Studies of relapsing neuromyelitis optica (RNMO) using advanced MRI techniques are limited compared with those done on multiple sclerosis (MS). The present study used diffusion tensor imaging (DTI) to investigate whether occult brain damage exists in RNMO patients. DTI scans using a 3.0T MRI scanner were performed in 24 clinically confirmed RNMO patients whose conventional brain MRI results were normal, and also in 24 age- and sex-matched healthy control subjects. DTI data were processed to generate fractional anisotropy (FA) and mean diffusivity (MD) maps, and region of interest (ROI) analyses were performed to obtain these parameters in white matter (including medulla oblongata, cerebral peduncle, optic radiation, genu of corpus callosum, splenium of corpus callosum, and internal capsule) and gray matter (including thalamus and putamen). Regional measures from patients at stable and acute phases were compared with healthy controls. Both acute and stable NMO patients had a higher average FA in ROIs of the thalamus and putamen. Acute NMO patients had significantly higher average MDs than controls in the genu of corpus callosum and optic radiation, and significantly lower average MDs in the medulla oblongata. Stable NMO patients had increased MDs in the genu of corpus callosum and optic radiation, but lower MDs in the medulla oblongata, internal capsule and thalamus. The DTI findings confirm the presence of occult tissue damage in normal-appearance white and gray matter, especially deep gray matter, in RNMO patients. This study adds further to the evidence that DTI is suitable as a tool for characterizing subtle brain tissue damage.  相似文献

【目的】运用扩散张量成像(DTI)技术,评价脑室周脑白质损伤患儿脑白质和深部灰质的扩散特性。【方法】采用配对设计方法,对25例脑室周脑白质损伤患儿和25例正常儿童进行常规MRI和DTI检查。用Functool2和DTIStudio软件处理原始图像,得到DTI图像。分析大脑、脑干不同脑白质区域及丘脑、豆状核、尾状核头部深部灰质核团感兴趣区(ROD的平均扩散率(MD)和部分各向异性值(FA)。采用配对资料t检验统计方法检验患儿组与对照组的MD和FA有无显著性差异。【结果】与对照组相比,怠儿组脑白质损伤病灶的ROI测量显示MD显著升高和FA显著下降,患儿组锥体束内囊后肢层面FA减低具有统计学意义,患儿组深部灰质核团MD和FA差异无统计学意义。【结论】不同区域运动通路的变性,在脑室周脑白质损伤患儿的病理生理学机制中发挥重要作用。  相似文献

Diffusion tensor imaging (DTI) studies in schizophrenia demonstrate lower anisotropic diffusion within white matter due either to loss of coherence of white matter fiber tracts, to changes in the number and/or density of interconnecting fiber tracts, or to changes in myelination, although methodology as well as localization of such changes differ between studies. The aim of this study is to localize and to specify further DTI abnormalities in schizophrenia by combining DTI with magnetization transfer imaging (MTI), a technique sensitive to myelin and axonal alterations in order to increase specificity of DTI findings. 21 chronic schizophrenics and 26 controls were scanned using Line-Scan-Diffusion-Imaging and T1-weighted techniques with and without a saturation pulse (MT). Diffusion information was used to normalize co-registered maps of fractional anisotropy (FA) and magnetization transfer ratio (MTR) to a study-specific template, using the multi-channel daemon algorithm, designed specifically to deal with multidirectional tensor information. Diffusion anisotropy was decreased in schizophrenia in the following brain regions: the fornix, the corpus callosum, bilaterally in the cingulum bundle, bilaterally in the superior occipito-frontal fasciculus, bilaterally in the internal capsule, in the right inferior occipito-frontal fasciculus and the left arcuate fasciculus. MTR maps demonstrated changes in the corpus callosum, fornix, right internal capsule, and the superior occipito-frontal fasciculus bilaterally; however, no changes were noted in the anterior cingulum bundle, the left internal capsule, the arcuate fasciculus, or inferior occipito-frontal fasciculus. In addition, the right posterior cingulum bundle showed MTR but not FA changes in schizophrenia. These findings suggest that, while some of the diffusion abnormalities in schizophrenia are likely due to abnormal coherence, or organization of the fiber tracts, some of these abnormalities may, in fact, be attributed to or coincide with myelin/axonal disruption.  相似文献

