z=-8.709、-9.570,P<0.05)。普通X射线摄影胸部PA、腰椎LAT入射体表剂量高于全国“九五”期间调查结果,差异有统计学意义(Z=3.262、2.538,P<0.05)。结论 河南省普通X射线摄影和CR摄影所致受检者胸部PA和LAT入射体表剂量超过医疗照射指导水平,普通X射线摄影部分照射部位入射体表剂量较全国“九五”期间调查结果有所提高。  相似文献

Computed radiography (CR) has been shown to have relatively low sensitivity for detection of pulmonary nodules. This poor sensitivity precludes its use as a screening modality despite the low cost, low dose and wide distribution of devices. The purpose of this study was to apply multi-objective frequency processing (MFP) to CR images and to evaluate its usefulness for diagnosing subtle lung abnormalities. Fifty CR images with simulated subtle lung abnormalities were obtained from 50 volunteers. Each image was processed with MFP. We cut chest images. The chest image was divided into two rights and left. A total of 200 half-chest images (100 MFP-processed images and 100 MFP-unprocessed images) were prepared. Five radiologists participated in this study. ROC analyses demonstrated that the detection rate of simulated subtle lung abnormalities on the CR images was significantly better with MFP (Az=0.8508) than without MFP (Az=0.7925). The CR images processed with MFP could be useful for diagnosing subtle lung abnormalities. In conclusion, MFP appears to be useful for increasing the sensitivity and specificity in the detection of pulmonary nodules, ground-glass opacity (GGO) and reticular shadow.  相似文献

The aim of this study was to evaluate the effectiveness of dual-exposure dual energy subtraction technique in flat-panel chest radiography for lung nodules detection. Chest radiographs were acquired in 100 patients (57 men and 43 women; mean age, 60.2 years; range, 18-89 years) using a flat-panel digital chest system. These images were evaluated by seven radiologists. A continuous rating scale of 0-100 was used to represent each observer's confidence level regarding the presence or absence of lung nodules. Observer performance for detection of lung nodules with subtraction images was tested by using receiver operating characteristic (ROC) analysis of individual and averaged reader data. The average area under the ROC curve (Az value) significantly increased with subtraction images (Az=0.79 in standard radiographs versus Az=0.84 with subtraction images, p<0.05). In conclusion, the two-exposure dual-energy subtraction chest radiography significantly would improve detection of lung nodules.  相似文献

The purpose of this retrospective study was to evaluate the impact of energy subtraction (ES) chest radiography on the detection of pulmonary nodules and masses in daily routine. Seventy-seven patients and 25 healthy subjects were examined with a single exposure digital radiography system. Five blinded readers evaluated first the non-subtracted PA and lateral chest radiographs alone and then together with the subtracted PA soft tissue images. The size, location and number of lung nodules or masses were registered with the confidence level. CT was used as standard of reference. For the 200 total lesions, a sensitivity of 33.5–52.5% was found at non-subtracted and a sensitivity of 43.5–58.5% at energy-subtracted radiography, corresponding to a significant improvement in four of five readers (p < 0.05). However, in three of five readers the rate of false positives was higher with ES. With ES, sensitivity, but not the area under the alternative free-response receiver operating characteristics (AFROC) curve, showed a good correlation with reader experience (R = 0.90, p = 0.026). In four of five readers, the diagnostic confidence improved with ES (p = 0.0036). We conclude that single-exposure digital ES chest radiography improves detection of most pulmonary nodules and masses, but identification of nodules <1 cm and false-positive findings remain a problem.  相似文献

目的 了解数字摄影(DR)检查中受检者的辐射剂量水平,为数字放射摄影受检者指导水平的制定提供基础数据。方法 使用热释光剂量计TLD测量受检者不同部位、不同投照方向的入射体表剂量(ESD);使用剂量面积乘积仪测量受检者不同部位、不同投照方向的剂量面积乘积(DAP),并利用测量的DAP值,估算出有效剂量(E)。结果 同类检查中,kV和mAs的变化范围较大,不同部位DR检查中ESD、DAP和E的差别均具有统计学意义(F=33.47、24.68、43.19,P<0.05)。其中,ESD和DAP均以腰椎(LAT)最高,均数为4.62mGy/次和2.26Gy·cm2/次;E以腹部(AP)最高,均数为0.59mSv,高于腰椎(LAT)的0.31mSv。结论 DR在加强受检者放射防护最优化方面很有潜力,应尽快建立适合我国国民体质特征的数字放射摄影受检者辐射剂量的指导水平。  相似文献

