目的 了解我院同时产质粒介导AmpC酶和超广谱 β 内酰胺酶 (ESBLs)肺炎克雷伯菌与大肠埃希菌的耐药性及其 β 内酰胺酶的基因型特征。方法  110株临床分离无重复肺炎克雷伯菌与大肠埃希菌耐药株 ,采用酶提取物三维实验检测AmpC酶 ,美国临床实验室标准委员会 (NCCLS)表型筛选和确认实验检测ESBLs ;琼脂二倍稀释法测定抗生素对同时产AmpC酶和ESBLs菌株的最低抑菌浓度 (MICs) ;等电聚焦实验测定 β 内酰胺酶等电点 (pIs) ;质粒接合实验定位耐药基因 ;PCR通用引物扩增AmpC酶与ESBLs基因及其序列测定以确定其基因亚型。结果 同时产AmpC酶和ESBLs菌株在肺炎克雷伯菌与大肠埃希菌中的检出率分别为 7.7% (5 6 5 )、8.9% (4 4 5 )。该类产酶菌株产 2~ 3种pI5 .4~ 9.0 β 内酰胺酶 ,对第三代头孢菌素、氨曲南、头孢美唑和含酶抑制剂复合制剂的敏感性极低 ,但对亚胺培南均敏感。 9株质粒中均检出AmpC酶基因DHA 1亚型 ,其中 1株同时检出ACT 1亚型 ;ESBLs基因亚型分别为CTX M 14、CTX M 3、CTX M 9,各有 4、3、2株 ;有 3株还携带广谱酶TEM 1基因。结论 我院存在同时产质粒介导DHA 1、ACT 1型AmpC酶和CTX M型ESBLs肺炎克雷伯菌、大肠埃希菌流行株 ,药敏检测结果显示对大多数新型广谱 β 内酰胺类抗生  相似文献

目的:检测本院临床产超广谱β-内酰胺酶(ESBLs)革兰阴性菌的TEM、SHV、CTX-M基因型特征。方法:双纸片法确定产ESBLs的临床分离菌,提取质粒DNA,PCR法扩增ESBLs的TEM、SHV、CTX-M基因片段,对临床分离的12株肺炎克雷伯菌,17株大肠埃希氏菌和16株铜绿假单胞菌,共45株革兰阴性菌进行TEM、SHV、CTX-M基因分型研究。结果:本院临床分离出的45株产ESBLs临床分离菌的质粒中,有36株扩增出TEM基因片段,33株扩增出SHV基因片段,31株扩增出CTX-M基因片段,TEM、SHV、CTX-M基因片段的检出率分别为80%、74%和68%。结论:TEM、SHV、CTX-M已成为本院临床分离产ESBLs菌的主要耐药基因型,有必要进一步加强临床耐药基因的监测,为临床用药和医院感染监控提供参考。  相似文献

目的研究大肠埃希菌和肺炎克雷伯菌同时产超广谱β内酰胺酶(ESBL)及质粒型Ampc酶菌株的比率及其基因型。方法收集北京两家教学医院2001--2002年产ESBL且对头孢西叮耐药的59株大肠埃希菌和21株肺炎克雷伯菌,采用等电聚焦电泳测定β内酰胺酶的等电点;接合试验证实酶基因有无可转移性,并用碱裂解法提取质粒;采用多重聚合酶链式反应(multiplex polymerase chain reaction,MPCR)及序列分析确定质粒AmpC酶的基因型。脉冲场凝胶电泳(pulsed field gelel ectrophoresis,PFGE)确定耐药菌株的亲缘关系。结果北京两家医院ESBL中质粒型AmCZ酶的发生率,大肠埃希菌分别为0和2%,肺炎克雷伯菌则分别为9.7%和17.1%。1株大肠埃希菌及9株肺炎克雷伯菌产生DHA-1型质粒AmpC酶,同时也产CTX-3型(6株)或CTX—M-14(1株)或SHV-12(3株)型ESBL。10株中,3株肺炎克雷伯菌可将头孢西叮耐药性传给受体菌。这10株菌均至少携带1个约33—36kb的大质粒,未发现质粒的传播。PFGE发现这10株菌来自不同的克隆株。结论北京地区发现同时产DHA-1质粒AmpC型及CTX—M型ESBL的肺炎克雷伯菌,它们来自不同的克隆。  相似文献