HIV associated dementia (HAD) is the most advanced stage of central nervous system disease caused by HIV infection. Previous studies have demonstrated that patients with HAD exhibit greater cerebral and basal ganglia atrophy than non-demented HIV+ (HND) patients. However, the extent to which white matter is affected in HAD patients compared to HND patients remains elusive. This study is designed to address the potential white matter abnormalities through the utilization of diffusion tensor imaging (DTI) in both HND and HAD patients. DTI and T1-weighted images were acquired from 18 healthy controls, 21 HND and 8 HAD patients. T1 image-based registration was performed to 1) parcellate the whole brain white matter into major white matter regions, including frontal, parietal, temporal and occipital white matter, corpus callosum and internal capsule for statistical comparisons of the mean DTI values, and 2) warp all DTI parametric images towards the common template space for voxel-based analysis. The statistical comparisons were performed with four DTI parameters including fractional anisotropy (FA), mean (MD), axial (AD), and radial (RD) diffusivities. With Whitney U tests on the mean DTI values, both HND and HAD demonstrated significant differences from the healthy control in multiple white matter regions. In addition, HAD patients exhibited significantly elevated MD and RD in the parietal white matter when compared to HND patients. In the voxel-based analysis, widespread abnormal regions were identified for both HND and HAD patients, although a much larger abnormal volume was observed in HAD patients for all four DTI parameters. Furthermore, both region of interest (ROI) based and voxel-based analyses revealed that RD was affected to a much greater extent than AD by HIV infection, which may suggest that demyelination is the prominent disease progression in white matter.  相似文献

The objective of this study was to determine the minimum change in fractional anisotropy (FA), mean diffusivity (MD), and transverse diffusivity (TD) that can be detected in a repeated diffusion tensor imaging (DTI) session with 95% confidence, i.e., the minimum detectable change (MDC). During each of three sessions, six DTI sets were collected from eight volunteers using a 3-T MR scanner, and maps of FA, MD, and TD were generated. Mean FA, MD, and TD were recorded for regions of interest placed within the corpus callosum, corticospinal tract, putamen, optic radiation, and ventricular cerebral spinal fluid. An analysis of variance was performed to calculate MDC. MDC decreased as data were averaged over scans. With three averages, MDC was lowest within the corticospinal tract and putamen, where MDC was 0.04 for FA, below 30 x 10(-6) and 40 x 10(-6) mm2/s, respectively, for MD, and below 40 x 10(-6) mm2/s for TD. No improvement was observed beyond three averages. Our results suggest that DTI can be used clinically in individual patients to detect changes in FA, MD, and TD over repeated sessions associated with neurological disease with 95% confidence, or in research to investigate changes in white matter connections in individual subjects that accompany behavioral change, such as learning.  相似文献

目的 采用DTI及基于纤维束追踪空间统计分析(TBSS)观察无先兆偏头痛患者脑白质变化。方法 对性别、年龄相匹配的25例无先兆性偏头痛患者(患者组)及25名健康志愿者(对照组)行DTI检查,采用TBSS分析 所有受试者脑白质纤维DTI各相关参数,包括FA、轴向弥散系数(AD)、平行弥散系数(RD)和平均弥散系数(MD),比较两组DTI各参数的差异。结果 与对照组比较,患者组脑白质纤维束参量值的改变以右侧半球为主,其中双侧大脑脚、双侧内囊后肢、右侧内囊前肢、胼胝体体部及压部、右侧上放射冠、右侧后放射冠、左侧扣带、右侧上纵束的FA值显著降低(P均<0.05,FWE校正);右侧大脑脚、右侧内囊后肢、右侧内囊前肢、右侧内囊豆状核后肢、胼胝体膝部、体部及压部、双侧上放射冠、双侧后放射冠、双侧丘脑后辐射、双侧扣带、双侧上纵束的RD值显著升高(P均<0.05,FWE校正);胼胝体体部及压部、右侧上放射冠、右侧后放射冠、右侧上纵束的MD值显著增高(P均<0.05,FWE校正);两组各脑区AD值差异均无统计学意义(P均>0.05,FWE校正)。结论 无先兆偏头痛患者脑白质纤维存在改变,以髓鞘脱失或完整性破坏为主。  相似文献