目的 评价双能量数字减影胸片和常规DR胸片对于不同肺野区域内结节性病变的检出差异。方法  2 0名正常成年志愿者前胸粘贴模拟结节 ,拍摄双能量数字减影正位胸片 ,应用ROC分析比较常规DR胸片与减影后软组织图像的诊断结果。结果 在双上、中肺野 ,软组织图像的曲线下面积大于常规DR胸片 ;在双上肺野、双中肺野外带 2种方法具有显著性差异。结论 双能量数字减影技术对双上肺野及双中肺野外带胸部结节性病变的检出具有优势 ,故应结合减影及常规胸片 ,以助诊断。  相似文献

To assess both sensitivity and specificity of digital chest radiography alone and in conjunction with dual-exposure dual-energy chest radiography for the detection and classification of pulmonary nodules. One hundred patients with a total of 149 lung nodules (3-45 mm; median, 11 mm) confirmed by CT were included in this study. Dual-exposure dual-energy chest radiographies of each patient were obtained using a CsI detector system. Experienced board-certified chest radiologists from four different medical centers in Europe reviewed standard chest radiographs alone and in conjunction with dual-energy images blinded and in random order. The reviewers rated the probability of presence, calcification and malignancy of all lung nodules on a five-point rating scale. Lesions detected were identified by applying a specific coordinate system to enable precise verification by the study leader. A receiver-operating characteristic (ROC) analysis was performed. In addition to the 149 true-positive CT proven lesions, 236 false-positive lung nodules were described in digital chest radiographies in conjunction with dual-energy chest radiographies. The cumulative sensitivity of chest radiography in conjunction with dual energy was 43%, specificity was 55%. For digital radiography alone, sensitivity was 35% and specifity was 83%. For the dual energy system, positive predictive value was 58%, and negative predictive value was 66% compared to the digital radiography with a positive predictive value of 59% and a negative predictive value of 65%. Areas under the curve in a ROC analysis resulted in 0.631 (95% confidence interval =0.61 to 0.65) for radiography with dual energy and 0.602 (95% confidence interval =0.58 to 0.63) for digital radiography alone. This difference was not statistically significant. For the detection of lesion calcification or the determination of malignancy, ROC analysis also failed to show significant differences. CsI-based flat-panel dual-exposure dual-energy imaging added to standard chest radiography did not show statistically significant improvement for the detection of pulmonary nodules, nor the identification of calcifications, nor the determination of malignancy.  相似文献

PURPOSE: The purpose of this study was to assess the effect of dual-exposure dual-energy (DE) subtraction chest radiography with flat-panel detector. MATERIALS AND METHODS: One hundred patients underwent dual-exposure DE subtraction chest radiography and chest CT for evaluation of pulmonary nodules. Fifty-two patients with pulmonary nodules and 48 patients with normal lungs were selected for receiver operating characteristic (ROC) curve analysis. Ten radiologists who were unaware of the CT results evaluated chest radiography alone and chest radiography with DE subtraction images in the detection of pulmonary nodules. For each radiologist, we calculated the areas under the ROC curve (Az) for chest radiography alone and chest radiography with DE subtraction images. RESULTS: The average detectability of dual-exposure DE subtraction chest radiography was statistically significantly higher than that of chest radiography without subtraction images (mean Az value increased from 0.784 to 0.815, p<0.001). CONCLUSION: Dual-exposure DE subtraction chest radiography improves diagnostic accuracy of pulmonary nodules.  相似文献

目的 通过调查全国15个省市不同级别医疗机构使用数字和屏片设备开展X射线摄影时受检者的入射体表剂量(ESD),为制定适合我国国民体质特征的X射线摄影诊断参考水平提供数据。方法 按照《医用辐射危害评价与控制技术研究》实施方案的要求,选择年龄在20~70岁的受检者,男性体重在55~80 kg,女性体重在50~70 kg;采用热释光探测器(TLD)测量不同X射线摄影程序中成人受检者ESD,每台设备同一体位患者应不少于10名受检者;测量部位包含头颅、胸部的后前位(PA)和侧位(LAT),腹部、骨盆、腰椎、胸椎的前后位(AP)和腰椎、胸椎的LAT等。结果 共在全国15个省的342家医院调查了19 975例X射线摄影受检者,1 813台不同类型X射线摄影设备,包括屏片X射线摄影、计算机X射线摄影(CR)和直接数字化X射线摄影(DR)设备,对于这3种类型的设备,不同摄影体位时受检者ESD的平均值分别为头颅PA:1.75、1.90、1.15 mGy;头颅LAT:1.69、1.46、1.03 mGy;胸部PA:0.75、0.65、0.36 mGy;胸部LAT:1.81、1.26、0.88 mGy;腹部AP:4.37、3.77、2.15 mGy;骨盆AP:3.73、3.56、2.75 mGy;腰椎AP:5.49、5.84、4.17 mGy;腰椎LAT:12.01、9.37、6.82 mGy;胸椎AP:4.53、3.65、2.49 mGy;胸椎LAT:6.91、6.43、4.15 mGy。结论 不同照射部位X射线摄影致受检者ESD有较大的差异。DR设备致受检者的ESD均低于屏片设备;除胸椎AP外,DR设备致受检者ESD均低于CR设备。在所有检查部位中,CR和屏片设备所致受检者ESD的差异均无统计学意义。  相似文献