目的研究产生超广谱β-内酰胺酶(ESBLs)的肺炎克雷伯菌基因型及其药敏特征。方法对2003年12月~2004年12月哈尔滨地区临床分离的74株产ESBLs肺炎克雷伯菌,测定菌株对常用β-内酰胺类和非β-内酰胺类抗菌药物的敏感性,并采用SHV型和TEM型特异性引物检测ESBLs的基因型。结果产ESBLs细菌对头孢吡肟、氨曲南、阿米卡星和环丙沙星的敏感率依次为71.6%、20.3%、54.1%、31.1%,对头孢哌酮/舒巴坦的敏感率为54.1%,69株(93.2%)对亚胺培南敏感。74株产ESBLs细菌中携带blaSHV和6kTEM基因的阳性例数分别是45例(60.8%)和23例(31.1%)。结论哈尔滨地区产ESBLs肺炎克雷伯菌对常用β-内酰胺类和非β-内酰胺类抗菌药物的敏感性降低,临床上需慎用酶抑制剂复合物及第4代头孢菌素,亚胺培南仍是治疗产ESBLs菌感染的首选药物。产ESBLs肺炎克雷伯菌blaSHV的流行频率高于blaTEM.  相似文献

目的对阴沟肠杆菌所产CTX-M型β-内酰胺酶进行基因克隆、原核表达重组质粒的构建及其特性研究。方法以产CTX-M型β-内酰胺酶的阴沟肠杆菌45号基因组DNA为模板,设计两对引物PCR扩增CTX-M、将其克隆入pGEM-T载体后测定该核苷酸序列,再将CTX-M基因克隆到pGEX-6P-3表达质粒并转入感受态BL21.选择阳性菌落进行药物敏感试验和酶动力学检测。结果PCR扩增出大小为876bp和1067bp的基因片段,与GenBank上CTX—M-9和CTX—M-3酶的基因序列同源性为100%。CTX-M-3型重组菌株药敏试验结果与原菌株相比,对青霉素类和头孢噻肟具有明显的水解作用,第三、四代头孢菌素对之较为稳定,动力学结果与酶稳定性结果基本一致。结论重组质粒pGEX-6P-3/CTX—M构建成功,CTX—M-3型重组菌株与原菌株比较,对β-内酰胺类抗生素的敏感性和稳定性增加,亲和力下降。  相似文献

目的了解革兰阴性菌质粒介导qmA耐药基因的发生率、分子遗传学背景及其阳性株的耐药谱。方法收集2004年4月-2006年4月对萘啶酸耐药的临床分离无重复株共629株,采用特异引物PCR结合测序进行qnrA阳性株的识别,表型确认试验结合PCR检测识别产ESBL或AmpC酶的qnrA阳性株,Kirby-Bauer法和Etest法进行qnrA阳性株的药敏检测,质粒接合转移及Southem杂交检测进行qmA基因的质粒定位,PCR策略克隆携带qnrA基因整合子基因结构并进行引物步移测序。结果qnrA阳性株的总检出率为1.9%(12/629)。菌种分布为肺炎克雷伯菌2.2%(3/138),阴沟肠杆菌17.1%(6/35),产气肠杆菌9.1%(1/11),枸橼酸杆菌属12.5%(1/8),沙门菌属14.3%(1/7)。qnrA基因定位在80~180kb大小质粒上的su/1型Ⅰ类整合子基因结构中。其中4株菌qnrA基因定位在整合子In37上,另外8株菌qnrA基因定位在一种新型的整合子InX上。所有qnrA阳性株均产ESBL,并具有可转移多重耐药的特征。结论广东地区喹诺酮抗菌药耐药株中存在着质粒介导的耐药机制,但发生率较低;其耐药基因qnrA的水平传播能力有可能导致细菌耐药性的播散。  相似文献

目的探讨重组质粒pET一28a(+)/CTX—M一3分别在大肠埃希菌ER2566和BL21(DE3)中进行原核表达的最佳条件。方法分别在不同的宿主菌、诱导温度、诱导时间和诱导剂浓度中诱导pET.28a(+)/CTX—M一3融合表达载体,目的产物经SDS—PAGE和BandScan凝胶分析软件分析,以获得最佳表达条件。结果表达载体的最佳诱导条件是重组质粒在BE21(DE3)中18℃诱导24h,IPTG终浓度为0.8mmol/L;表达载体在最佳条件下表达时,目的蛋白的最高表达量占菌体总蛋白的33%。结论获得pET一28a(+)/CTX—M一3在大肠埃希菌中表达CTX—M一3型超广谱β一内酰胺酶(extended—spectrumB—lactamase,ESBLs)蛋白的最佳条件,为此酶的大量纯化奠定了基础。  相似文献