目的应用磁共振扩散张量成像(diffusion tensor imaging,DTI)及扩散张量纤维束成像(diffusion tensor tracking,DTT),探讨其对脑瘫患儿颈总动脉交感神经网剥脱术后脑白质恢复的评估价值。材料与方法对30例痉挛型单侧脑瘫患儿手术前、后进行临床评估和常规MRI、DTI检查,测量并比较术前术侧和对侧、术前和术后双侧内囊后肢及大脑脚的各向异性分数(fractional anisotropy,FA)值,并对白质纤维束进行三维重建。运用粗大运动功能分级系统(gross motor function classification system,GMFCS)评价患儿运动功能。采用配对样本t检验比较术前术侧和对侧、术前和术后脑白质不同部位FA值的差异,以Spearman相关分析不同白质纤维束DTI参量与GMFCS的相关性。结果术前术侧不同部位白质纤维束FA值均显著低于对侧,差异有统计学意义(P0.01)。术后6个月脑瘫患儿术侧不同部位白质纤维束FA值与术前相比显著增加(P0.01),粗大运动功能获得改善。术后6个月患儿对侧不同部位白质纤维束FA值与术前相比差异无统计学意义(P0.05)。患儿临床GMFCS与FA值呈负相关(P0.01),且与内囊后肢、大脑脚的相关性都较高(r=-0.933,P0.01;r=-0.873,P0.01)。结论 DTI联合DTT可用于定量评价脑瘫患儿术后脑白质的恢复情况,对辅助临床治疗和预后评估具有重要作用。  相似文献

Diffusion tensor imaging (DTI) has been used widely to show structural brain changes during both development and aging. Lifespan studies are valuable because they connect these two processes, yet few DTI studies have been conducted that include both children and elderly subjects. This study used DTI tractography to investigate 12 major white matter connections in 403 healthy subjects aged 5-83 years. Poisson fits were used to model changes of fractional anisotropy (FA) and mean diffusivity (MD) across the age span, and were highly significant for all tracts. FA increased during childhood and adolescence, reached a peak between 20 and 42 years of age, and then decreased. MD showed an opposite trend, decreasing first, reaching a minimum at 18-41 years, and then increasing later in life. These trajectories demonstrate rates and timing of development and degradation that vary regionally in the brain. The corpus callosum and fornix showed early reversals of development trends, while frontal-temporal connections (cingulum, uncinate, superior longitudinal) showed more prolonged maturation and delayed declines. FA changes were driven by perpendicular diffusivity, suggesting changes of myelination and/or axonal density. Tract volume changed significantly with age for most tracts, but did not greatly influence the FA and MD trajectories. This study demonstrates clear age-related microstructural changes throughout the brain white matter, and provides normative data that will be useful for studying white matter development in a variety of diseases and abnormal conditions.  相似文献

目的 采用DTI基于纤维骨架空间统计方法(TBSS)分析视神经脊髓炎谱系疾病(NMOSD)患者脑白质纤维束微结构的改变。方法 对20例NMOSD患者(NMOSD组)和20名健康志愿者(正常对照组)行全脑常规MR和DTI扫描,使用FSL软件包中TBSS步骤对DTI数据行全脑非参数统计分析,并对NMOSD组有统计学差异的纤维束FA值与临床扩展残疾状态量表(EDSS)评分进行相关性分析。结果 与正常对照组比较,NMOSD组大脑及小脑白质区纤维束如胼胝体、穹隆、皮质脊髓束、钩束、小脑脚等广泛FA值减低(P<0.05,FEW校正),其中扣带束、胼胝体、穹隆等AD值减低、RD值增高,穹隆和左侧内囊豆核后部MD值升高(P<0.05,FEW校正)。NMOSD患者左侧钩束、右侧外囊、左侧小脑下脚及双侧内囊前肢、内侧丘系FA值与EDSS评分呈负相关(P均<0.05)。结论 TBSS显示NMOSD患者大脑及小脑存在广泛脑白质损伤,可解释NMOSD部分临床症状,与残疾症状无关的脑区纤维束微结构改变提示NMOSD患者这些区域病变可能存在更复杂的病理机制。  相似文献