PURPOSE: To compare three tube voltages in digital selenium radiography for the detection of simulated interstitial lung disease, nodules, and catheters. MATERIALS AND METHODS: Simulated catheters, nodules, and ground-glass, linear, miliary, and reticular patterns were superimposed over an anthropomorphic chest phantom. Digital selenium radiography was performed with different tube voltages (70, 90, and 150 kVp). Hard-copy images were generated. Detection performance of five radiologists was compared by using receiver operating characteristic (ROC) analysis involving 54,000 observations. RESULTS: The detection of ground-glass, linear, miliary, and reticular patterns over lucent lung and of nodules equal to, smaller than, and larger than 10 mm increased when 70 kVp and/or 90 kVp was used. However, only the reticular pattern was significantly better detected at lower peak voltage (P <.05). Simulated catheters and nodules over the mediastinum showed smaller areas under the ROC curve at lower peak voltage. These results were not statistically significant (P >.05). CONCLUSION: The diagnostic performance of digital selenium radiography at lower peak voltage is at least as good as that at higher peak voltage for interstitial lung disease over lucent lung. Performance is equivalent for nodules and catheters over obscured chest regions at lower peak voltages compared with that at 150 kVp. Our results implicate that the use of high-voltage technique in digital selenium radiography should be reassessed.  相似文献

Objectives

To assess the effect of bone suppression imaging on observer performance in detecting lung nodules in chest radiographs.

Materials and methods

Posteroanterior (PA) and lateral digital chest radiographs of 111 (average age 65) patients with a CT proven solitary nodule (median diameter 15 mm), and 189 (average age 63) controls were read by 5 radiologists and 3 residents. Conspicuity of nodules on the radiographs was classified in obvious (n = 32), moderate (n = 32), subtle (n = 29) and very subtle (n = 18). Observers read the PA and lateral chest radiographs without and with an additional PA bone suppressed image (BSI) (ClearRead Bone Suppression 2.4, Riverain Technologies, Ohio) within one reading session. Multi reader multi case (MRMC) receiver operating characteristics (ROC) were used for statistical analysis.

Results

ROC analysis showed improved detection with use of BSI compared to chest radiographs alone (AUC = 0.883 versus 0.855; p = 0.004). Performance also increased at high specificities exceeding 80% (pAUC = 0.136 versus 0.124; p = 0.0007). Operating at a specificity of 90%, sensitivity increased with BSI from 66% to 71% (p = 0.0004). Increase of detection performance was highest for nodules with moderate and subtle conspicuity (p = 0.02; p = 0.03).

Conclusion

Bone suppressed images improve radiologists’ detection performance for pulmonary nodules, especially for those of moderate and subtle conspicuity.  相似文献

PURPOSE: To compare the exposure dose requirements and performance of a portable indirect flat-panel detector for pediatric use in the depiction of catheters, simulated pulmonary nodules, and simulated interstitial lung disease with those of storage phosphor radiography. MATERIALS AND METHODS: Catheters and simulated nodules and subtle interstitial lung disease (miliary, reticular, linear, and ground-glass patterns) were superimposed over an anthropomorphic chest phantom. Images were obtained with different exposures corresponding to simulated speeds of 400 and 800 with a portable flat-panel detector and printed on hard copies. These images were compared with those from storage phosphor radiography at a simulated speed of 400, which is typically used in pediatric radiology. Four independent readers recorded 7200 observations per pattern (for a total of 600 statistically independent observations), and these observations were subjected to receiver operating characteristic (ROC) analysis. Differences were considered significant at a P value of .05. RESULTS: Catheters over obscured chest areas, nodules 10 mm or smaller and larger than 10 mm over lucent lung, nodules 10 mm or smaller over obscured chest areas, and miliary and linear patterns over lucent lung showed higher areas under the ROC curve (A(z)) with the flat-panel detector at 400 and 800 digital speed compared with storage phosphor radiography. A(z) values for reticular and ground-glass patterns with the flat-panel detector were equal to or less than those with storage phosphor radiography. These differences, however, were not statistically significant. CONCLUSION: In the detection of catheters, nodules, and almost all interstitial lung disease, A(z) values were higher with the portable flat-panel detector than with storage phosphor radiography at equivalent and reduced speeds. These results suggest that the portable flat-panel detector could be used with reduced exposure dose in pediatric patients.  相似文献