目的对产超广谱β-内酰胺酶(ESBLs)的大肠埃希菌和肺炎克雷伯菌携带的ESBLs耐药基因进行分型,为临床合理应用抗生素提供理论依据。方法收集住院患者标本中分离出的51株大肠埃希菌和32株肺炎克雷伯菌,经PCR对上述菌株所携带的ESBLs耐药基因进行分型。结果产ESBLs大肠埃希菌中耐药质粒编码TEM型、SHV型和非TEM非SHV型超广谱β-内酰胺酶基因的百分率依次为80.4%,7.8%和11.8%;而在肺炎克雷伯菌中的百分率依次为78.1%,71.9%和25.0%。多数产ESBLs肺炎克雷伯菌同时产生一种以上的β-内酰胺酶。结论获得了吉林地区大肠埃希菌和肺炎克雷伯菌ESBLs的不同基因型,ESBLs基因型具有地区性差异。  相似文献

目的:研究我院产头孢菌素酶(AmpC酶)阴沟肠杆菌的检出率、耐药情况厦ampC基因型。方法:收集临床分离的耐药阴沟肠杆菌15株;三维试验检测AmpC酶;NCCLS方法检测超广谱β-内酰胺酶(ESBLs);琼脂二倍稀释法测定MIC值;PCR扩增检测ampC基因及序列测定。结果:15株菌中8株菌(53,3%)产AmpC酶,3株菌(20.0%)产ESBLs。产AmpC酶的菌株除对亚胺培南全敏感外,对其它抗菌药不同程度耐药。5株菌的ampC基因与阴沟肠杆菌ECLC074的ampC基因100%同源,3株菌与之99%同源.2株菌的AmpC酶发生了1个氨基酸残基的改变。结论:产AmpC酶是阴沟肠杆菌对口.内酰胺类抗生素耐药的主要机制之一。阴沟肠杆菌ECLC074 ampC基因是我院主要的阴沟肠杆菌ampC基因型。产AmpC酶的阴沟肠杆菌常呈多重耐药,亚胺培南是治疗此类菌所致感染的最有效药物。  相似文献

A family is reported in which two sibs, one male and the other female, both died within 24 hours of birth with enlarged polycystic kidneys. Postmortem histology in the second child showed gross renal dysplasia. In both children the pancreas was enlarged, nodular and cystic but the liver appeared macroscopically normal. In the second child, histological examination confirmed pancreatic fibrosis with cystic dilation of ducts, but showed portal fibrosis with bile duct proliferation in the liver.
This combination of findings is very reminiscent of those in a girl and her brother reported by Ivemark et al. (1959). The children reported here also showed absence or hypoplasia of the spleen, cardiac anomalies and other features of the Ivemark syndrome (Ivemark 1955), a quite different, usually sporadic, congenital disorder. It is suggested that the children described here have a distinct lethal congenital disorder, probably inherited in an autosomal recessive manner.  相似文献

Over 200 schizophrenic patients belonging to three major and interrelated pedigree complexes have been investigated over the past 30 years in a North Swedish geographically isolated population, presently numbering about 6,000. An intensive investigation of a number of biochemical correlates and genetic markers in a few selected families belonging to one of the major pedigrees has indicated new strategies for the current research program.
Schizophrenia, as defined operationally, is significantly associated with decreased activities of two enzymes (1) blood platelet monoamine oxidase, (2) plasma dopamine-β-hydroxylase, and (3) with the genetic marker Gc2 (group specific antigen). Both enzymes are subject to genetic variation. A positive score for linkage between schizophrenia and low plasma DBH activity has been calculated, but, so far, available data are insufficient for discrimination between linkage and partial contribution of genetically controlled low plasma DBH to the pathogenesis of the disease. Alternatively, both mechanisms could be involved.
As a model for continued research, schizophrenia is explained as based on a double dominant-recessive genotype (Aabb), representing a vulnerability which in about 50 % of cases develops into clinical schizophrenia. It is suggested that the dominant mutation (A) operates on or affects MAO activity, and that the recessive genotype (bb) is instrumental in low variates of DBH activity and very likely such variates within the normal range of physiological variation. Moreover, it is suggested that the combined effects of MAO- and DBH-reduced efficiency on the metabolism of e.g. dopamine could be an essential pathogenic mechanism for the schizophrenic illness which is segregating in this population.  相似文献