BACKGROUND: Mesial temporal lobe epilepsy (mTLE) with hippocampus sclerosis (HS) is an important cause for focal epilepsy. In this study, we explored the integrity of connecting networks using diffusion tensor imaging (DTI) and two whole-brain voxel-based methods: statistical parametric mapping (SPM) and tract-based spatial statistics (TBSS). METHODS: Thirty-three consecutive patients with mTLE and HS undergoing presurgical evaluation were scanned at 3 T, a DTI data set was acquired and parametric maps of fractional anisotropy (FA) and mean diffusivity (MD) were calculated. Twenty-one patients had left hippocampal sclerosis (LHS) and 12 patients had right HS (RHS). These groups were compared to 37 normal control subjects using both SPM5 and TBSS. RESULTS: The ipsilateral temporal lobe showed widespread FA reduction in both groups. The limbic system was clearly abnormal in the LHS group, also involving the arcuate fasciculus. In RHS, changes were more restricted but also showed involvement of the contralateral temporal and inferior frontal lobe. Increased MD was found in the ipsilateral hippocampus by SPM that was only marginally detected by TBSS. In white matter regions, however, TBSS was more sensitive to changes than SPM. CONCLUSION: DTI detects extensive changes in mTLE with HS. The affected networks were principally in the ipsilateral temporal lobe and the limbic system but also the arcuate fasciculus. SPM and TBSS gave complementary information with higher sensitivity to FA changes using TBSS.  相似文献

Using diffusion tensor MR imaging (DTI) and advanced voxel-wise analysis tools, we study diffusivity and anisotropy changes of white matter from late childhood to young adulthood, and correlate quantitative diffusion indices with Chinese and English reading performance scores. Seventy-five normal healthy school going ethnic Chinese students and young adults of three age groups were recruited (group 1, n=24, mean+/-SD=7.4+/-0.3 years; group 2, n=27, mean+/-SD=10.3+/-0.5 years; group 3, n=24, mean+/-SD=22.8+/-2.3 years). DTI was performed with 3 mm isotropic resolution to cover the entire brain. Voxel-wise analysis was performed using Tract-Based Spatial Statistics (TBSS) to localize regions of white matter showing significant changes of fractional anisotropy (FA), mean diffusivity (MD), and axial and radial diffusivities between groups. We found increased FA and decreased MD with increasing age in regions of cerebellar white matter, right temporal white matter, and a large portion of the superior frontal and parietal white matter driven by both the reduction of radial diffusivity and axial diffusivity with the former to a greater extent. Changes were continual from late childhood to young adulthood. Findings were confirmed by region-of-interest analysis in specific white matter tracts. After controlling for the effect of age, significant correlation was found between diffusion indices of the anterior limb of the left internal capsule and Chinese reading score (p=0.05), and of the corona radiata and English reading score (p=0.026 and p=0.029 for left and right, respectively).These DTI indices likely reflect the multiple biological processes that occur during brain development which provide the neural substrate for ongoing functional connectivity including for reading development.  相似文献

目的 应用基于纤维束示踪的空间统计分析(TBSS)方法,观察先天性感音神经性耳聋患者听觉中枢及相关中枢的脑白质改变.方法 对15例先天性耳聋患者和28名正常志愿者的听觉中枢及相关中枢进行扩散张量成像(DTI)及三维快速扰相梯度回波数据采集.应用TBSS对先天性耳聋患者和正常志愿者的DTI数据进行分析,获取部分各向异性(FA)值具有显著性差异的脑区.结果 与正常志愿者相比,先天性耳聋患者的双侧额下回、颞横回、颞上回、颞中回中后部内侧的脑白质以及双侧内囊前肢、外囊、视辐射FA值显著下降(P<0.05),左侧颞中回中后部内侧脑白质及视辐射FA值下降的范围大于右侧.结论 先天性耳聋患者听觉中枢及相关中枢脑白质FA值显著下降,提示出入听觉中枢及相关中枢的脑白质纤维髓鞘形成不良和(或)白质纤维数量减少.  相似文献