目的探讨VolumeRAD容积成像检测肺结节的价值。方法39例,男21例,女18例,平均年龄(53.8±9.7)岁。用CT进行随访的肺结节患者进行了额外的PA胸部平片检查和VolumeRAD容积成像。三名胸部x线诊断医师在PACS工作站上独立对胸部平片和VolumeRAD图像进行分析,结节依据清晰度被分为“清晰可见”,“不确定”,和“不可见”,依据大小分为:A组(直径〈5mm),B组(直径5~10mm),C组(直径〉10mm)。胸部CT作为参考标准,结节“清晰可见”为真阳性,计算胸部平片和VolumeRAD成像对肺结节的诊断灵敏度。结果CT上共显示结节247个,胸部平片和VolumeRAD成像对肺结节诊断的灵敏度分别为(69.90±1.30)%和(24.02±1.17)Y00(P〈0.05),在各组间和总数间差别均有统计学意义。结论与PA胸片比较,VolumeRAD容积成像具有提高已知肺小结节检测性能的潜力。  相似文献

The advantages of imaging the chest with digital storage phosphor radiography (SR) may be nullified by its spatial resolution, which is lower than that of conventional film radiography (FR). To test the reader detection performance with the two modalities under clinical conditions, the authors compared 140-kVp isoexposure SR (system resolution: 0.2 mm, 10 bits) and FR images of a variety of chest abnormalities proved by computed tomography (CT) (157 patients, 244 abnormalities, 5,652 observations, six readers). In all tests, SR was as good as or better than FR (P less than .05). In overall detection, indicated by the average area of receiver operating characteristics, SR and FR were equivalent. SR was superior for mediastinal lesions and for pulmonary opacities greater than 2 cm in diameter. For all other types of pulmonary lesions and pleural abnormalities, SR and FR were equivalent. Currently available commercial SR systems can replace film radiographic systems in the detection of a wide variety of chest lesions. SR is likely to enable better visualization than FR in the detection of mediastinal and large pulmonary abnormalities.  相似文献

目的探索碘化铯/非晶硅数字摄影系统(间接DR)拍摄胸片的适宜管电压。方法(1)分别对60名正常成人拍摄80、100、150 kV正位胸片,由5位放射科医师对比每人的3张图像质量,对多个解剖部位的显示情况和图像总体印象进行评分;(2)分别固定表面入射剂量和出射剂量,对人胸部体模拍摄80、100、120、150 kV正位胸片,对肺内模拟病变进行评分。结果成人胸片和肺内模拟病变的显示均随管电压增高,图像质量降低;80 kV对比度最好,但外观略接近低千伏摄影胸片。结论间接DR拍摄正位胸片的适宜管电压为80~100 kV,高千伏摄影不再适用于DR。  相似文献

To evaluate the efficacy of AMBER, a multiple-beam equalization system for chest radiography, the authors performed a nodule detection study using an anthropomorphic chest phantom. AMBER and conventional images were compared. The images were read by four observers, and analysis was done by means of modified receiver-operating characteristic (ROC) curves (free ROC curves [FROC]). The results of the FROC analysis show a significant increase in the detectability of nodules (P less than .001) projected over the mediastinum with the use of AMBER. No significant difference between AMBER and conventional images was noted in detectability of nodules projected over the lung.  相似文献

