急诊工作的"预防为主"以及"濒危意识"和"濒危状态"

危险性急症的发现、诊断和抢救，是全部急诊工作中最重要、也是最困难的工作。如果一个急诊医生已经具有了很强的“危机意识”、掌握了急诊工作的全部思维特点，因此具备了很强的“三快”能力，从而能够迅速地发现并且抢救危险性急症，那么是否可以认为他是一个优秀的急诊医生呢？

"游泳"与"抚触"对新生儿健康的临床研究

目的探讨"游泳"与"抚触"活动对新生儿健康的影响.方法将95例正常新生儿按分娩顺序分为观察组("游泳"与"抚触")48例;对照组(单纯沐浴)47例.统计对比2组新生儿的出生体重、出生后第10天体重、出生后第42天体重、生理性黄疸开始消退时间、生理性黄疸完全消退时间.结果观察组新生儿第10天平均体重3 500g,第42天平均体重4 560 g,对照组新生儿第10天平均体重3 300 g,第42天平均体重4 080 g.观察组生理性黄疸开始消退时间(5.5±1.3)天,生理性黄疸完全消退时间(11.87±1.2)天.对照组生理性黄疸开始消退时间(6.3±1.1)天,生理性黄疸完全消退时间(13.67±1.7)天.结论新生儿"游泳"与"抚触"有助于新生儿身心的健康发育.

实验诊断学是临床医学生必修课程之一,名词解释即定义在实验诊断学课程中是最基本的学习内容,正确理解和掌握定义的本质和外延至关重要,不仅影响现阶段的学习,更直接影响其后续课程的学习,其教学效果甚至可能涉及学生毕业以后的职业生涯,因此对医药院校从事实验诊断学教学的教师提出了更高的要求.     

"Footprints on the sands of time"

The author's reflections on Becky's legacy (see Werth, this issue) revolve around the idea that she can serve as a role model and that Becky's and Jim's experiences provide guidance to others who will inevitably go through the dying process. She also highlights several themes she saw in Werth's article: a cultural revolution around dying, the individuality of the dying experience, the need to expand the framework of care for the dying, and access to care and limits on access. She concludes by focusing on the need for communication between the dying person and loved ones.     

"Hitting the nail on the head"

Subungual malignant melanoma is a rare form of malignancy that can present at an advanced stage. We describe a case that was diagnosed after a presentation to the emergency department for a traumatic injury of the affected area. Initial presentations of malignant disease and its complications form a relatively low proportion of the caseload of emergency doctors. In this case, a patient presented after a minor injury that had failed to recover; subsequent investigation of this injury led to the diagnosis of subungual malignant melanoma in an otherwise asymptomatic patient. This is a rare case which presented in an atypical fashion, with a confounding history of minor trauma. It is presented to highlight the differential diagnosis of destructive bone lesions.     

Reflections on "the last kiss"

The sensuous, spiritual musings of a man nearing death have been recounted in an essay entitled "The Last Kiss" which is published as an occasional paper in this issue of the Journal. This commentary on that essay examines some near-death experiences for causation and content and shows the value of diversity of training and experience in hospice and palliative care teams.