There are an estimated over 200 million yearly cases of malaria worldwide. Despite concerted international effort to combat the disease, it still causes approximately half a million deaths every year, the majority of which are young children with Plasmodium falciparum infection in sub-Saharan Africa. Successes are largely attributed to malaria prevention strategies, such as insecticide-treated mosquito nets and indoor spraying, as well as improved access to existing treatments. One important hurdle to new approaches for the treatment and prevention of malaria is our limited understanding of the biology of Plasmodium infection and its complex interaction with the immune system of its human host. Therefore, the elimination of malaria in Africa not only relies on existing tools to reduce malaria burden, but also requires fundamental research to develop innovative approaches. Here, we summarize our discoveries from investigations of ethnic groups of West Africa who have different susceptibility to malaria.  相似文献

About 1900, modern food selection and processing caused widespread epidemics of the B vitamin deficiency diseases of beriberi and pellagra which, for genetic reasons, often expressed as different diseases ranging from bowel and heart disease to dermatoses and psychoses. But the B vitamins merely help convert essential fatty acids (EFA) into the prostaglandin (PG) tissue regulators and it now turns out that, through hydrogenation, milling and selection of w3-poor southern foods, we have also been systematically depleting, by as much as 90%, a newly discovered trace Nordic EFA (w3) of special importance to primates and sole precursor of the PG3(4) series, even as a concurrent fiber deficiency increases body demand for EFA. Since substrate EFA is processed by many B vitamin catalysts, an EFA deficiency will mimic a panhypovitaminosis B, i.e., a mixture of substrate beriberi and substrate pellagra resembling vitamin beriberi and pellagra but exhibiting as even more diverse endemic disease. This would consitute a second stage of the Modern Malnutrition and explain why some workers now hold the dominant diseases of modermized societies to be new, nutritionally based, pellagraform yet lipid-related and to range, once again, from heart disease to psychosis. It is an assumption that our dominant diseases are unrelated to each other or are merely revealed by our diagnostic acumen and therapeutic success; and that hydrogenating millions of tons of food oils annually, to destroy the rancidity producing w3-EFA, is safe for primates. Extensive beriberiform disease is reported here in 32 typical cases taken from medical practice which responds strikingly to linseed oil supplements (60% w3-EFA) in confirmation of identical results in Capuchins.  相似文献

Zusammenfassung Eine Reihe pathologischer Zustände bedingen Magnesiummangel. Zustände mit Hypermagnesämie sind ebenfalls bekannt, doch wesentlich seltener. Für den Kardiologen beachtenswert ist, daß unter Therapie mit bestimmten Diuretica bei Herzinsuffizienz, bei Herzinfarkt, Kardiomyopathie, Digitalisintoxikation und bestimmten Herzrhythmusstörungen Hypomagnesämie beobachtet wurde. Leider kann in der klinischen Routine nur ein extracelluläres Magnesiumdefizit durch Serumbestimmungen gemessen werden; über Magnesiummangel einzelner Organe kann nichts ausgesagt werden. Hinweise für Magnesiummangel geben aber neben der Messung des Serumspiegels Anamnese, klinischer Befund, bestimmte EKG-Veränderungen wie auch evtl. Hypokalämie, ein Zustand, bei dem sich oft — besonders bei Aldosteronismus — parallele Veränderungen zeigten.Tierexperimente deuten darauf hin, daß infarktähnliche Läsionen unter Magnesiummangel entstehen, doch ob Herzinfarkt beim Menschen durch Magnesiummangel ausgelöst werden kann, ist noch ungeklärt. In Leichenherzen zeigte sich im Infarktgebiet neben Calciumakkumulation signifikanter Magnesiumverlust, wobei unklar blieb, ob sich Ursache oder Folge des Infarktes widerspiegelten. Falls ein ursächlicher Zusammenhang besteht, ist er im Myokardstoffwechsel selbst zu suchen, wie bei der Alkoholkardiomyopathie, wo myokardialer Magnesiummangel zumindest als pathogenetischer Teilfaktor anerkannt wird. Andererseits versucht man aber auch Beziehungen zwischen Atherosklerose, Blutgerinnung und Hypomagnesämie herzustellen, in der Meinung, daß Magnesiummangel auch über den coronaren Pathomechanismus des Herzinfarktes wirken könnte. Sicher scheint, daß gewisse EKG-Veränderungen und Herzrhythmusstörungen durch einen irritierten Magnesiumhaushalt bedingt sein können, da sie bei Gabe bzw. Entzug von Magnesium verschwinden. Daß Magnesiummangel die Glykosidtoleranz verringert, wird tierexperimentell bestätigt. Unter Hypomagnesämie bewirkt Acetylstrophanthidin eher und länger Rhythmusstörungen als ohne, außerdem lassen diese sich durch Magnesiumgaben eliminieren. Da in gewissen Fällen spontane und digitalisinduzierte Herzrythmusstörungen durch Magnesiuminjektionen beseitigt wurden, scheint Magnesium als Therapeuticum angebracht. Einsatz verschiedener Magnesiumsalze bei Angina pectoris, degenerativen Herzerkrankungen und Herzinsuffizienz ohne geprüften und offensichtlich gestörten Magnesiumhaushalt ist fragwürdig, weil keine eindeutigen klinischen Erfolgsbeweise vorliegen. Immerhin mag es aber larvierte, durch Serumbestimmungen nicht erfaßbare Mangelzustände geben. Allgemein erscheint es aus kardiologischer Sicht ratsam, den Magnesiumhaushalt zu überwachen und in entsprechenden Fällen auszugleichen, um möglichen Myokardläsionen oder fatalen Herzrhythmusstörungen entgegenzuwirken.  相似文献