PURPOSE: To access the influence of anatomic noise on the detectability of subtle lung nodules depicted on chest radiographs. MATERIAL AND METHODS: From normal chest radiography images, 132 square regions were extracted, of which the centers were on the upper margin of a rib, the inside of a rib, the lower margin of a rib, and the central region between two adjoining ribs. Simulated nodules were digitally superimposed at the centers of these extracted square images. Twelve radiologists viewed 50 soft-copy images consisting of these 792 processed images, including the noise-added images. The observer's confidence level for the square images containing single nodules was used as an index of observer performance. RESULTS: Results indicated statistically reliable effects of the relationship between rib structures and nodule positions on the detection performance (P < 0.001). The nodule detectability on the images with a center located between two adjoining ribs was significantly the best, whereas it was significantly the worst on the noise-added images with a center located between two adjoining ribs. CONCLUSION: The rib structures overlying a subtle lung nodule on chest X-ray images have a detrimental effect on nodule detection performance as anatomic noise, regardless of the nodule location on ribs.  相似文献

A comparison of the diagnostic performance of six methods of performing posteroanterior radiography of the chest was carried out using images of an anthropomorphic phantom. Experienced observers were asked to detect nodular (coin) lesions inserted in the lung fields and mediastinal areas of the phantom and the results were subjected to an ROC analysis. Results showed that there were only marginal, non-significant differences in ROC performance (Az) between the individual systems: Lanex, Dupont Ultravision, Kodak HC and Kodak VHC and Fuji AC (P>0.1) but that a league table of different (Az) scores could be formed. The AMBER system was superior to the other systems at various levels of significance for mediastinal lesions (P<0.02 to >0.1), for lung lesions (P<0.01 to >0.1), and for lesions in any location (P<0.01 to >01). The computed radiography system Fuji AC performed least well in this league table but operational factors were felt to have contributed to this outcome. Significantly the system was tested under the normal operating conditions for its clinical setting, that is at a lower kV than the other systems and without the use of a grid. Comments and suggestions are made on why computed radiography may underperform if (a) it is not operated at optimum acquisition conditions and (b) its unique post-processing attributes are not exploited.  相似文献

Digital storage phosphor radiography (SR) has a wide dynamic range and unique postprocessing capabilities that may improve the performance of screening studies for asbestos-related pleural disease compared with conventional film radiography (FR). In a group of 32 asbestos-exposed and nine control subjects with established pleural data, we compared the screening performance of FR and SR obtained with a single isoexposure, dual-energy technique (system resolution 0.2 mm, 10 bits). Performance was evaluated for 7320 observations by eight readers using a paired t test (P less than .02 with Bonferroni correction) of averaged receiver operating characteristic curve (ROC) areas (Az +/- standard error). We found that SR alone and SR supplemented by dual-energy soft-tissue and calcium images (SRde) were superior to FR in the overall detection of pleural abnormalities (Az = 0.90 +/- 0.01, 0.90 +/- 0.01, and 0.88 +/- 0.01, respectively). In the specific detection of pleural calcification, SRde was superior to FR (Az = 0.91 +/- 0.01 and 0.87 +/- 0.01, respectively; P less than 0.01). Analysis of variance indicated that SRde most closely reproduced an established pleural score based on the International Labor Organization (ILO) classification of the pneumoconioses (P less than 0.05, Scheffé's multiple comparison test). We conclude that isodose SR performs at least as well as FR in screening for asbestos-related pleural disease. SR supplemented by dual-energy images might improve the specific detection of pleural calcifications compared with FR.  相似文献

Purpose The aim of this study was to investigate how much the radiation dose can be reduced for the identification and characterization of focal ground-glass opacities (GGOs) by high resolution computed tomography (HRCT). Materials and methods A chest CT phantom including GGO nodules was scanned with a 40-detector CT scanner. The scanning parameters were as follows: tube voltage 120 kVp; beam collimation 32 × 1.25 mm; thickness and intervals 1.25 mm; tube current and rotation time 180, 150, 120, 90, 60, and 30 mA. 180 mA was the standard. Using a three-point scale at different currents, we visually evaluated image quality. Furthermore, we carried out observer performance tests using receiver operating characteristic (ROC) analysis to evaluate the ability to identify GGO nodules at each current. Results By visual analysis, the scores for all particulars were significantly lower on images obtained at less than 120 mA than at 180 mA (Steel’s test, P < 0.05). There was no statistically significant difference in any particulars other than artifact on images obtained at 180, 150, and 120 mA. By ROC analysis there was no statistical difference in the Az value to identify GGO nodules on images obtained at 180, 150, 120, 90, or 60 mA. However, the Az value at 30 mA was significantly lower than at 180 mA (Dunnett’s test, P < 0.01). Conclusion The minimum current necessary for the characterization of GGO nodules on HRCT was 120 mA, although their identification was possible at currents of >30 mA.  相似文献