目的 采用基于纤维束的空间统计方法(TBSS)探讨原发性三叉神经痛(PTN)患者脑白质微结构异常。方法 对38例PTN患者(PTN组)和38名年龄、性别匹配的健康志愿者(对照组)行全脑DTI扫描,并用FSL软件进行分析,寻找FA值、平均扩散系数(MD)、径向扩散系数(RD)和轴向扩散系数(AD)存在组间统计学差异的脑区,并与视觉模拟评分(VAS)进行相关分析。结果 与对照组相比,PTN组胼胝体、放射冠、左侧上纵束、左侧下纵束及下额枕束FA值降低;且PTN组双侧丘脑后辐射、双侧内囊、双侧外囊、穹窿、右侧上纵束同时存在MD值和RD值升高,而AD升高脑区仅局限于双侧内囊前支、左侧内囊后支、双侧上放射冠和双侧上纵束。PTN患者左侧上纵束FA值与VAS呈负相关(r=-0.502,P=0.001),左侧上纵束MD值与VAS评分呈正相关(r=0.437,P=0.006)。结论 丘脑、内囊MD值和RD值同向升高而不伴FA值降低可能是PTN的相对特征性DTI表现;左侧上纵束FA值和MD值对评估PTN患者疼痛水平有参考价值。  相似文献

目的采用DTI技术,定量检测肌萎缩侧索硬化症(ALS)患者脑内上运动神经元(UMN)的受累情况。方法运用GE Signa3.0T磁共振成像系统,对15例临床确诊及拟诊的ALS患者和12例性别、年龄相当的正常对照者进行了全脑DTI扫描。分别计算双侧大脑皮层下白质、皮质脊髓束及胼胝体各部的各向异性分数(FA)和平均扩散系数(MD),并与多种临床参数进行相关分析。结果ALS患者脑内的FA值在双侧中央前后回皮层下白质、放射冠、内囊后肢后部、大脑脚及胼胝体各部较正常对照者显著下降;而相应部位的MD值无显著变化。FA值与ALSFRS及最大手指敲击率呈正相关,与病程、年龄及病情进展速度呈负相关;MD值与ALSFRS、病情进展速度及年龄呈负相关,而与其他临床指标之间无显著相关性。结论DTI可以通过定量检测ALS患者脑内FA及MD值的变化帮助确定上运动神经元的受累情况,尤其是在临床诊断不是很明确时。  相似文献

目的 探讨终末期肾病(ESRD)患者脑白质结构异常与认知损伤的相关性。方法 对26例ESRD患者(ESRD组)和25名健康志愿者(HC组)进行蒙特利尔认知量表(MoCA)评估,并比较两组受试者整体认知评分和子项评分的差异;采用基于DTI的纤维束骨架的空间统计分析(TBSS)方法获得各向异性分数(FA)、平均扩散系数(MD)、轴向扩散系数(AD)及平行扩散系数(RD);将ESRD组MoCA得分与有差异脑区FA值进行基于体素的Spearman相关分析。结果 ESRD组与HC组比较,MoCA总分和MoCA子项包括视觉空间与执行能力、语言和延迟回忆差异有统计学意义(P均<0.05);ESRD组双侧下额枕束、辐射冠、上纵束、穹窿、丘脑后辐射、大脑脚,右侧内囊及外囊,脑桥,胼胝体膝部和体部FA值减低(P均<0.05);ESRD组双侧大脑白质对称MD、AD、RD升高(P均<0.05)。ESRD组右侧外囊FA值与MOCA得分呈正相关(r=0.61,P<0.05)。结论 终末期肾病存在包括记忆、执行力等领域认知损伤,且与脑白质纤维束异常存在一定相关性;右侧外囊区的FA值减低与认知损伤密切相关,可为ESRD患者在血液透析过程中脑结构-功能损伤研究提供新的诊断学标记。  相似文献