Introduction: The etiology of atopic dermatitis (AD) is multifactorial with interaction between genetics, immune and environmental factors.

Areas covered: We review the role of prenatal exposures, irritants and pruritogens, pathogens, climate factors, including temperature, humidity, ultraviolet radiation, outdoor and indoor air pollutants, tobacco smoke exposure, water hardness, urban vs. rural living, diet, breastfeeding, probiotics and prebiotics on AD.

Expert commentary: The increased global prevalence of AD cannot be attributed to genetics alone, suggesting that evolving environmental exposures may trigger and/or flare disease in predisposed individuals. There is a complex interplay between different environmental factors, including individual use of personal care products and exposure to climate, pollution, food and other exogenous factors. Understanding these complex risk factors is crucial to developing targeted interventions to prevent the disease in millions. Moreover, patients require counseling on optimal regimens for minimization of exposure to irritants and pruritogens and other harmful exposures.  相似文献

Georgia (or Sakartvelo in its own language) is a South Caucasus Mts. country with its easternmost part is enigmatically named Iberia, like the Iberian Peninsula, which may refer to rivers “Kura” and “Ebro” or their valleys respectively. Most of their inhabitants speak Georgian which is included within Dene-Caucasian group and Usko-Mediterranean subgroup of languages. The latter includes Basque, Berber, ancient Iberian-Tartessian, Etruscan, Hittite, Minoan Lineal A and others. In the present paper, HLA class II -DRB1 and -DQB1 alleles has been studied and extended haplotypes calculated. Most frequent haplotypes are also of Mediterranean origin (i. e.: (A*02-B*51)-DRB1*11:01-DQB1*03:01, (A*02-B*51)-DRB1*13:01-DQB1*06:03, or (A*24-B*35)-DRB1*01:01-DQB1*05:01) and DA genetic distances show that closest world populations to Georgians are Mediterraneans. Georgians also show common extended haplotypes ((A*02-B*51)-DRB1*11:01-DQB1*03:01, (A*02-B*13)-DRB1*07:01-DQB1*02:01 and (A*03-B*35)-DRB1*11:01-DQB1*03:01) with Svan people, a secluded population in North Georgia mountains. We can conclude that Georgians belong to a very old Mediterranean substratum according to both linguistics (Usko Mediterranean languages) and HLA genetics.  相似文献

Sick children were ubiquitous in early modern England, and yet they have received very little attention from historians. Taking the elusive perspective of the child, this article explores the physical, emotional, and spiritual experience of illness in England between approximately 1580 and 1720. What was it like being ill and suffering pain? How did the young respond emotionally to the anticipation of death? It is argued that children’s experiences were characterised by profound ambivalence: illness could be terrifying and distressing, but also a source of emotional and spiritual fulfilment and joy. This interpretation challenges the common assumption amongst medical historians that the experiences of early modern patients were utterly miserable. It also sheds light on children’s emotional feelings for their parents, a subject often overlooked in the historiography of childhood. The primary sources used in this article include diaries, autobiographies, letters, the biographies of pious children, printed possession cases, doctors’ casebooks, and theological treatises concerning the afterlife.  相似文献