Magnetic resonance diffusion tensor imaging (DTI) enables the discrimination of white matter pathways before myelination is evident histologically or on conventional MRI. In this investigation, 14 premature neonates with no evidence of white matter abnormalities by conventional MRI were studied with DTI. A custom MR-compatible incubator with a novel high sensitivity neonatal head coil and improved acquisition and processing techniques were employed to increase image quality and spatial resolution. The technical improvements enabled tract-specific quantitative characterization of maturing white matter, including several association tracts and subcortical projection tracts not previously investigated in neonates by MR. Significant differences were identified between white matter pathways, with earlier maturing commissural tracts of the corpus callosum, and deep projection tracts of the cerebral peduncle and internal capsule exhibiting lower mean diffusivity (Dav) and higher fractional anisotropy (FA) than later maturing subcortical projection and association pathways. Maturational changes in white matter tracts included reductions in Dav and increases in FA with age due primarily to decreases in the two minor diffusion eigenvalues (lambda2 and lambda3). This work contributes to the understanding of normal white matter development in the preterm neonatal brain, an important step toward the use of DTI for the improved evaluation and treatment of white matter injury of prematurity.  相似文献

目的 探讨弥散张量成像(DTI)中不同数目的梯度方向对成人脑组织各向异性分数(FA)的离散程度及其信噪比(SNR)的影响.方法 应用7种不同弥散梯度方向数目(NDGD)对8名健康志愿者脑组织进行DTI扫描并获取FA图,计算FA图中4个感兴趣区(ROI)(胼胝体膝部、胼胝体压部、内囊膝部、内囊后肢)的FA值及其SNR,分析其随NDGD增加的变化趋势.结果 脑内各个ROI的FA值不随NDGD的增加而显著变化;信号强的ROI(胼胝体膝部、胼胝体压部)内FA值方差随机波动,其SNR增加不大;信号弱的ROI(内囊膝部、内囊后肢)内FA值方差随着NDGD的增加而逐渐减小.结论 在DTI的临床应用中,要根据检查的部位选择最适宜的NDGD.  相似文献

目的 探讨基于纤维束骨架的空间统计方法研究维持性透析终末期肾病(ESRD)患者脑白质微结构变化。方法 采用3.0T MR仪对32例行维持性血透的ESRD患者和30名健康对照行DTI扫描,应用基于纤维束骨架的空间统计(TBSS)方法分析DTI多参数图,获得部分各向异性分数(FA)、平均弥散度(MD)、轴向弥散度(AD)及径向弥散度(RD)有显著差异的脑区。将ESRD患者差异脑区FA值与简易精神状态测试(MMSE)评分进行偏相关分析。结果 与健康对照比较,ESRD患者双侧额叶白质、双侧前辐射冠、胼胝体体部及膝部、左侧颞叶白质及多条联络纤维FA值显著减低;MD值呈广泛且对称性显著升高;右侧上纵束、左侧上辐射冠AD值显著升高,左侧丘脑前辐射AD值显著降低;双侧额叶、胼胝体及双侧上纵束等白质区RD值显著升高(P均<0.05)。右侧额叶白质(r=0.404,P=0.022)及左侧上纵束(r=0.475,P=0.006)FA值与MMSE评分呈正相关。结论 ESRD患者脑区白质微结构受损,可能导致其认知障碍。  相似文献

目的 应用扩散张量成像技术测量偏瘫患者内囊后肢的各向异性(FA)值,分析皮质脊髓束完整性与患侧上肢运动功能的关系.方法 选择17例亚急性期脑卒中后偏瘫患者,测量患侧及健侧内囊后肢FA值,并计算FA非对称比值,临床检测包括上肢运动功能指标(WMFT)及运动功能、平衡、感觉及关节功能综合评价指标(FM),利用线性相关分析FA非对称比值与患手FM 评分、WMFT 非对称比值及log WMFT之间的相关关系.结果 患手运动功能评分与内囊后肢FA非对称比值显著相关,FA非对称性比值与患手FM(R~2=0.655,P<0.002)呈线性负相关,与WMFT非对称性比值(R~2=0.649,P<0.002)、lg WMFT(R~2=0.636,P=0.002)以及握力非对称性比值(R~2=0.414,P=0.02)均呈线性正相关.结论 偏瘫患者亚急性期皮质脊髓束完整性与上肢运动功能显著相关;利用扩散张量成像技术可早期监测皮质脊髓束的完整性.  相似文献
